Kidney graft rejection studies with labeled platelets and lymphocytes.

The usefulness of In-111-labelled platelets and lymphocyte scintigraphy in acute kidney graft rejection is evaluated. One hundred fifty-five patients (36 treated with cyclosporine A) were studied with labelled platelets and 27 with labelled lymphocytes. Blood cels were labelled with 100-150 microCi of In-111-oxine and reinjected. Subsequently patients were scanned once daily from 2 hours post-reinjection up to a week. The graft/contralateral area activity ratio was calculated in all scans (index I). Four groups of patients were established: Functioning grafts (FG); post-operative acute renal f

[Rejection of a kidney graft after pregnancy and delivery].

Eighteen months after cadaver kidney transplantation a 22-year-old woman was successfully delivered of a healthy female child. Onset of allograft rejection in the third trimenon was followed three weeks after delivery by progressive renal failure and resumption of regular dialysis treatment. Pregnancy risk factors after successful renal transplantation and immunosuppressive therapy are discussed.

Influence of rejection on graft survival after renal transplantation.

The clinical course was reviewed of 102 renal allograft recipients between December 1967 and December 1973. Only 4 of 21 patients (19 per cent) who had 2 or more episodes of rejection during the first 2 months had a functioning graft at the end of 1 year, compared to 24 of 30 patients (83 per cent) who had no rejection episodes. A similar trend was seen 2 to 6 months after transplantation. During the first 2 years vigorous immunosuppressive therapy for rejection in the first few months resulted in 12 deaths (44 per cent) of 27 patients. Subsequent to this immunosuppressive therapy was modified

Immunology in corneal transplantation-From homeostasis to graft rejection.

Immunology depends on maintaining a delicate balance within the human body, and disruptions can result in conditions such as autoimmune diseases, immunodeficiencies, and hypersensitivity reactions. This balance is especially crucial in transplantation immunology, where one of the primary challenges is preventing graft rejection. Such rejection can lead to organ failure, increased patient mortality, and higher healthcare costs due to the limited availability of donor tissues relative to patient needs. Xenotransplantation, like using porcine corneas for human transplants, offers a potential solu

Pancreas rejection: quieting the storm to preserve graft function.

PURPOSE OF REVIEW: Allograft rejection remains enigmatic and elusive following pancreas transplantation. In the absence of early technical pancreas graft failure, pancreas allograft rejection is the major cause of death-censored pancreas graft loss both short- and long-term. Despite this circumstance, there are variations in the diagnosis and treatment of pancreas rejection. In this article, we summarize recent literature, review common practices, and discuss various management algorithms. RECENT FINDINGS: Although pancreas allograft biopsy is the gold standard for the diagnosis of rejection,

Comprehensive Analysis of Cardiac Xeno-Graft Unveils Rejection Mechanisms.

Porcine heart xenotransplantation is a potential treatment for patients with end-stage heart failure. To understand molecular mechanisms of graft rejection after heart transplantation, we transplanted a 31-day-old alpha-1,3-galactosyltransferase knockout (GTKO) porcine heart to a five-year-old cynomolgus monkey. Histological and transcriptome analyses were conducted on xenografted cardiac tissue at rejection (nine days after transplantation). The recipient monkey's blood parameters were analyzed on days -7, -3, 1, 4, and 7. Validation was conducted by quantitative real-time PCR (qPCR) with sel

Subconjunctival bevacizumab for vascularized rejected corneal grafts.

We describe 3 patients who experienced corneal neovascularization following keratoplasty and were treated with a single subconjunctival injection of 2.5 mg bevacizumab. Although there was immediate regression of the corneal vascularization and haze, as well as improvement in the anterior chamber reaction, the effects were short-lived and starting from the second week, the corneal vessels began to progress. All 3 cases ended in permanent graft failure. The series suggests a possible role for bevacizumab in the management of graft rejection with vascularization.

Valve function during graft rejection and before graft failure in pediatric heart transplant recipients.

