The ultrastructure of sensory nerve endings in the human knee joint capsule.

The ultrastructure of sensory nerve endings in the human knee joint capsule was studied. Three types of nerve endings were found: free nerve endings (FNE), Ruffini corpuscles and Pacini corpuscles. In the joint capsule, FNE are located below the synovial layer and within the fibrous layer near blood vessels. These nerve terminals derive from myelinated A delta-fibres or from unmyelinated C-fibres. Their structure is almost identical to FNE in human hairy and non-hairy skin. Ruffini corpuscles are present within the fibrous layer and the ligaments of the capsule in three variations: small Ruffi

MR arthrography of the second and third metatarsophalangeal joints for the detection of tears of the plantar plate and joint capsule.

OBJECTIVE: Tears of the plantar plate and joint capsule of the metatarsophalangeal joint are underrecognized causes of lesser metatarsalgia. This study evaluated MR arthrography for evaluation of this condition in 45 metatarsophalangeal joints in 41 patients. CONCLUSION: Most metatarsophalangeal joint capsular tears occur at the distal lateral border of the plantar plate, often with injury of the adjacent interosseous tendon. Plantar plate tears are less common. MR arthrography of the metatarsophalangeal joint delineates capsular tears and associated abnormalities.

Clinical management of finger joint capsulitis/synovitis in a rock climber.

This case study presents a 23-year-old male recreational rock climber, who climbed an average of 3-4 times per week and presented with finger joint capsulitis/synovitis after increasing his climbing intensity and training from moderate to high over 6 months, which led up to injury. During the exam, the diagnosis was ruled in with clinical orthopedic testing. Additional movement analyses revealed improper gripping mechanics contributing to asymmetric finger loading. A comprehensive rehabilitation program was developed based on the concept of a progressive framework that included unloading of th

Anatomy of the Tarsal Canal and Sinus in Relation to the Subtalar Joint Capsule.

BACKGROUND: Anatomical knowledge of the tarsal canal and sinus is still unclear owing to the complexity of the ligamentous structures within them, particularly the relationship with the capsules of the subtalar joints. The aim of this study was to examine the anatomical relationship between the fibrous tissues of the tarsal canal and sinus and the articular capsules of the subtalar joint. METHODS: We conducted a descriptive anatomical study of 21 embalmed cadaveric ankles. For a macroscopic overview of the subtalar joint, we removed the talus in 18 ankles and separated the fibrous tissues fro

Tensile properties of the knee-joint capsule at an elevated intraarticular pressure.

In order to evaluate the capsular reaction to high intraarticular pressure (IAP) in the knee during arthroscopy, we examined 13 patients admitted for elective surgery with joint infusion to 30, 70, 120 and 170 mmHg IAP (4, 9.3, 16 and 22.7 kPa). The infusion was repeated once, at all IAP levels except 30 mmHg, after 2 min and was recorded for another 2 min. A pressure-time curve was recorded as a combined effect of viscoelastic properties of the capsule and extraarticular fluid absorption. 3 more knee joints were infused directly to 170 mmHg and a pressure-time curve was recorded for 15 min, a

[Anterior and lateral disk and capsule tissue of the temporomandibular joint and its relevance for temporomandibular joint diagnosis].

OBJECTIVE: Fibrocartilage that bears tensile stress is typically fixed bilaterally to adjacent bone by collagenous fibres. For the temporomandibular joint, such disc fixations in the anterior and lateral regions have not been described, even in MRT findings of anterior disc dislocations without reduction. By observing collagenous fibres of the anterior and lateral discuss and capsule tissue, we sought to establish whether tensile load capacities typical for fibrocartilage also apply to the discuss articularis of the mandibular joint in sagittal direction. MATERIALS AND METHODS: Using dissecti

Ultrasound assessment of temporomandibular joint capsule-condyle distances across Angle's occlusal classifications.

OBJECTIVE: To investigate the morphological characteristics of the temporomandibular joint (TMJ) capsule-condyle distance using ultrasound imaging in individuals with different Angle's occlusal classifications. METHODS: Ultrasound imaging was performed in both transverse and longitudinal planes, capturing measurements with the mouth closed and open. Six specific joint space distances were recorded: LLC, LLO, LTC, LTO, ATC, and ATO. RESULTS: All six joint space measurements showed significant deviation from a normal distribution. Median values ranged from 0.9 mm to 1.1 mm, with interquartile

Anatomical study of the radial side of thumb metacarpophalangeal joint: the joint capsule and flexor pollicis brevis muscle.

