The history of treating meniscal lesions has been characterized by firm belief in "radical" surgery, with serious long-term consequences for the individual and society. The menisci play a critical protective role for the knee joint through shock absorption and load distribution. Currently, the consensus in surgical treatment of meniscal tears is to preserve as much functional meniscal tissue as possible. Still, meniscal lesions are common, especially in the osteoarthritic knee. For health professionals, these lesions present a challenge in choosing the treatment that is best for the patient in

BACKGROUND: An altered tibial position is still present despite anterior cruciate ligament (ACL) reconstruction. It has been demonstrated that an abnormal tibial position after an ACL injury may play a role in subsequent injuries to the meniscus, which can lead to early cartilage degeneration.
PURPOSE: To determine changes in both the tibial position and the meniscal matrix present before and after ACL reconstruction as well as to evaluate the association between these 2 variables in ACL-injured knees 3 years after reconstruction.
STUDY DESIGN: Cohort study; Level of evidence, 2.
METHODS: B

BACKGROUND/OBJECTIVE: The menisci of the mammalian knee joint balance the incongruence between femoral condyle and tibial plateau and thus menisci absorb and distribute high loads. Degeneration processes of the menisci lead to pain syndromes in the knee joint. The origin of such degenerative processes on meniscal tissue is rarely understood and may be described best as an imbalance of anabolic and catabolic metabolism. A standardized animal model of meniscal degeneration is needed for further studies. The aim of the current study was to develop a porcine animal model with early meniscal degene

In the treatment of proximal and distal tibial fractures, special attention has to be focused on the severity of tissue damage. Guidelines are given in the paper. (1) For tibial head fractures, stable internal fixation is usually necessary but should be delayed for 1 week. The best approach is lateral or ventral. The use of implants for buttressing has to be kept to a minimum. Concomitant ligamentous injuries are reconstructed, and damaged menisci should be preserved. (2) In the case of distal tibial fractures, those not involving the joint can be treated non-operatively. The internal fixation

CASE: We present a rare "flipped lid" type of fracture of the lateral tibial plateau with dislocation of the lateral meniscus into the notch with intact roots. This was fixed using headless screws through arthrotomy and peripheral suturing of the meniscus. At the 1-year follow-up, although there was terminal restriction of 10° flexion with redislocation of the lateral meniscus, the patient was able to perform all daily activities and declined further surgery.
CONCLUSION: Flipped lid-type fracture is rare and results in good outcome when fixed early. Meniscal dislocation repair requires close

OBJECTIVE: To evaluate the correlation between features of knee joint rheumatoid arthritis (RA) identified on MRI and histological examination as a means of elucidating the pathogenesis of joint destruction in RA.
METHODS: This is a prospective analysis of 26 knee joints of 22 patients who underwent total knee arthroplasty (TKA) for the treatment of RA. Based on the degree of destruction of articular cartilage and the menisci, the occurrence of bone marrow edema and bone erosion, and synovial thickening, the stage of the knee joints were classified using MRI by two radiologists. Differences i

Posterior medial meniscal root tears and meniscal extrusion have come under increasing focus in recent years, with management that prevents or delays advancing degenerative changes in the medial compartment being the ultimate goal. Although meniscal root repair can restore meniscal function and slow degeneration, it does not eliminate osteoarthritis progression, with conversion to total knee arthroplasty reported as high as 33%. Augmenting posterior medial meniscal root repair with centralization techniques appears to reduce short-term postoperative extrusion and may mitigate early radiographi

BACKGROUND: While increased posterior tibial slope (PTS) is an established risk factor for anterior cruciate ligament tears, the association between tibial slope and meniscal posterior root tears is not well-defined.
PURPOSE: To summarize the available literature evaluating the association between PTS and meniscus root injuries compared with patients without root tears.
STUDY DESIGN: Systematic review; Level of evidence, 4.

Operations to treat meniscal injuries rank among the most frequent procedures performed by orthopedic surgeons. Ongoing research into the natural history, basic science, and biomechanics of meniscal injury has highlighted the importance of preserving the meniscus to maintain normal knee biomechanics and function. The arthroscopic inside-out suture repair is currently the gold standard by which other meniscal repair techniques are judged. Although it is difficult to identify meniscal tears amenable to repair preoperatively, an assessment of patient factors and tear characteristics on the basis

Arthroscopic resection of bucket handle tears of the meniscus is difficult owing to the obstructed view of the posterior horn. It often requires an additional portal for access or additional tools. We describe a technique involving conventional tools including grasper, shaver, and basket forceps. No additional portal is required. The basket forceps are used to cut the anterior attachment of the tear, and the bucket handle is shaved to a suitable size. The grasper is then used to hold the remaining meniscus and move it to the posterior space. The posterior corner of the meniscus is then turned

MR imaging is the preferred imaging modality for evaluating the meniscus. Overall, when strict criteria are followed, it is accurate in diagnosing meniscal tears in patients who have not had prior meniscal surgery. However, an accurate interpretation requires a through knowledge of the normal meniscal anatomy, common meniscal variants, and common diagnostic pitfalls. In this article, the author emphasizes the importance of describing meniscal tears properly and discusses treatment options. Diagnosing a recurrent tear is more complicated in patients who have had prior partial meniscal resection

The unstable inferior leaf of the anterior horn in the horizontal tear of the lateral meniscus is a challenging lesion to the arthroscopist. However, there are no devices for it and no proper procedures are introduced. We describe a new arthroscopic technique of partial meniscectomy by using three portals. This technique uses a unique portal, extreme far anteromedially.

Posterior medial meniscal root tears represent a clinically significant injury with biomechanical consequences comparable to total meniscectomy, underscoring the need for effective repair strategies to restore joint function and prevent degeneration. Current meniscal root repair techniques vary in biomechanical performance, with different constructs demonstrating trade-offs between ultimate failure load and resistance to cyclic displacement. Although constructs with higher failure loads may appear advantageous, emerging evidence suggests that cyclic stability may be more critical for maintaini

Because of the crucial role of the menisci in maintaining cartilage and joint health, meniscal tears affect the long-term health of the knee. Although partial meniscectomy has a role in the treatment of complex degenerative tears and tears with low healing capacity, advances in the concepts and understanding of meniscal repair, along with improvements in repair techniques and instrumentation, have expanded the indications for meniscal repair. With appropriate patient selection and preoperative planning, repair of meniscal tears can lower the rate of degenerative changes when compared with meni

INTRODUCTION: The past 2 decades have been marked by substantial progress in our knowledge of meniscus anatomy, function, and biomechanics, and also by the shifting of the surgical treatment of meniscal lesions from traditional meniscectomy towards arthroscopic repair to get away from the early osteoarthritis associated with meniscectomy. Posterior horn injuries of the lateral meniscus (LM) have been less studied due to their lower incidence and also due to the historical technical complexity of performing a repair in the posterolateral compartment.
MATERIALS AND METHODS: A retrospective anal

A transection (root tear or complete radial tear) injury of the medial meniscus posterior horn is not rare in the oriental area and needs to be repaired to restore the hoop tension and to reduce the extruded meniscus, which leads to osteoarthritis of the knee. In cases with transection of the medial meniscus posterior horn, the meniscus can be repaired by a pull out suture technique. However, it is difficult to manipulate a suture hook and drill a tibial tunnel in the narrow medial joint space using the traditional anterior arthroscopic technique. This article describes a new pull out suture t
