Anticonvulsant drug-induced rickets and multiple slipped epiphyses in a child treated non-operatively: a case report.

The clinical and radiographic presentation of a child with spastic tetraplegia, anticonvulsant drug-induced rickets, borderline hypothyroidism and multiple slipped epiphyses is described. While the metabolic abnormalities were being treated, the parents denied surgical treatment and have been non-compliant with bracing of the wrist, ankle and knee deformities. By two years of medical treatment, rickets had resolved and the growth plates of the lower limbs' joints had closed. Non weight-bearing, gentle physiotherapy and bracing led to good results in the hip, ankle and wrist joints and to unacc

Comparative study of pinning in situ and open epiphysiodesis in 105 patients with slipped capital femoral epiphyses.

One hundred five patients were treated for slipped capital femoral epiphyses during the period from 1964 to 1976. Attempts were made to evaluate the differences in results of multiple pinning and open epiphysiodesis performed to treat this problem. Pinning in situ was performed in 61 hips, and open epiphysiodesis was performed in 33 hips. The average follow-up period was seven years four months for pinning in situ and six years seven months for open epiphysiodesis. The average slippage was 22 degrees for patients treated by pinning in situ and 30 degrees for patients treated by open epiphysiod

Epiphyseal Translation as a Predictor of Avascular Necrosis in Unstable Slipped Capital Femoral Epiphysis.

BACKGROUND: Physeal instability has been shown to be associated with a higher risk of avascular necrosis (AVN) in patients with slipped capital femoral epiphysis (SCFE). The purpose of this study was to identify additional preoperative factors associated with AVN in patients with unstable SCFE. METHODS: Basic demographic information, chronicity of symptoms, and estimated duration of nonambulatory status were noted. Preoperative radiographs were used to measure the Southwick slip angle, slip severity by Wilson criteria, and epiphyseal translation. Translation was measured by 3 distinct radiogr

Intraoperative Monitoring of Epiphyseal Perfusion in Slipped Capital Femoral Epiphysis: Surgical Technique.

INTRODUCTION: Intraoperative monitoring of epiphyseal perfusion in slipped capital femoral epiphysis (SCFE) is a procedurally simple and readily accessible percutaneous technique to accurately guide decision-making and help to prevent osteonecrosis. STEP 1 PATIENT PREPARATION AND POSITIONING: Following anesthesia induction, position the patient and assess the physeal stability fluoroscopically to determine the need for a reduction. STEP 2 GUIDEWIRE PLACEMENT: Place a 3.2-mm threaded guidewire from the anterolateral aspect of the femur to provide initial stability of the slipped epiphysis. S

[Reduction of epiphyseal displacement and fixation in the treatment of unstable slipped capital femoral epiphysis].

UNLABELLED: Reduction of epiphyseal displacement by manipulation in the treatment of unstable slipped capital femoral epiphysis (scfe) can lead to very severe complications as avascular necrosis or acute necrosis of cartilage. The aim of this study is to analyze the results of treatment of unstable scfe with open reduction and fixation. Material consists of 14 patients (10 boys and 4 girls) with 15 hips treated surgically because of unstable scfe with an average follow up of 10 years and 6 months (ranged from 2 to 26 yrs). The age of the patients ranged from 9 to 17 yrs (mean 13). There was 14

Slipped capital femoral epiphysis: factors affecting shear forces on the epiphyseal plate.

Very little is known regarding the magnitude of physiological shear forces in the normal capital femoral epiphyseal plate. Changes in these forces related to deformities in the proximal femur have been postulated to increase significantly the shear forces and predispose to slipped epiphysis. By force analysis techniques, a three-dimensional model has been developed to study the change in shear forces that might be expected from abnormal angulation in the frontal and sagittal planes. This study suggests that within the range of normal activity, shear forces on the epiphyseal plate are well belo

Failures of pin removal after in situ pinning of slipped capital femoral epiphyses: a comparison of different pin types.

We reviewed 75 pin retrievals after in situ pinning of 35 children with slipped capital femoral epiphysis (SCFE). Four types of pins were used. Ten pins in 8 hips could not be removed because of pin breakage or stripping. Our failure rate was significantly related to pin type and size (p less than 0.039). As compared with the expected values using the chi-square method of analysis, the noncannulated large steel pins and cannulated steel pin groups had fewer failures (p less than 0.001) as compared with the cannulated titanium and noncannulated small steel pin groups. We now avoid using cannula

Slipped capital femoral epiphyses: A major on-target adverse event associated with FGFR tyrosine kinase inhibitors in pediatric patients.

Fibroblast growth factor receptor (FGFR) tyrosine kinase inhibitors (TKIs) are increasingly being used off label in pediatrics. Long-term safety data are limited, and serious toxicities unique to pediatrics may emerge. In a retrospective analysis of patients less than 18 years of age with recurrent/refractory FGFR altered gliomas treated with FGFR TKIs at MSKCC (n = 7), we observed slipped capital femoral epiphyses in three of seven patients along with increased linear growth velocity. Clinicians should closely monitor bone health and have a low index of suspicion for serious orthopedic compli

Comparison of Fatigue Performance Between Fully and Partially Threaded Cannulated Screws Used for Stabilization of Slipped Capital Femoral Epiphyses.

Cannulated screws are widely used for the treatment of slipped capital femoral epiphysis; however, the optimal choice and number of implants have not been clearly defined. Studies have shown that two screws are biomechanically superior to a single screw in load-to-failure testing, but the fatigue performance of different screw designs has not been investigated. This study compared the fatigue characteristics of fully and partially threaded cannulated screws. Partially and fully threaded 7.3-mm screws were subjected to cyclic loading on a custom testing fixture. Screws were tested in three-poin

[Slipped capital femoral epiphyses in ten members of one family: probably autosomal dominant transmission with variable penetrance (author's transl)].

