External rotation contracture of the extended hip. A common phenomenon of infancy obscuring femoral neck anteversion and the most frequent cause of out-toeing gait in children.

External rotation contracture of the extended hip is common in young infants, decreasing progressively with growth so that it appears to be present in less than 5 per cent of the children over age 18 months. Persistence of the external rotation contracture was the main cause of toeing-out gait in this study. Femoral neck anteversion, as a cause of internal rotation posturing of the limb, does not become clinically recognizable until complete resolution of the external rotation contracture of the hip has occurred, that is, usually after 18 months of age. Femoral neck retroversion was not presen

A holistic approach for severe flexion contracture of bilateral hip, knee, and ankle joints in a neglected patient with prolonged knee-chest positioning on extreme undernourishment: a case report and review of the literature.

BACKGROUND: Flexion contracture in the lower extremity is a common finding in the patient with neuromusculoskeletal disorders. However, severe cases due to prolonged immobilization in knee-chest position are rarely established and remain underreported. This condition is associated with high morbidity and reduced quality of life, especially when it comes to neglected cases with missed injury and late presentation for adequate primary care and rehabilitative program. It remains a difficult challenge to treat, with no established treatment protocol. In addition, other factors related to psycholog

Pelvic deformity secondary to tensor fascia lata tightness associated with desmoid tumor.

The iliotibial band is a thick, condensed fascia that, when contracted, leads to a hip flexion, abduction, and external rotation contracture in addition to other joint contractures. Iliotibial band tightness occurs secondary to iliotibial band friction syndrome, which commonly occurs at the lateral femoral epicondylar region. However, a proximal cause of iliotibial band/tensor fascia lata friction syndrome leading to a secondary hip contracture is swelling around the hip; this swelling being a desmoid tumor has not been explicitly described in the literature. The authors present a rare case of

Gait analysis in low lumbar myelomeningocele patients with unilateral hip dislocation or subluxation.

The surgical indications for the treatment of unilateral hip dislocations or subluxations in patients with low lumbar myelomeningocele remain highly debatable. This study examines the influence of unilateral hip dislocation or subluxation on the gait of these patients using three-dimensional gait analysis. Twenty patients with a diagnosis of low lumbar myelomeningocele underwent three-dimensional gait analysis. All patients were community ambulators with solid ankle-foot orthoses and crutches who presented with unilateral hip dislocation or subluxation and no scoliosis. The patients were divid

Idiopathic Capital Femoral Chondrolysis: A Case Report.

Idiopathic capital femoral chondrolysis is a rare condition most commonly seen in African-American pre-adolescent females. The primary symptoms are hip stiffness and pain, which are accompanied by limping. Physical examinations typically reveal an external rotation contracture, flexion contracture, and abduction contracture. There is also immobility at the hip secondary to muscle spasms. Here, we present and discuss the follow-up case of a 10-year-old female patient who was diagnosed with idiopathic chondrolysis of capital femoral epiphysis, right side, and underwent right hip arthroscopic eva

[Symptoms of spinal osteochondrosis].

Having screened 384 patients suffering from Scheuermann's disease and 2992 eleven-year-old children, the authors conclude that the early and salient symptoms of the disease are mobile thoracic kyphosis associated with the flexious contracture of the hip joint, and occasional anteflexious contracture of the shoulder joint together with a left-curved functional scoliosis. The authors analyse the nature and frequency of scoliosis, excluding other causes of the flexion contracture of the hip joint. Their findings are supported by data processed on computer.

Natural history of flexion deformity of the hip in myelodysplasia.

For definition of the incidence and natural history of hip flexion contracture, 5,147 serial measurements of the range of hip extension in 966 patients with spina bifida were analyzed. Contractures were generally present in the first few months of life (physiologic flexion posture); this then diminished during the first 27 months in all but those with thoracic lesions. Hence, surgical management is seldom appropriate until after that age. In general, contractures reappear or worsen between the ages of 3 and 6 years, but few patients with sacral lesions develop this deformity. Our findings also

The hip in arthrogryposis multiplex congenita.

The hip is involved in up to 80% of individuals with a diagnosis of arthrogryposis multiplex congenita. The hip deformity consists of contracture with or without dislocation. Isolated contracture can usually be treated conservatively by manipulation and splinting, only occasionally requiring operative intervention. Dislocation is as frequently bilateral as unilateral. Bilateral hip dislocations are best left unreduced; only accompanying contractures should be treated. The unilateral dislocation should be treated aggressively, because persistent dislocation will give rise to pelvic obliquity an

Incidence of spondylolisthesis in ambulatory cerebral palsy patients.

We prospectively studied 50 consecutive ambulatory cerebral palsy (CP) patients to determine the incidence of isthmic spondylolisthesis. In addition, we examined the relationship of hip flexion contractures to development of spondylolisthesis and low back pain. Three patients who had undergone previous spine operation were eliminated from the study group. Of the remaining 47 patients, one patient (2%) demonstrated an asymptomatic grade I spondylolisthesis. Another patient (2%) demonstrated spondylolysis without spondylolisthesis. Only six patients reported occasional low back pain. Pain did no

Is there a directional relationship between windswept hips and scoliosis in children with cerebral palsy?

BACKGROUND: The purpose of this study was to analyse if there is a relationship between the directions of windswept hip deformity and scoliosis for children with cerebral palsy (CP) and to explore potential associations with spasticity, contractures and hip displacement. METHODS: A cross-sectional registry-based study was conducted using data from the combined Swedish CP Follow-up Program and National Registry, including children aged 0–18 years across all levels of the Gross Motor Function Classification System. Prevalence and direction of windswept hip deformity was assessed based on calcula

Natural history of the dislocated hip in spastic cerebral palsy.

A review was made of 88 adult institutionalized patients with spastic cerebral palsy and contractural deformity of the hips. 21 were untreated for dislocated hip, and 11 of these suffered from hip pain. The degree of pain was directly related to neurological maturity and to the coexistence of athetosis and spasticity. Decubitus ulcers and perineal care problems were more associated with contractures than with dislocation alone. It is concluded that dislocation and subluxation should be prevented by surgical means, but that surgical treatment of the already dislocated hip should be reserved for

Lower extremity joint contracture according to ambulatory status in children with Duchenne muscular dystrophy.

BACKGROUND: Lower extremity joint contractures have negative effects on gait in children with Duchenne muscular dystrophy (DMD). Thus, contracture prevention is essential for maintaining a patient's functional ability and an acceptable quality of life. This study investigated hip flexion (HF), knee flexion (KF), and ankle joint plantar flexion (APF) contractures among male patients with DMD, based on the patients' ambulatory status. Differences in major joint contractures, based on passive stretching exercise participation, were also investigated. METHODS: A total of 128 boys with DMD, follow