OBJECTIVE: Management of immunosuppression after human valve transplant remains unclear and may be aided by understanding valve function during rejection and with chronic graft failure after heart transplant. We assessed valve function during biopsy-diagnosed rejection and before death/retransplantation in pediatric heart transplant recipients. METHODS: This was a single-center, retrospective study. Patients in cohort 1 had 2 serial biopsies showing nonrejection followed by rejection and concurrent echocardiograms. Cohort 2 had graft loss (death or retransplant) with a pre-event echocardiogra

Local graft irradiation after failure of modern immunosuppression in acute cellular and vascular graft rejection.

PURPOSE: With improved chemical immunosuppressive agents, approximately 90% of rejection episodes can be reversed. However, in situations of failed immunosuppression, graft loss becomes inevitable. Our objective is to assess the efficacy of local graft irradiation (LGI) as an effort of last resort in a contemporary group of patients in whom graft failure to irreversible cellular and vascular rejection is imminent. METHODS AND MATERIALS: A total of 308 renal transplantations were performed at our institution from 1992 to 1995, and an overall 1-year graft survival rate of 90% has been seen as a

Beyond rejection: engineering immune responses to redefine skin graft survival.

Skin grafting remains a fundamental reconstructive technique in reconstructive and dermatologic surgery for restoring skin integrity following burns, trauma, chronic ulcers, and excision of malignancies. However, a graft survival rate is determined by multiple factors and by a delicate balance between inflammatory and reparative immune system processes. Controlled inflammation promotes wound healing while excessive or prolonged immune activation has the potential to result in cytotoxicity and complement activation which will eventually lead to graft rejection. Early rejection is driven by anti

Gut microbiome alterations precede graft rejection in kidney transplantation patients.

Kidney transplantation (KT) is the best treatment for end-stage kidney disease, with graft survival critically affected by the recipient's immune response. The role of the gut microbiome in modulating this immune response remains underexplored. Our study investigates how microbiome alterations might be associated with allograft rejection by analyzing the gut microbiome using 16S rRNA gene amplicon sequencing of a multicenter prospective study involving 562 samples from 245 individuals of whom 217 received KT. Overall, gut microbiome composition showed gradual recovery post-KT, mirroring chroni

[Incidence of acute rejection in patients with renal graft dysfunction].

INTRODUCTION: Acute rejection has been identified as the main cause of renal graft dysfunction during the first year after transplantation; it is associated with chronic structural and functional damage, which causes loss of graft and decrease in patient survival. MATERIAL AND METHODS: We performed a retrospective and descriptive research consisting in a review of the final reports of biopsies performed due to renal graft dysfunction during the postransplant period. Patients included were transplanted at the Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubirán (INCMNSZ) from Ja

Effect of donor and recipient factors on corneal graft rejection.

PURPOSE: To assess the relationship between donor and recipient factors and corneal allograft rejection in eyes that underwent penetrating keratoplasty in the Cornea Donor Study. METHODS: Overall, 1090 subjects undergoing corneal transplantation for a moderate risk condition (principally Fuchs dystrophy or pseudophakic corneal edema) were followed for up to 5 years. Associations of baseline recipient and donor factors with the occurrence of a probable or definite rejection event were assessed in univariate and multivariate proportional hazards models. RESULTS: Eyes with pseudophakic or aphak

Graft loss associated with late humoral rejection after renal transplantation.

Antibody-mediated rejection (AMR) is an important cause of renal allograft loss after renal transplantation. With the improvement of the crossmatching method and the use of more aggressive immunosuppressants, the incidence and outcome of early AMR has been reduced. Late humoral rejection is becoming an important cause of graft loss. In this chapter, we demonstrated 8 renal allograft recipients who experienced renal allograft dysfunction associated with late humoral rejection, which suggests that graft loss could be caused by anti-donor antibodies.

Rejection episodes and patient and graft survival after renal transplantation.

The occurrence of acute rejection episodes following renal transplantation was correlated with one-year patient mortality and graft failure rates in a series of 102 consecutive transplant recipients. Twenty-one patients had two or more rejection episodes in the first two months post transplantation; 17 of these expired or lost their kidney compared to only 5 of 28 patients with no rejection episodes in this time period. Similarly, of the 14 patients who developed a second rejection episode 2--6 months post transplantation, 8 died or lost the graft compared to only 2 of 21 patients who had no r