Ten members of a family suffered from slipped capital femoral epiphyses (ECF), and two further descendants had coxarthrosis possibly following subclinical course of an ECF. This pedigree is suggestive for a dominant inheritance with variable penetrance. Besides a few families with more than two affected members, almost 95% of the cases in the literature occur sporadically, many of these probably representing phenocopies. In view of the recurrence risk (at least 5% in sporadic cases), genetic counseling of the parents of any patient is recommended. Since early onset of treatment improves the pr

[Treatment of chronic slipped capital femoral epiphysis : Use of dynamic epiphyseal telescopic screws].

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a multifactorial structural loosening in the area through the epiphyseal plate between the epiphysis and metaphysis accompanied by slippage of the femoral head in the mid-dorsal-caudal direction without additional adequate trauma. In this retrospective study, all patients with chronic SCFE were assessed who had been treated by implanting a dynamic epiphyseal telescopic (DET) screw. METHODOLOGY: All patients who had been treated at our hospital with a DET screw implant between December 2006 and November 2014 following diagnosis of chronic

An anatomic study of the epiphyseal tubercle and its importance in the pathogenesis of slipped capital femoral epiphysis.

BACKGROUND: It has been proposed that the epiphyseal tubercle on the inferior surface of the capital femoral epiphysis may be responsible for the clinical distinction between a stable and an unstable slipped capital femoral epiphysis (SCFE). The anatomy of the tubercle and its relationship to the lateral epiphyseal vessels have not previously been rigorously defined. METHODS: Twenty-two cadaveric capital femoral epiphyses from donors who had been three to seventeen years of age were analyzed and then digitized with use of a high-resolution laser scanner. The height, location, and approximate

Shear stress in epiphyseal growth plate is a risk factor for slipped capital femoral epiphysis.

BACKGROUND: Slipping of the capital femoral epiphysis is an important orthopaedic problem of early adolescence. Many hypotheses about its etiology have been examined, yet the underlying mechanisms have not yet been fully elucidated. We examined elevated shear stress in the epiphyseal growth plate and elevated contact hip stress exerted on the femoral head as risk factors for slipping of the capital femoral epiphysis. METHODS: Two groups of hips were compared: a group of 100 hips contralateral to the slipped ones and a group of 70 age- and gender-matched healthy hips. The characteristics of in

Slipping of the upper femoral epiphyses in patients with intracranial tumours causing hypopituitarism and chiasmal compression.

Four cases of slipped upper femoral epiphyses in patients with intracranial tumours causing hypopituitarism and chiasmal compression are presented. Detailed endocrine studies in three cases showed severe deficiencies of growth hormone as well as of gonadotrophin and sex hormones. The literature is reviewed and the aetiology is discussed with special reference to Harris's hypothesis that an increase in growth hormone relative to oestrogen predisposes to slipping of the upper femoral epiphysis in humans, which these cases do not seem to support. In all cases the slip was bilateral, and it is emp

Capital Femoral Epiphyseal Extension May Confer Physeal Stability in Slipped Capital Femoral Epiphysis.

BACKGROUND: Factors including obesity and morphologic parameters around the hip that increase physeal stress are associated with an increased risk of slipped capital femoral epiphysis (SCFE). Recent evidence suggests that superior epiphyseal extension may confer stability to the physis and help protect against SCFE. The purpose of this study is to investigate the relationship between epiphyseal extension and SCFE using an age-matched and sex-matched cohort study. METHODS: We generated 2 separate cohorts for comparison: 89 patients with unilateral SCFE and 89 healthy subjects with no evidence

Smaller epiphyseal tubercle in hips with slipped capital femoral epiphysis compared to the uninvolved contralateral hip.

Recent investigations suggest that physeal morphologic features have a major role in the capital femoral epiphysis stability and slipped capital femoral epiphysis (SCFE) pathology, with a smaller epiphyseal tubercle and larger peripheral cupping of the femoral epiphysis being present in hips with progressive SCFE compared to healthy controls. Yet, little is known on the causal versus remodeling nature of these associations. This study aimed to use preoperative magnetic resonance imaging (MRI) of patients with unilateral SCFE to perform a comparison of the morphology of the epiphyseal tubercle,

What Is the Association Among Epiphyseal Rotation, Translation, and the Morphology of the Epiphysis and Metaphysis in Slipped Capital Femoral Epiphysis?

BACKGROUND: Contemporary studies have described the rotational mechanism in patients with slipped capital femoral epiphysis (SCFE). However, there have been limited patient imaging data and information to quantify the rotation. Determining whether the epiphysis is rotated or translated and measuring the epiphyseal displacement in all planes may facilitate planning for surgical reorientation of the epiphysis. QUESTIONS/PURPOSES: (1) How does epiphyseal rotation and translation differ among mild, moderate, and severe SCFE? (2) Is there a correlation between epiphyseal rotation and posterior or

Comparison of three radiographic assessment methods for detecting slipped capital femoral epiphyses in cats: Klein's line, modified Klein's line and the S-sign.

OBJECTIVES: The aim of the present study was to investigate whether diagnostic assessment methods used on radiographs in humans with slipped capital femoral epiphysis (SCFE) can be used in cats. METHODS: The ventrodorsal (VD) extended-leg and VD frog-leg pelvic radiographs of 20 cats with SCFE without fully displaced femoral capital epiphyses (FCE), eight cats with fully displaced FCE and five control cats with normal pelvic anatomy were assessed by five observers on two separate occasions 3 months apart.
