Delivery room emergencies due to birth injuries are serious, usually unexpected, and can be distressing situations that necessitate immediate action to reduce neonatal morbidity and prevent neonatal mortality. Birth injuries requiring immediate, urgent care in the delivery room are uncommon, hence knowledge of obstetric risk factors and prenatal conditions linked to birth injury is an important first step in the management of affected neonates. Furthermore, immediate recognition of injury and quick action upon delivery is essential in order to achieve the best possible outcomes. This chapter b

The approach to the management of meconium-stained newborns in the delivery room has been changing for over 40 years. The goal is to prevent meconium aspiration syndrome (MAS) and complications related to MAS. For decades, airway obstruction was believed to be a major component of MAS and, consequently, suction maneuvers to remove meconium from the airways were recommended to decrease the frequency and severity of MAS. Initial recommendations were based on observational studies. However, the incidence of MAS and mortality related to MAS has declined since the 1970s, mostly because of a decreas

The aim of neonatal resuscitation is to establish an effective ventilation, avoiding lung damage. PEEP and T-piece use, as well as Sustained Lung Inflation, lower O2 target and appropriately timed surfactant administration analyzed. A new "gentle" respiratory approach in delivery room can improve newborn respiratory outcome.

BACKGROUND: Gastroschisis is a congenital abdominal wall defect with the small and large bowel protruding through. Early closure prevents heat and water loss, infection, and bowel edema. Immediate primary fascial closure should be done when possible.
OBJECTIVE: To compare the outcome of a group of gastroschisis neonates diagnosed before birth who underwent delivery and immediate surgical repair in the operating room (IOR group) with another group who underwent delivery outside the operating room and urgent surgical repair in the operating room later (OOR group).
MATERIAL AND METHOD: A retros

The need for resuscitation of a distressed newborn in delivery room is more and more easily predictable. The two principal reasons are improvement of obstetrical survey and best perinatal regionalisation. Perinatal asphyxia and premature labour, especially before 32 weeks of gestational age, are the more frequent situation needing resuscitation at birth. A good survey of pregnancy and labor, verification of availability and efficiency of care devices and material in the delivery room are essential. In all guidelines respiratory resuscitation is today the priority in the first minutes. Non inva

The preterm surfactant-deficient lung is highly susceptible to tissue injury with the initiation of ventilation. The respiratory management of preterm infants in the delivery room may be the key to minimizing acute lung injury and its sequelae, including chronic lung disease. Volume-targeted ventilation, optimization of the alveolar recruitment with positive end-expiratory pressure and prophylaxis with surfactant in infants with small volumes of lung gas and delicate lung tissue might help in limiting acute damage during resuscitation in the delivery room. It is desirable to monitor oxygen sat

Issues relevant to term newborns include screening for functional neurologic deficits that may be improved by early detection and intervention. Papers are presented that consider ocular, acoustic, and cognitive evaluations in the term neonate. Delivery room management of the meconium-stained infant remains controversial and is discussed by several authors. Data are presented regarding intubation and its role in the prevention of meconium aspiration syndrome, causes of meconium passage in utero, and its significance for neonatal outcome. In addition, illicit drug use, especially cocaine, is a s

All infants born in 1973-1978 to nurses working in anesthesiology or as operating room nurses were identified from a nationwide registry of all births in Sweden, a registry of hospitalized spontaneous and legally induced abortions that covers 70% of Sweden, and a nurse registry (n = 1323). For comparison, a group was formed that consisted of nurses working in medical wards (n = 1382). Delivery outcome was also compared with the estimate expected from nationwide figures. No statistically significant differences were seen, but infants of the anesthesiology/operating room nurses had a slightly hi

The approach to respiratory support in preterm infants at birth has shifted from invasive to non-invasive techniques. The effectiveness of non-invasive respiratory support relies on a good mask seal and maintaining a patent airway, but this appears to be more challenging than initially thought. The force applied to the mask must be sufficient for a good mask seal to reduce leak, but too much pressure on the face mask can inhibit breathing. Also, airway obstruction due to a closed glottis can prevent the lungs from being effectively ventilated. It is now evident that spontaneous breathing is es

