A patient-mediated strategy to improve nutrition care after transition from hospital to home for pancreatic cancer surgery: a pilot randomised controlled trial study protocol.

INTRODUCTION: Nutrition counselling is recommended after pancreatic cancer surgery given the complex nutritional problems patients experience. In practice, access and delivery of nutrition counselling after pancreatic surgery varies across settings. To address this gap, our study team developed the Support Through Remote Observation and Nutrition Guidance (STRONG) programme, an implementation strategy that addresses common barriers to nutrition care delivery in oncology. METHODS AND ANALYSIS: The STRONG programme includes a standardised protocol to specify the timing and amount of nutrition c

A scoping review of generative AI and digital twins in tackling loneliness during hospital-to-home transitions.

Loneliness is an important psychosocial concern for older adults and may be intensified during the transition from hospital to home, when established routines, professional contact, functional capacity, and social participation can change simultaneously. This scoping review examined how generative artificial intelligence (GenAI), AI-enabled technologies, and digital twins (DTs) have been investigated in relation to loneliness, social connectedness, and psychosocial support for older adults, with particular attention to their relevance to hospital-to-home transitions. Exploring success in sub-acute hospital-to-home transitions for palliative patients: a descriptive qualitative study.

BACKGROUND: Transitioning from a sub-acute hospital to home is highly desirable to some palliative and end-of-life individuals and their family caregivers, but the transition process itself is complex and can be logistically challenging. This formative assessment sought to better understand what constitutes a successful sub-acute hospital-to-home transition from the perspectives of health care providers connected to a sub-acute care facility in Ottawa, Canada, with reflections from a patient and a family caregiver. METHODS: Our descriptive qualitative study involved 13 virtual interviews and

Engaging patients in designing a transmural allied health pathway: A qualitative exploration of hospital-to-home transitions.

INTRODUCTION: The transition from hospital to home is often suboptimal, resulting in patients not receiving the necessary allied healthcare after discharge. This may, in turn, lead to delayed recovery, a higher number of readmissions, more emergency department visits and an increase in mortality and healthcare costs. This study aimed to gain insight into patients' experiences, perceptions, and needs regarding hospital-to-home transition, focusing on allied healthcare as a first step towards the development of a transitional integrated allied healthcare pathway for patients with complex care ne

Examining the course of transitions from hospital to home-based palliative care: A mixed methods study.

BACKGROUND: Hospital-to-home transitions in palliative care are fraught with challenges. To assess transitions researchers have used patient reported outcome measures and qualitative data to give unique insights into a phenomenon. Few measures examine care setting transitions in palliative care, yet domains identified in other populations are likely relevant for patients receiving palliative care. AIM: Gain insight into how patients experience three domains, discharge readiness, transition quality, and discharge-coping, during hospital-to-home transitions. DESIGN: Longitudinal, convergent pa

'I went home not feeling prepared from the outset': describing the co-design of a technological solution to address gaps in the transition from hospital to home for people with acquired brain injury.

People with acquired brain injury (ABI) experience difficulties returning to everyday life post hospital discharge, despite specialised inpatient rehabilitation. This study sought to describe the co-design process of a project that aimed to explore how technology could be used to better prepare people with ABI for the transition home, as well as present participant feedback on experiences of participating in the process. A series of workshops were conducted with people with ABI (n = 4), health professionals (n = 3) and technology designers (n = 2). Participants identified that a smart device a

A Mediation Model of Medication Literacy and Adherence in Type 2 Diabetes During Hospital-Home Transition.

PURPOSE: The hospital-home transition is a high-risk period for medication management in patients with type 2 diabetes mellitus (T2DM). While bivariate relationships among medication literacy, beliefs, self-efficacy, and adherence have been explored, the integrated mediating mechanisms during this phase remain unclear. This study examined these interrelationships, focusing on the serial mediation beliefs and self-efficacy between literacy and adherence. PATIENTS AND METHODS: A cross-sectional study enrolled 315 T2DM patients at discharge from a tertiary hospital in Dongguan, China, between Au

Transitioning from hospital to home with non-invasive ventilation: who benefits? Results of a cohort study.

BACKGROUND: Non-invasive ventilation (NIV) is effective in a variety of acute respiratory illnesses in hospitalised patients. Home NIV is effective for stable patients with hypercapnia due to neuromuscular or chronic pulmonary disease. However, there are little data to guide which patients may benefit from NIV immediately following hospitalisation with hypercapnia. OBJECTIVE: To evaluate outcomes of patients with daytime hypercapnia at the end of an acute hospital admission. DESIGN: Retrospective cohort study. PARTICIPANTS: Entry into the cohort was by querying the hospital electronic medic

Exploring Patient and Caregiver Work and Continuity of Care During Hospital-to-Home Transitions of Care in the Canadian Context.

BACKGROUND: Patients and caregivers are expected to engage in work that involves both physical and cognitive tasks to manage their care and well-being during hospital-to-home transitions. Patient and caregiver work is defined as the health-related tasks they must manage, as well as the physical and cognitive effort required to complete those tasks. When continuity of care is well-supported, work for patients and caregivers may be reduced or better managed. However, there is a need to better understand how continuity of care affects patients' and caregivers' work to optimise their outcomes. OB

Unpaid caregivers' process of collaborating with others during older adult hospital-to-home transitions: A grounded theory study.

Unpaid/family caregivers provide support critical to older adult hospital-to-home transitions, but lack time and preparation. There is limited evidence regarding important collaboration for caregivers during the transition. The objective was to examine caregivers' process of collaborating with others, including other family members, healthcare professionals, and community, social, and professional networks, during older adult hospital-to-home transitions. This study utilized grounded theory methodology. One-on-one interviews were conducted with unpaid/family caregivers of an older adult during
