Hoarseness: is it really laryngopharyngeal reflux?

OBJECTIVE/HYPOTHESIS: This study will test the hypothesis that proton pump inhibitor (PPI) use is prevalent among patients referred for hoarseness and will assess the ultimate diagnosis and factors associated with patients' voice outcomes. STUDY DESIGN: Retrospective review of patients in a tertiary care voice clinic. METHODS: Patients with a primary diagnosis of hoarseness, who were taking or had taken PPIs in the previous 2 months and referred to a tertiary care voice clinic, were identified. The dosage and length of PPI administration, patient report of gastroesophageal reflux (GER), pres

[The correlation between gastric bubble size and laryngopharyngeal reflux pattern in patients with laryngopharyngeal reflux disease].

Objective: To investigate the correlation between gastric bubble size and laryngopharyngeal reflux pattern in patients with laryngopharyngeal reflux disease(LPRD). Methods: A total of 52 LPRD patients who underwent Dx-pH monitoring and anteroposterior chest radiography at the same time from February 2016 to November 2018 were retrospectively studied. Patients were devided into three position-related groups according to the Ryan score of upright and supine: isolated upright reflux(IUR), isolated supine reflux(ISR) and bipositional reflux(BR) groups. In addition, 13 healthy volunteers with negat

Reflux and the Voice: Getting Smarter About Laryngopharyngeal Reflux.

Dysphonia is often blamed on laryngopharyngeal reflux (LPR) in the face of a normal flexible laryngoscopic examination. LPR remains primarily (and unfortunately) a diagnosis of exclusion rather than inclusion in the face of vague throat complaints and laryngeal signs attributed to reflux. LPR remains misdiagnosed and overdiagnosed as the cause of many identical, vague throat symptoms and laryngeal complaints. Despite LPR being commonly implicated as the cause of a myriad of nonspecific pharyngeal symptoms, LPR as a physiologic process is common. Whether or not it is a contributor to a patient'

Laryngopharyngeal reflux is different from classic gastroesophageal reflux disease.

The patterns, mechanisms, manifestations, and treatment of laryngopharyngeal reflux (LPR) and gastroesophageal reflux disease (GERD) differ, and the gastroenterology model of reflux disease does not apply to LPR. LPR patients have head and neck symptoms, but heartburn is uncommon. Consequently, LPR is often called silent reflux. LPR patients have predominantly upright (day-time) reflux and normal esophageal motility; most do not have esophagitis, which is the diagnostic sine qua non of GERD. Moreover, the laryngopharyngeal epithelium is far more susceptible to reflux-related tissue injury than

The San Diego Consensus for Laryngopharyngeal Symptoms and Laryngopharyngeal Reflux Disease.

INTRODUCTION: The term laryngopharyngeal reflux (LPR) is frequently applied to aerodigestive symptoms despite lack of objective reflux evidence. The aim of this initiative was to develop a modern care paradigm for LPR supported by otolaryngology and gastroenterology disciplines. METHODS: A 28-member international interdisciplinary working group developed practical statements within the following domains: definition/terminology, initial diagnostic evaluation, reflux monitoring, therapeutic trials, behavioral factors and therapy, and risk stratification. Literature reviews guided statement deve

The key timing of pharyngeal reflux in patients with laryngopharyngeal reflux.

OBJECTIVE: To analyze the incidence of pharyngeal reflux in laryngopharyngeal reflux patients over a 24-hour period and find out the key timing of pharyngeal reflux. METHODS: We reviewed 69 patients who visited our clinic with LPR-related symptoms and were proven to have pharyngeal reflux via 24-hour multichannel intraluminal impedance-pH (24hr MII-pH) monitoring. Quantitative analysis was conducted for the LPR profiles, such as the acidity of reflux, nighttime reflux, and positional reflux. The time series of pharyngeal reflux episodes and mealtimes were analyzed over a 24-hour period. Also,

Laryngopharyngeal reflux mimicking limited wegener granulomatosis.

We present a case of subglottic stenosis (SGS) in a young patient with positive ANCA but a wrong diagnosis of Wegener granulomatosis (WG). Instead, she was diagnosed as having laryngopharyngeal reflux (LPR). Pitfalls of ANCA testing in this scenario, the route to diagnosis of LPR and the contribution of this entity to subglottic stenosis (SGS) in WG are discussed. Caution when interpreting ANCA results is mandatory to avoid improper management.

