Characterization of an Artificial Liver Support System-Related Vasovagal Reaction.

OBJECTIVE: An artificial liver support system (ALSS) is an effective therapy for patients with severe liver injury. A vasovagal reaction (VVR) is a common complication in various treatment settings but has not been reported previously in ALSS. METHODS: This study retrospectively evaluated patients who suffered an ALSS-related VRR between January 2018 and June 2019. We collected data from VVR episodes including onset time, duration, changes in heart rate (HR) and blood pressure (BP), and drug treatment. RESULTS: Among 637 patients who underwent ALSS treatment, 18 were included in the study. T

Liver dysfunction associated with artificial nutrition in critically ill patients.

INTRODUCTION: Liver dysfunction associated with artificial nutrition in critically ill patients is a complication that seems to be frequent, but it has not been assessed previously in a large cohort of critically ill patients. METHODS: We conducted a prospective cohort study of incidence in 40 intensive care units. Different liver dysfunction patterns were defined: (a) cholestasis: alkaline phosphatase of more than 280 IU/l, gamma-glutamyl-transferase of more than 50 IU/l, or bilirubin of more than 1.2 mg/dl; (b) liver necrosis: aspartate aminotransferase of more than 40 IU/l or alanine amino

Evaluation of artificial-intelligence-based liver segmentation and its application for longitudinal liver volume measurement.

BACKGROUND: Accurate liver-volume measurements from CT scans are essential for treatment planning, particularly in liver resection cases, to avoid postoperative liver failure. However, manual segmentation is time-consuming and prone to variability. Advancements in artificial intelligence (AI), specifically convolutional neural networks, have enhanced liver segmentation accuracy. We aimed to identify optimal CT phases for AI-based liver volume estimation and apply the model to track liver volume changes over time. We also evaluated temporal changes in liver volume in participants without liver

Emerging applications of traditional and generative artificial intelligence in liver transplantation.

Artificial intelligence (AI) has emerged as a transformative force in liver transplantation (LT), spanning patient selection, donor-recipient matching, intraoperative management, and post-transplant care. Historically, applications have relied on traditional machine learning (ML) and deep learning (DL) techniques trained on structured clinical datasets to improve risk stratification and outcome prediction. Since 2023, however, advances in generative AI (GenAI) and large language models (LLMs) have expanded the scope of AI in LT, enabling multimodal reasoning and the extraction of clinically me

Artificial intelligence in the diagnosis and management of colorectal cancer liver metastases.

Colorectal cancer (CRC) is the third most common malignancy worldwide, with approximately 50% of patients developing colorectal cancer liver metastasis (CRLM) during the follow-up period. Management of CRLM is best achieved via a multidisciplinary approach and the diagnostic and therapeutic decision-making process is complex. In order to optimize patients' survival and quality of life, there are several unsolved challenges which must be overcome. These primarily include a timely diagnosis and the identification of reliable prognostic factors. Furthermore, to allow optimal treatment options, a

Bio-artificial liver from cultured human foetal hepatocytes: feasibility and prospects.

BACKGROUND: Conventional therapy for acute liver failure has not been able to improve survival beyond 40%. Apart from liver transplantation, the most promising development in this field is the utilization of cultured hepatocytes to make 'bio-artificial liver support systems' as a 'bridge to transplantation' or ideally as a 'bridge to total recovery'. This study examines the feasibility of culturing foetal hepatocytes without the use of growth factors and formulating a bio-artificial liver support device in our set-up. METHODS: Foetal hepatocytes were harvested from the liver obtained from mid

Artificial intelligence in liver transplant allocation: Promise, pitfalls, and ethical safeguards.

Liver transplantation (LT) is an ethically complex procedure that involves allocating a limited, life-saving resource. For over twenty years, the Model for End-Stage Liver Disease (MELD) score and its variations have implemented a sickest-first ethical approach, enhancing waitlist survival rates and adding objectivity to the process. Nonetheless, ongoing disparities based on sex, race, and other structural factors reveal the shortcomings of relying on simplified, static models of disease severity. As LT progresses into an era increasingly shaped by artificial intelligence (AI) and machine lear

Artificial intelligence in advanced liver disease: From risk stratification to decision.

The diagnosis and management of liver cirrhosis and portal hypertension have been challenging for hundreds of years, placing a tremendous burden on medical and social resources. In routine practice, clinicians usually depend on various documented scoring and stratification systems to establish the diagnosis and make decisions. These systems have traditionally been constructed through manual review of retrospective data, image interpretation, and statistical analyses. However, the development of artificial intelligence (AI) has allowed for the analysis of large and varied datasets, and conseque