INTRODUCTION: Radial collateral ligament reconstruction for thumb metacarpophalangeal joint instability can result in residual pain and limited motion. While the flexor pollicis brevis, located on the radial side of the metacarpophalangeal joint, is believed to contribute to dynamic stability, the radial collateral ligament provides static stability. However, the structural relationship between the flexor pollicis brevis, radial collateral ligament and joint capsule remains unclear. This study aimed to clarify the anatomical and functional relationship between flexor pollicis brevis, radial co

Joint capsule pathology as a contributor to pain in hallux valgus: a histological study.

Pain in hallux valgus (HV) often arises from the medial aspect of the first metatarsophalangeal (MTP) joint, but its mechanisms remain unclear. This study investigated the histological and molecular features of the medial capsule (MC) and their association with pain. Fifty feet from 49 HV patients undergoing corrective surgery were analyzed histologically and immunohistochemically for calcitonin gene-related peptide (CGRP), matrix metalloproteinase-3 (MMP3), and collagen type I/III. The severity of capsular degeneration was scored, and pain was evaluated using the Self-Administered Foot Evalua

The role of MRI and Ultrasonography in Diagnosis and Treatment of Glenohumeral Joint Adhesive Capsulitis.

Adhesive capsulitis is a common disorder of the glenohumeral joint. Delayed diagnosis is the result of overlapping signs and symptoms with other disorders affecting the shoulder. Typically, the disease shows gradual progression of pain and loss of the range of motion. The hallmark of the physical examination is limitation of both passive and active motion without any associated degenerative changes on plain radiographs. Conservative and/or surgical treatments have shown conflicting results. Poor outcome may be related to co-morbid factors mainly including prolonged immobilization, rotator cuff

A large-scale assessment of the healthcare burden of adhesive capsulitis of the shoulder joint.

BACKGROUND AND AIM: Frozen shoulder is a poorly understood pathological entity that is characterised by a painful and stiff shoulder. To analyse the socioeconomic impact of frozen shoulder in terms of cost generation and prolonged absence from work, we conducted a retrospective cohort study based on shoulder injuries through accidents in Switzerland. METHODS: Data were obtained from the comprehensive database of the Statistical Service for the Swiss National Accident Insurances (SSUV). Cases with shoulder injuries (ICD-10 codes S4* and M84.3*) and/or an additional code of adhesive capsulitis

Interpositional Arthroplasty Using Mammary Capsule for Finger Joints: A Novel Technique.

Degenerative osteoarthritis frequently affects the hands, altering the movements; surgical therapy includes arthrodesis and arthroplasty. We report the case of a female patient who presented arthrosis in the hands, severe on the proximal interphalangeal (PIP) joints, initially in her right index finger, and subsequently in the index and middle left fingers. At first, she received treatment in the PIP joint of the right index finger with a silicone implant; later on, she presented fracture of it and required replacement 8 years later. In the PIP joints of index and middle left fingers, treatmen

Analysis of the capsule and ligament insertions about the acromioclavicular joint: a cadaveric study.

PURPOSE: The purpose of this study was to analyze the capsular and ligamentous insertions about the acromioclavicular (AC) joint to determine the amount of bone that can be removed without destabilizing the joint. METHODS: We dissected 28 cadaveric shoulders. The AC ligament insertions were measured under loupe magnification with a digital caliper on the acromial and clavicular sides on the anterior, posterior, superior, and inferior edges. We measured the distance to the coracoacromial (CA) and coracoclavicular ligaments. In addition, the axial and coronal angle of the AC joint was measured.

Cystic adventitial degeneration: ectopic ganglia from adjacent joint capsules.

Cystic adventitial degeneration is a rare non-atherosclerotic cause of peripheral arterial occlusive disease, mainly seen in young men without other evidence of vascular disease. Diagnosis will be established by clinical findings and by ultrasound or angiography and can be treated by excision or enucleation of the affected arterial segment or by percutaneous ultrasound-guided aspiration. However, the etiology of adventitial cysts remains unknown. We report a case of cystic adventitial degeneration showing a connection between the joint capsule and the adventitial cyst, supporting the theory th

Adhesive capsulitis of the glenohumeral joint with an unusual neuropathic presentation: a case report.

A 37-yr-old woman presented with a 7-mo history of unilateral shoulder girdle stiffness, pain, and weakness and had already been diagnosed with frozen shoulder. Physical examination revealed scapular winging and suspicious focal paralysis of shoulder girdle muscles. Subsequently, electrodiagnostic studies reported denervation of deltoid, infraspinatus, serratus anterior, and lower cervical paraspinal muscles, in addition to a prolonged long thoracic nerve latency. The history, physical examination, and cervical magnetic resonance imaging scan seemed most consistent with neuralgic amyotrophy, a