Extralaryngeal Manifestations of Laryngopharyngeal Reflux Disease.

Extralaryngeal manifestations of laryngopharyngeal reflux are widely recognised, although evidence to support mechanistic pathways is still lacking. Evidence to support reflux contribution to the following disorders is accumulating: otitis media, sinusitis, obstructive sleep apnea, dental erosions, subglottic and tracheal stenosis, inflammatory lung disease and lung transplant rejection. Acid likely only plays a small part in symptom generation and therefore proton pump inhibitor therapy will not provide sustained relief for many, and should no longer be considered as a diagnostic trial. Laryngopharyngeal reflux induced sleep-related laryngospasm.

BACKGROUND: Sleep-related laryngospasm (SRL) has been defined as the sustained closure of the vocal cords during sleep. Studies have suggested that it is a rare manifestation of laryngopharyngeal reflux (LPR). Difficulties in diagnosing SRL and LPR have led to the condition being under-recognised in the clinical setting. AIMS: The aim of this study was to determine if LPR was the cause of the SRL symptoms seen in our patients. METHODS: A retrospective chart assessment of patients with SRL. Patients with risk factors for LPR were identified. These included smoking status, alcohol intake, a hi

[Laryngopharyngeal reflux disease (LPRD) in children].

LPRD in children is characterized by symptoms, clinical findings, and sequelae caused by the reflux of gastric acid, bile acid or refluxate containing pepsin beyond the esophagus. For diagnostic procedures and therapy of gastroesophageal reflux disease (GERD) in children and adults widely accepted guidelines have been established. However, diagnosis and therapy of LPRD in children remains a continuous issue of inter- and intradisciplinary discussions. Although both GERD and LPRD in children are reflux-induced diseases, these two entities are different in symptoms, cause, diagnostic procedures,

Relationship between Depression and Laryngopharyngeal Reflux.

This study investigated the relationship between depression, somatization, anxiety, personality, and laryngopharyngeal reflux (LPR). We prospectively analyzed 231 patients with symptoms with LPR using the laryngopharyngeal reflux symptom index and the reflux finding score. Seventy nine (34.2%) patients were diagnosed with LPR. A significant correlation was detected between the presence of LPR and total scores on the Patient Health Questionnaire-9 (5.6±5.3 vs. 4.0±4.6, p=0.017) and the 7-item Generalized Anxiety Disorder Scale (4.3±4.9 vs. 3.0±4.5, p=0.041). LPR was significantly more frequent

Normal esophageal physiology and laryngopharyngeal reflux.

Understanding the basic anatomy and physiology contributing to esophageal peristalsis can help with comprehension of esophageal disease states. Laryngopharyngeal reflux (LPR), an extraesophageal variant of gastroesophageal reflux disease, is retrograde movement of gastric contents into the laryngopharynx and is associated with hoarseness, chronic cough, throat clearing, sore throat, and dysphagia. Various direct and indirect mechanisms have been proposed to contribute to LPR. The current diagnostic tests for LPR have significant shortcomings. This article reviews the anatomy and physiology of

Pathophysiology of Laryngopharyngeal Reflux Disease: The Reflux vs Reflex Theory.

Laryngopharyngeal reflux (LPR) involves the reflux of stomach contents into the upper aerodigestive tract, causing symptoms through both direct injury and reflex-mediated mechanisms. While traditional reflux and reflex theories provide insights into LPR pathogenesis, they do not fully encompass its clinical spectrum. Emerging evidence highlights the role of hypersensitivity-both peripheral and central-in LPR, with significant implications for treatment responses and the impact of psychological factors on symptom severity. Understanding these complex interactions is crucial for developing more

Laryngopharyngeal Reflux Disease and Gastroesophageal Reflux Disease Can Mutually Influence.

OBJECTIVE: To investigate the mutually relationship between gastroesophageal reflux disease (GERD) and laryngopharyngeal reflux disease (LPRD). METHODS: All included patients completed simultaneous 24-hour hypopharyngeal intraluminal multichannel impedance pH monitoring (24h-MII-pH), Reflux Symptom Index (RSI), and Reflux Finding Score (RFS). The LPRD diagnosis was based on the occurrence of ≥1 acid or non-acid hypopharyngeal proximal reflux episode(HRE), GERD was defined as a length of time >4.0% of the 24-hour recording spent below pH 4.0 or a DeMeester score >14.72. Patients with both posi