In the delivery room, fetal well-being is evaluated through laboratory tests, biosignals like cardiotocography, and imaging techniques such as fetal echocardiography. We have developed a multimodal machine learning model that integrates medical records, biosignals, and imaging data to predict fetal acidosis, using a dataset from a tertiary hospital's delivery room (n=2,266). To achieve this, features were extracted from unstructured data sources, including biosignals and imaging, and then merged with structured data from medical records. The concatenated vectors formed the basis for training a

BACKGROUND: Thermoregulation interventions in the delivery room have historically focused on preterm infants and studies often exclude term infants or those infants with known congenital anomalies.
PURPOSE: The purpose of this quality improvement project was to reduce the rate of admission hypothermia in neonates of all gestational ages born with congenital anomalies and admitted to the intensive care unit (ICU).
METHODS: Utilizing the Institute for Healthcare Improvement model for improvement, implementation of plan, do study, act cycles focused on standardizing temperatures of the delivery

BACKGROUND: Heart rate (HR) is considered the main vital sign in newborns during perinatal transition, with a threshold of 100 beats per minute (bpm), below which, intervention is recommended. However, recent changes in delivery room management, including delayed cord clamping, are likely to have influenced normal HR transition.
OBJECTIVE: To summarize the updated knowledge about the factors, including measurement methods, that influence HR in newborn infants immediately after birth. Additionally, this paper provides an overview of delivery room HR as a prognostic indicator in different subgr

Quality improvement has become a foundation of neonatal care. Structured approaches to improvement can standardize practices, improve teamwork, engage families, and improve outcomes. The delivery room presents a unique environment for quality improvement; optimal delivery room care requires advanced preparation, adequately trained providers, and carefully coordinated team dynamics. In this article, we examine quality improvement for neonatal resuscitation. We review the published literature, focusing on reports targeting admission hypothermia, delayed cord clamping, and initial respiratory sup

Multiple births are increasing in frequency related to advanced maternal age and fertility treatments, and they have an increased risk for congenital anomalies compared to singleton births. However, twins have the same congenital anomalies <15% of the time. Thus, having multiple births with discordant anomalies is a growing challenge for neonatologists. Although external anomalies can often be spotted quickly at delivery or sex differences between multiples can rapidly identify those with internal anomalies described on prenatal ultrasound, we present a case of male multiples, who would optima

BACKGROUND: In 2011, the Neonatal Resuscitation Program (NRP) added consideration of continuous positive airway pressure (CPAP) for spontaneously breathing infants with labored breathing or hypoxia in the delivery room (DR). The objective of this study was to determine if DR-CPAP is associated with symptomatic pneumothorax in infants 35 to 42 weeks' gestational age.
METHODS: We included (1) a retrospective birth cohort study of neonates born between 2001 and 2015 and (2) a nested cohort of those born between 2005 and 2015 who had a resuscitation call leading to admission to the NICU and did n

Oxygen is the most commonly used medicine used during neonatal resuscitation in the delivery room. Oxygen therapy in delivery room should be used judiciously to avoid oxygen toxicity while delivering sufficient oxygen to prevent hypoxia. Measurement of appropriate oxygenation relies on pulse oximetry, but adequate ventilation and perfusion are equally important for oxygen delivery. In this article, we review oxygenation while transitioning from fetal to neonatal life, the importance of appropriate oxygen therapy, its measurement in the delivery room, and current recommendations for oxygen ther

The immediate newborn transition is a time of great physiologic adjustments and many infants need assistance to make a successful transition to newborn life. Assisted ventilation is the most important intervention performed during this transitional period. Noninvasive ventilation is a necessary skill for all pediatric providers because it is the most frequently required lifesaving measure provided in the delivery room. Providing ventilation in the least injurious manner is also necessary and many aspects of how this can best be done are still unknown. Following the normal physiology of fetal t

There is a paucity of data to support the recommendations for cardiac compressions for the newly born. Techniques, compression to ventilation ratios, hand placement, and depth of compression guidelines are generally based on expert consensus, physiologic plausibility, and data from pediatric and adult models.
