The objective of this study was to identify major pathologies among firefighters, being the cause of leaving the job. The study was based on the analysis of temporary work disability and disability certificates, taking account of age and duration of employment in the capacity of a firefighter. The study was performed on a representative sample of 1503 firefighters, employed between 1994 and 1997 in 29 fire stations. During that period, 214 firefighters left their job (40% of them got retired and 38% of persons were given the right to receive disability pension). The analysis revealed the avera

UNLABELLED: Firefighting is one of the most physically demanding, dangerous, and stressful jobs, constituting several physical activities, which include moving equipment up the stairs in tall buildings and deploying charged hoses. In fire management, structured training exercises are fire drills used to prepare staff for successful responses to fires situations. Firefighters, emergency responders, and other pertinent people can practice responding to a real fire by simulating real-life fire events. However, South African fire departments and stations, particularly in Mpumalanga, Nkangala distr

BACKGROUND: Linking firefighter injury reporting to general motion patterns may provide insight into potential injury mechanisms and the development of prevention strategies.
OBJECTIVE: To characterize the injuries sustained by members of a large Canadian metropolitan fire department over a 5-year span.
METHODS: Data were taken from injury reports filed by career firefighters between 2007 and 2011. Injuries were described by job duty, type, body part affected, and the general motion pattern employed at the time of injury (e.g. lifting).
RESULTS: Of the 1311 injuries reported, 64% were categ

Background: Firefighting is a hazardous occupation, yet data online-of-duty deaths in European firefighter populations remain limited. This study aimed to characterise the mechanisms and circumstances of firefighter fatalities in Poland and to estimate exposure-based fatality rates, with particular attention to differences between career and volunteer personnel. Methods: In this retrospective observational study, line-of-duty firefighter fatalities in Poland from 1995 to 2025 were identified from a nationwide repository and cross-validated against official sources. The mechanism of death was c

BACKGROUND: Alcohol misuse is prevalent among firefighters, and associated adverse cardiometabolic health consequences could negatively impact readiness for duty. Mental health conditions may confer additional risk. Therefore, we aimed to determine whether alcohol misuse increases cardiometabolic risk among firefighters and whether mental health conditions modify these relationships.
METHODS: Deidentified data from firefighters (N = 2405; 95.8% males, 38 ± 9 years, 29.6 ± 4.6 kg/m2) included demographics, Alcohol Use Disorders Identification Test (AUDIT) and AUDIT-C scores, mental health scre

BACKGROUND: By the nature of their work, first responders are at risk for post-traumatic stress disorder (PTSD). Efficient screening instruments are useful to identify at-risk first responders and connect them to services.
AIMS: The current study aimed to (i) evaluate the diagnostic properties of the Primary Care PTSD for DSM-5 (PC-PTSD-5) scale among firefighters, (ii) explore the use of an adapted PC-PTSD-5 on a five-point Likert-type scale and (iii) examine sensitivity and specificity of the adapted instrument in this population.
METHODS: Pooled data were analysed among firefighters (N =

OBJECTIVE: Firefighters are plagued with cardiometabolic disease (CMD). Obesity, poor cardiorespiratory and muscular fitness, and blood lipids (low-density lipoprotein cholesterol, triglycerides, low high-density lipoprotein cholesterol) are risk factors for CMD. However, markers of oxidative stress, inflammation, and insulin resistance can provide further insight regarding CMD risk.
METHODS: This study investigated the relationships between fitness metrics (cardiorespiratory and muscular fitness, percent body fat, waist circumference), blood lipids, blood pressure, and years of experience as

BACKGROUND: In California, state prison inmates are employed to fight wildfires, which involves performing soil-disrupting work. Wildfires have become more common, including areas where Coccidioides, the soil-dwelling fungus that causes coccidioidomycosis, proliferates. However, work practices that place wildland firefighters at risk for coccidioidomycosis have not been investigated.
METHODS: On August 17, 2017, the California Department of Public Health was notified of a cluster of coccidioidomycosis cases among Wildfire A inmate wildland firefighters. We collected data through medical recor

