An Over-the-Counter Healing Ointment: Benefits in Dry Skin and Wound Healing.

The use of ointments can be beneficial for dry, chapped, or cracked skin and also for supporting wound healing. We describe the results of 2 studies with an over-the-counter healing ointment (HO) to evaluate the effects on skin hydration and in the setting of wound healing after dermatologic procedures.  Methods: Study 1 was a single-center, in-use study using HO on qualified areas at least once daily for 4 weeks in subjects with dry, cracked body skin and self-perceived sensitive skin. Study 2 was a multi-center study of wound healing in subjects using HO on a daily basis after having de

Surgical Approach to Hidradenitis Suppurativa.

Hidradenitis suppurativa is a chronic inflammatory skin condition characterized by painful nodules, abscesses, and sinus tracts that may lead to irreversible scarring complications. Therapeutic options include antibiotics, biologic therapies, and surgical procedures. Current management of hidradenitis suppurativa favors early surgical intervention along with medical therapy to promote healing and minimize scars and complications in a disease characterized by a therapeutic window of opportunity. Surgical techniques range from incision and drainage to wide excision, with varying recurrence rates

Use of ultra-high frequency electrosurgery (radiosurgery) for cosmetic surgical procedures.

BACKGROUND: Electrosurgery has been used for many decades in medicine. Some machines generate ultra-high frequency current waveforms (radiosurgery), which minimize collateral tissue damage. OBJECTIVE: Personal experience using radiosurgery in the performance of cosmetic surgery is related in the narrative form to offer the reader a "how we do it" perspective. CONCLUSION: Radiosurgery facilitates the performance of blepharoplasty, face lifting, hair restoration surgery, abdominoplasty, nasal reduction sculpturing of rhinophyma, and minor cosmetic procedures. Collateral heat damage is minimal

Surgical treatment of hidradenitis suppurativa.

The treatment of hidradenitis suppurativa is multimodal, even though it begins with a dermatologic examination and therapy prescribed by the dermatologist. A multidisciplinary approach is necessary to achieve the best results and to optimize the treatment. Surgical treatment should be individualized according to the degree of the disease. Adequate preoperative preparation of the patient and the wound is necessary. Surgical treatment alone does not guarantee a 100% safe outcome, and remissions are possible regardless of the treatment method. We present current surgical modalities available for

The external rhinoplasty approach for rhinologic surgery.

The technique of external rhinoplasty has enjoyed a renaissance over the last ten years, primarily for cosmetic and functional septorhinoplasty, and we have found this to be an effective method for nasal reconstruction. Moreover, we have recognized the versatility of this approach for a variety of rhinologic problems and have utilized it for transsphenoidal hypophysectomy, sphenoidotomy, unilateral choanal atresia, septal perforation, nasal valvuloplasty and rhinophyma. We describe our technique and the rationale for employing it. We conclude that the enhanced exposure provided by the cutaneou

Dermatologists perform the majority of cutaneous reconstructions in the Medicare population: numbers and trends from 2004 to 2009.

BACKGROUND: Dermatologists are experts in skin cancer treatment. Their experience with cutaneous reconstruction may be underrecognized. OBJECTIVE: We sought to determine the percentage of skin reconstruction claims submitted to Medicare by dermatologists relative to other specialists. METHODS: The Medicare Physician Supplier Procedure Master File from 2004 to 2009 was accessed to determine the proportion of layered closures, grafts, and flaps by specialty. RESULTS: In 2009, dermatologic surgeons' (DS) claims accounted for 60.8% of intermediate closures, 75.1% of complex repairs, 55.5% of lo

Reappraising Elements of the Aseptic Technique in Dermatology: A Review.

Dermatologic procedures are performed under varying degrees of antisepsis, and no clear guidelines exist regarding the role of the aseptic technique in dermatology. This review aims to clarify the terminology surrounding surgical asepsis and examines the importance of various components of the aseptic technique in cutaneous surgery. Included are studies examining optimal glove type, surgical instruments, skin antisepsis, and cost-reducing protocols. Our review highlights that most dermatology procedures are not performed under completely sterile conditions due to the lack of environmental and

Tranexamic Acid in Cutaneous Oncologic and Cosmetic Surgery: A Comprehensive Narrative Review.

Tranexamic acid (TXA) is a synthetic lysine analog with antifibrinolytic properties used to reduce bleeding during various surgical procedures. In dermatologic surgery, it is often applied topically (soaked gauze), subcutaneously (intralesional with local anesthesia), and intravenously. We conducted a narrative review on the utility of TXA in dermatologic surgery, both oncologic and esthetic. Therefore, we conducted a literature search across PubMed and Google Scholar during March 2025, including retrospective and prospective studies, and systematic reviews. We eventually found multiple random

The role of imaging in the prevention of wrong-site surgery in dermatology.

Wrong-site surgery in dermatology often results from inaccurate identification of a skin cancer biopsy site. Factors making biopsy-site identification difficult include background actinic damage, delays from biopsy to surgery, and lack of photographic documentation. While other methods exist for biopsy-site identification, photography is the most helpful tool available. Although modern technology has made high-quality photographic equipment ubiquitous and easy to use, photography for biopsy-site identification continues to be underutilized. The authors recommend that photographic documentation

Ergonomics in Dermatologic Procedures: Mobility Exercises to Incorporate In and Out of the Office.

Dermatologists-particularly dermatologic surgeons-commonly experience neck, shoulder, and back pain due to prolonged static positions and repetitive motions inherent in their practice. Musculoskeletal injuries not only can impact career longevity and satisfaction but also can affect the quality of patient care provided. We highlight mobility techniques and exercises that dermatologic surgeons can incorporate both in and outside the procedure room to reduce pain and prevent future injuries.

Lack of complications in minor skin lesion excisions in patients taking aspirin or warfarin products.

Many patients undergoing surgical procedures take medications that influence the coagulation system. It is common practice to discontinue the use of aspirin and warfarin products 7 to 10 days before any major surgical procedure. However, there is some controversy as to whether these medications should be discontinued for minor dermatological procedures. Our aim was to study the incidence of complications in patients receiving aspirin or warfarin and undergoing minor dermatological procedures. Two thousand three hundred twenty-six patients, operated on by a single surgeon, were studied for comp

Skin substitutes in dermatosurgery.

Skin substitutes are a growing market since technical advancements have allowed a substantial progress in treating extensive defects of the skin. A variety of skin substitutes with different properties and thus resulting different indications is offered on the market. Important benefits of skin substitutes are their ready availability in almost any quantity and the predictable product properties concerning implantation, incorporation, resorption and long-term outcome. Although, most skin substitutes still need skin grafts at a later date which is disadvantageous. Nevertheless dermal substitute

Keloid Formation and Any Skin Complications in Patients Treated With Isotretinoin and Undergone Any Skin-Related Procedures.

BACKGROUND: Isotretinoin is widely used for moderate to severe acne vulgaris. Despite its broad application, isotretinoin carries a risk of permanent scarring and keloid formation following various skin procedures. As a result, a delay of at least 6-12 months after completing or discontinuing isotretinoin treatment is commonly recommended before undergoing skin procedures. AIMS: This study aims to evaluate the necessity of delaying skin procedures performed concurrently with or soon after isotretinoin treatment at different dosages in patients with acne vulgaris, based on the dermatological s
