Interventricular Septal Thickness as a Diagnostic Marker of Fetal Macrosomia.

Serious complications in both mother and newborn arising as a result of fetal macrosomia indicate the need for early diagnosis and prevention. Unfortunately, current predictors, such as fetal biometry, fundal height, and amniotic fluid index, appear to be insufficient. Therefore, we decided to assess the predictive potential of interventricular septal thickness (IVST), as measured at ≥33 weeks of gestation. Two hundred and ninety-nine patients met the inclusion criteria: complete medical history including all necessary measurements-namely, IVST obtained by M-mode echocardiography, fetal biomet…

Fetal Macrosomia at a Tertiary Care Centre in Lagos, Nigeria.

BACKGROUND: Large babies have attracted immense attention as they present obstetric problems with associated increase in perinatal morbidity and mortality. The major risk of fetal macrosomia is trauma to the fetus and mother during vaginal delivery. OBJECTIVE: To compare the outcome of macrosomic babies with babies of normal birth weights. METHODS: This was a retrospective comparative study of deliveries over a period of two years from 1 "August 2005 to 31st July 2007. RESULTS: There were 198 macrosomic babies (6.9%) out of a total of 2,879 deliveries that occurred within the study period. …

Counseling for fetal macrosomia: an estimated fetal weight of 4,000 g is excessively low.

OBJECTIVE: Because of the known complications of fetal macrosomia, our hospital's policy has been to discuss the risks of shoulder dystocia and cesarean section (CS) in mothers with a sonographic estimated fetal weight (SEFW) ≥ 4,000 g at term. The present study was performed to determine the effect of this policy on CS rates and pregnancy outcome. STUDY DESIGN: We examined the pregnancy outcomes of the macrosomic (≥ 4,000 g) neonates in two cohorts of nondiabetic low risk women at term without preexisting indications for cesarean: (1) SEFW ≥ 4,000 g (correctly suspected macrosomia) and (2) S…

Prediction of fetal macrosomia: effect of sonographic fetal weight-estimation model and threshold used.

OBJECTIVE: To compare the accuracy of 21 sonographic fetal weight-estimation models and abdominal circumference (AC) as a single measure for the prediction of fetal macrosomia (> 4000 g) using either fixed or optimal model-specific thresholds. METHODS: A total of 4765 sonographic weight estimations performed within 3 days prior to delivery were analyzed. The predictive accuracy of 21 published sonographic fetal weight-estimation models was calculated using three different thresholds: a fixed threshold of 4000 g; a model-specific threshold obtained from the inflexion point of the receiver-oper…

Obstetric challenges in suspected fetal macrosomia: Risk factors and assessment.

BACKGROUND: Suspected fetal macrosomia is associated with increased rates of cesarean deliveries, assisted vaginal deliveries, and shoulder dystocia. Obstetric management strategies are limited by the lack of accuracy in estimating fetal weight. In this context, the associated factors for difficult delivery remain poorly defined. OBJECTIVE: To assess antenatally identified maternal and fetal factors associated with adverse obstetric outcomes in pregnancies with suspected fetal macrosomia. STUDY DESIGN: Secondary analysis of a randomized controlled trial including women with suspected fetal m…

Ultrasonographic Fetal Weight Estimation: Should Macrosomia-Specific Formulas Be Utilized?

OBJECTIVE: This study aims to derive an estimated fetal weight (EFW) formula in macrosomic fetuses, compare its accuracy to the 1986 Hadlock IV formula, and assess whether including maternal diabetes (MDM) improves estimation. STUDY DESIGN: Retrospective review of nonanomalous live-born singletons with birth weight (BWT) ≥ 4 kg and biometry within 14 days of birth. Formula accuracy included: (1) mean error (ME = EFW - BWT), (2) absolute mean error (AME = absolute value of [1]), and (3) mean percent error (MPE, [1]/BWT × 100%). Using loge BWT as the dependent variable, multivariable linear reg

Fetal macrosomia in a military hospital: incidence, risk factors, and outcome.

The purpose of this paper is to determine the incidence of fetal macrosomia, analyze risk factors, and review maternal and fetal outcome. Macrosomia occurred in 15.1% of deliveries, with very macrosomic fetuses comprising 4.1% of these pregnancies; 61.3% were male. Diabetes mellitus, post-term pregnancy, and excessive weight gain were identified as maternal risk factors. The incidence of shoulder dystocia, birth injury, and low Apgar scores was significantly higher than in controls. In addition, cesarean section rates were higher for the macrosomic groups. Fetuses delivered vaginally had more …

Erb/Duchenne's palsy: a consequence of fetal macrosomia and method of delivery.

Methods of delivery, maternal and neonatal characteristics were examined to determine their role in the occurrence of Erb/Duchenne's palsy. Data from 210,947 Washington state birth certificates from 1980 through 1982 were examined. The incidence was 50.2 cases of Erb's palsy per 100,000 live births. A case control study design was used to analyze 106 cases and 386 controls by both univariable and multivariable analysis. Birth weight was shown to be a significant risk factor regardless of which method of delivery was used. A high birth weight infant (4001-4500 g) had 2.5 times the risk of incur