Firefighting is a high-risk occupation that accounts for vulnerability to a range of mental health problems and addictive behaviours. However, no research has addressed whether this vulnerability extends to gambling problems, and the aim of this study was thus to provide new data on frequency and implications of such problems in this occupational context. The sample consisted of n = 566 career and retained firefighters who participated in a cross-sectional survey of an Australian metropolitan fire service. The Problem Gambling Severity Index (PGSI) was used to operationalise both clinically si

Freezing is a defensive response to acute stress that is associated with coping and alterations in attentional processing. However, it remains unclear whether individuals in high risk professions, who are skilled at making rapid decisions in emergency situations, show altered threat-induced freezing. Here we investigated the effect of incident experience in a high risk profession on freezing. Additionally, we explored whether any effect of incident experience on freezing would be different for profession-related and -unrelated threat. Forty experienced and inexperienced firefighters were prese

BACKGROUND: Exposure to occupational hazards among firefighters may lead to increased mortality from cancer, lung, or heart disease.
METHODS: Age- and gender-adjusted mortality rates of 34,796 male and 2,017 female Florida professional firefighters between 1972 and 1999 were compared with the Florida general population.
RESULTS: One thousand four hundred eleven male and 38 female firefighter deaths with known causes were identified. In male firefighters, mortality due to all causes and most non-malignant diseases was significantly less than expected. There was no excess overall mortality fro

Long-term psychological effects of exposure to a dangerous chemical were evaluated in a sample of firefighters who fought a large polyvinyl chloride (PVC) fire. When compared with nonexposed firefighter controls (n = 22), the exposed firefighters (n = 64) had significantly higher levels of demoralization and specific emotional distress 22 months after the incident. Longitudinal analyses of a subsample of exposed firefighter subjects (n = 55) surveyed 5 to 6 weeks after the fire and again 22 months later revealed that there was no reduction in symptomatology over time. Some psychological distre

Cardiovascular disease (CVD) is the leading cause of on-duty death among firefighters (45% of on-duty fatalities) and a major cause of morbidity. CVD in the fire service also has adverse public safety implications as well as significant cost impacts on government agencies. Over the last decade, our understanding of CVD among firefighters has significantly improved and provides insight into potential preventive strategies. The physiology of cardiovascular arousal and other changes that occur in association with acute firefighting activities have been well-characterized. However, despite the str

How do non-governmental standards matter for occupational and environmental health and safety? Internationally recognized standards specify requirements for structural firefighter personal protective equipment (PPE), but concerns have emerged about hazardous chemicals and firefighters' potential exposures. In this paper, we draw on the case of the National Fire Protection Association's standards for firefighter PPE to develop the concept of peri-governance, work done by non-governmental actors or organizations that exerts governance force through various mechanisms, including government entiti

Firefighting requires high levels of aerobic fitness, muscular strength and endurance to perform demanding tasks under extreme conditions. Although previous research has focused mainly on general physical fitness, fewer studies have examined the impact of physical training interventions on task-specific performance. This systematic review aims to evaluate the effectiveness of structured physical training programs in improving firefighters' occupational task performance. Following PRISMA guidelines, a systematic literature search was conducted in October 2024 across PubMed, Cochrane, Web of Sci

INTRODUCTION: Fire suppression can result in injury and death, with cardiac events being the leading cause of death of on-duty structural firefighters. Few data, however, are available regarding the physiologic responses to shipboard fires. Therefore, the purpose of this report was to determine physiologic responses of simulated shipboard firefighting tasks.
MATERIALS AND METHODS: Nineteen subjects (age: 24 ± 5 y, V˙O2peak: 44.9 ± 7.2 mL · kg-1 · min-1) performed firefighting tasks in protective garments and breathing apparatus. After completing each task, heart rate (HR), core temperature (T
