Varicella-zoster virus (VZV) vasculopathy is a granulomatous vasculitis that has a wide variety of clinical presentations. It is most common in patients with HIV not on anti-retroviral therapy (ART) with low cluster of differentiation (CD)4 cell counts. This disease affects the central nervous system and can cause small intracranial bleeds. Our patient presented with stroke-like symptoms in the setting of recent VZV reactivation in the ophthalmic distribution with HIV on ART. Her MRI scan showed a small punctate bleed and the CSF workup was consistent with VZV vasculitis. She was treated with

Pediatric hemorrhagic stroke (HS) has become a problem, determining both a high frequency of fatal outcomes and early disability. The priority objective determining the effectiveness of diagnosis, treatment, and prevention of hemorrhagic stroke is to identify the etiology of the disease. We report a case of hemorrhagic stroke in a 2-month old child with cytomegalovirus infection, which led to a negative outcome of the disease. In the context of the current presentation, a set of neuroimaging, somatic, and laboratory changes in the child with HS is shown. Our case demonstrates a high probabilit

Symptomatic hemorrhagic transformation (HT) is one of the complications most likely to lead to death in patients with acute ischemic stroke. HT after acute ischemic stroke is diagnosed when certain areas of cerebral infarction appear as cerebral hemorrhage on radiological images. Its mechanisms are usually explained by disruption of the blood-brain barrier and reperfusion injury that causes leakage of peripheral blood cells. In ischemic infarction, HT may be a natural progression of acute ischemic stroke and can be facilitated or enhanced by reperfusion therapy. Therefore, to balance risks and

OBJECTIVE: We sought to examine the diagnostic utility of existing predictors of any hemorrhagic transformation (HT) and compare them with new perfusion imaging permeability measures in ischemic stroke patients receiving alteplase only.
METHODS: A pixel-based analysis of pretreatment CT perfusion (CTP) was undertaken to define the optimal CTP permeability thresholds to predict the likelihood of HT. We then compared previously proposed predictors of HT using regression analyses and receiver operating characteristic curve analysis to produce an area under the curve (AUC). We compared AUCs using

Coronavirus associated severe acute respiratory syndrome (SARS-CoV-2) causes a worldwide syndrome called Covid-19 that has caused 5,940,441 infections and 362,813 deaths until May 2020. In moderate and severe stages of the infection a generalized swelling, cytokine storm and an increment of the heart damage biomarkers occur. In addition, a relation between Covid-19 and neurological symptoms have been suggested. The results of autopsies suggest thrombotic microangiopathy in multiple organs. We present 2 cases of patients infected with severe Covid-19 that were hospitalized in the Reanimation Un

BACKGROUND: This is the first case to our knowledge of a serious adverse event following the Epley maneuver, which is the treatment of choice for benign paroxysmal positional vertigo (BPPV), the most common vestibular disorder in adults.
CASE PRESENTATION: A 77 year old female presented for outpatient evaluation of vertigo at a tertiary otolaryngology clinic. She was found to have BPPV clinically, and elected to have a particle repositioning maneuver (Epley maneuver) performed in clinic. Immediately following Epley maneuver, she had severe nausea and vomiting, with evolving visual changes. A

Hemorrhagic stroke, including subarachnoid hemorrhage (SAH), intracerebral hemorrhage (ICH), and intraventricular hemorrhage (IVH), is associated with high morbidity and mortality driven by secondary injury processes such as neuroinflammation, blood-brain barrier disruption, and oxidative stress. Increasing evidence identifies the complement system as a central mediator of these responses. Following hemorrhage, exposure of blood components and damaged tissue rapidly activates the classical, lectin, and alternative complement pathways, converging on C3 and leading to generation of anaphylatoxin

Hemorrhagic stroke occurs due to a rupture of a blood vessel in the brain. This leads to initial mechanical damage at the site of injury and secondary injuries including axonal degeneration (AxD). Since axons are critical for all brain functions, we systematically reviewed studies that focused on AxD in two major types of hemorrhagic stroke, intracerebral hemorrhage and subarachnoid hemorrhage, to understand how and to what extent AxD develops and to interrogate underlying mechanisms and potential therapeutic targets. After screening 817 publications published until September 18, 2024, we iden

A 45-year-old Japanese woman had sudden-onset subarachnoid hemorrhage and subsequently developed recurrent intracerebral hemorrhages and multiple infarctions in the left and right cerebral hemispheres, respectively. Angiotensin-converting enzyme (ACE) levels in the cerebrospinal fluid (CSF) were elevated, and a pathological examination of brain biopsy samples demonstrated multinucleated giant cell infiltration and epithelial granuloma formation in thickened pial vessel walls. After treatment with intravascular methylprednisolone 1,000 mg for 3 days, followed by oral prednisolone, she had no re

Hemorrhagic stroke, mainly caused by intracerebral hemorrhage (ICH), is a severe neurological condition with high mortality and lasting disability. ICH involves bleeding into the brain parenchyma, hematoma formation, and subsequent edema and tissue damage, triggering inflammatory and degenerative responses that worsen secondary brain injury (SBI). Microglia, the brain's resident immune cells, are key mediators in this process. Their ability to sense, engulf, and clear hematoma-derived debris is essential for controlling neuroinflammation and promoting tissue repair. Central to microglial phago

Hemorrhagic transformation (HT) is a serious complication that worsens outcomes and increases mortality in patients with ischemic stroke (IS). HT can occur both spontaneously and after reperfusion therapy. Severe ischemic injury in IS is not sufficient in itself to cause HT; one of the key elements in its development is reperfusion. Delayed reperfusion in the area of severe ischemic injury mainly increases the likelihood of HT due to disruptions in the blood-brain barrier (BBB), which, in turn, play a key role in the formation of HT in the acute period of IS. Currently, perfusion CT and MRI ar

Several neurological manifestations are recognized in dengue infection, but stroke is a rare complication. We report a case of ischemic stroke in a patient with dengue hemorrhagic fever. A 52-year-old previously healthy male presented with a history of fever for 2 days, and left-sided weakness and numbness of sudden onset. MRI scanning showed a right-sided thalamic lacunar infarct. Diagnosis of dengue fever was made based on leuco-thrombocytopenia, positive dengue nonstructural protein-1 (NS-1) antigen, and positive dengue IgM antibodies. Severity of limb weakness correlated with the critical

BACKGROUND AND PURPOSE: Hemorrhagic transformation (HT) is a complication of stroke that can occur spontaneously or after treatment. We aimed to assess the inter- and intrarater reliability of HT diagnosis.
METHODS: Studies assessing the reliability of the European Cooperative Acute Stroke Study (ECASS) classification of HT or of the presence (yes/no) of HT were systematically reviewed. A total of 18 raters independently examined 30 post-thrombectomy computed tomography scans selected from the Aspiration versus STEnt-Retriever (ASTER) trial. They were asked whether there was HT (yes/no), what

BACKGROUND: The clinical distinction between hemorrhagic and ischemic stroke cannot be achieved by simple clinical evaluation, and it is impossible to submit all stroke patients to computed tomography. A simple, reliable, and safe diagnostic tool for acute stroke syndrome is needed. This study tested the Siriraj stroke score to verify its accuracy for distinguishing among the pathological subtypes of stroke.
METHODS: This study included the one hundred and seventy-one patients with acute supratentorial stroke syndromes consecutively admitted to the Emergency Room of the Taichung Veterans Gene
