Naifold capillaroscopy in mixed connective tissue disease patients.

INTRODUCTION: Mixed connective tissue disease (MCTD) is a rare systemic disease characterized by overlapping features of systemic lupus erythematosus (SLE), systemic sclerosis (SSc), dermato-/polymyositis (DM/PM), and rheumatoid arthritis (RA). Naifold capillaroscopy (NFC) is a non-invasive test for evaluating the capillaries of the nail shaft used in the diagnosis of rheumatic diseases. OBJECTIVES: To determine whether there are characteristic abnormalities in NFC in MCTD patients, and whether the type of NFC lesions correlates with organ involvement in these patients. METHODS: Clinical pic

Facts and controversies in mixed connective tissue disease.

Mixed connective tissue disease (MCTD) is a systemic autoimmune rheumatic disease (SARD) characterised by the combination of clinical manifestations of systemic lupus erythematosus (SLE), cutaneous systemic sclerosis (SSc) and polymyositis-dermatomyositis, in the presence of elevated titers of anti-U1-RNP antibodies. Main symptoms of the disease are polyarthritis, hand oedema, Raynaud's phenomenon, sclerodactyly, myositis and oesophageal hypomobility. Although widely discussed, most authors today accept MCTD as an independent entity. Others, however, suggest that these patients may belong to s

Course of mixed connective tissue disease in children.

Mixed connective tissue disease (MCTD) was first reported 25 years ago. This report provides an assessment of the course of juvenile (J) MCTD in 224 patients available in the literature until 1996, including our own 33 patients. Most patients improved and remissions were observed in 3-5% (up to 27%). Among the long-term problems, a loss in joint function was seen in up to 29% of the cases, renal involvement in up to 47%, restrictive lung disease in up to 54% and gastrointestinal manifestations consisting of oesophageal dysmotility in up to 29%. Cerebral involvement was rare but severe. Cardiov

[Nephropathy in patients with mixed connective tissue disease].

The first report of mixed connective tissue disease (MCTD) was described that nephropathy was rare complication of the patients with MCTD. Recently, some authors reported that the nephropathy was 25-50% of patients with MCTD. This report investigated the histopathological findings of patients with MCTD. This study was conducted on 17 patients associated with MCTD. In clinical features, there were six of nephrotic syndrome and two of hematuria and proteinuria. They were normal renal function, expect for one case (case MF). In pathological findings, there were four of membranous nephropathy, two

[Heart involvement in mixed connective tissue disease (MCTD)].

The incidence of cardial involvement in mixed connective tissue disease (MCTD) was investigated by two-dimensional echocardiography. Pericardial manifestation was found in two patients. Mitral valve prolapse was detected in 61% of the patients, which may indicate a specific cardial involvement in MCTD. Myocardial changes were rare.

Mixed connective tissue disease in children and adolescents.

Since the recognition of mixed connective tissue disease in 1972, a small number of pediatric and adolescent patients with this disorder have been described in detail. Four additional patients, two male and two female, are reported in this study. Age range at onset of symptoms was 11 to 18 years. All had arthritis or arthralgia, cervical lymphadenopathy, and hypergammaglobulinemia. Skin rashes and restrictive pulmonary disease were present in three; Raynaud phenomenon, skeletal myopathy, elevation of serum IgE, and neurologic abnormalities were present in two of the four patients. One patient

Symptomatic Sjögren's syndrome in mixed connective tissue disease.

Twelve of 25 patients with mixed connective tissue disease complained of xerostomia and/or ocular symptoms of keratoconjunctivitis sicca. In addition to the clinical features of mixed connective tissue disease, all 12 patients had high titers of antibody to the ribonuclease-sensitive component of the extractable nuclear antigen. Eight patients had both clinical xerostomia and keratoconjunctivitis sicca, one had keratoconjunctivitis sicca and salivary gland enlargement, while there had xerostomia but no ocular complaints. Sjörgren's syndrome was confirmed in all 12 patients by means of Schirmer

Unravelling IPAF, VEDOSS and connective tissue diseases classifications through the mixed connective tissue disease spectrum.

BACKGROUND: Mixed connective tissue disease (MCTD) has long been debated as an early nonspecific phase/symptom of differentiated connective tissue diseases (dCTD), similarly to interstitial pneumonia with autoimmune features (IPAF) and very early diagnosis of systemic sclerosis (SSc) (VEDOSS). OBJECTIVE: We aimed to evaluate the predictive value of IPAF, VEDOSS and dCTD classification criteria variables in MCTD patients. METHODS: We conducted an observational study within the French MCTD cohort. IPAF, VEDOSS and current dCTD classification criteria were used to classify patients. RESULTS: T

Following leads: connecting dysphagia to mixed connective tissue disease.

Mixed connective tissue disease (MCDT) is a rare condition characterised by the presence of high titres of anti-U1 ribonucleoprotein antibodies and selected clinical features of systemic lupus erythematosus, systemic sclerosis and polymyositis/dermatomyositis. Early symptoms are non-specific, including easy fatigability, myalgia, arthralgia and Raynaud's phenomenon. Some reports emphasised the favourable outcome and excellent response to glucocorticoids, but there are contradictory studies reporting worse prognosis. Also, a subset of patients evolve into a clinical picture more consistent with

The diagnosis and classification of mixed connective tissue disease.

The term "mixed connective tissue disease" (MCTD) concerns a systemic autoimmune disease typified by overlapping features between two or more systemic autoimmune diseases and the presence of antibodies against the U1 small nuclear ribonucleoprotein autoantigen (U1snRNP). Since the first description of this condition in 1972, the understanding of clinical manifestations and long-term outcome of MCTD have significantly advanced. Polyarthritis, Raynaud's phenomenon, puffy fingers, lung involvement and esophageal dysmotility are the most frequently reported symptoms among the different cohorts dur

Diagnostic workup for mixed connective tissue disease in childhood.

Raynaud's phenomenon, fatigue and pain (myalgia and arthralgia) are important presenting symptoms of pediatric-onset mixed connective tissue disease. The difficulty is that many adolescent girls complain of pain along with fatigue without evidence for serious disease. However, in patients with Raynaud's phenomenon one should search for evidence of connective tissue diseases. Capillaroscopy could be helpful since capillary changes of the SD-type significantly correlate with future development of scleroderma spectrum disorders. Symptoms of MCTD change in most patients during the disease course:

Panniculitis in a patient with mixed connective tissue disease.

A 51-year-old woman who had been suffering from mixed connective tissue disease (MCTD) for 8 years developed an erythematous rash with pain and tenderness on her left leg. A skin biopsy revealed septal panniculitis with multiple lymphangiectasis. No vasculitis was observed. An increase in her prednisolone dose from 5 mg to 20 mg/day led to an improvement in these lesions. Panniculitis is very rare in MCTD. The clinical significance of panniculitis in MCTD is also discussed.

[Mixed connective tissue disease--etiology, pathogenesis, clinical significance, treatment].

Some patients have features of more than one rheumatic disease and thus do not fit into traditional classification. Patients with combination of clinical finding similar to those of systemic lupus erythematosus (SLE), progressive systemic sclerosis (PSS), polymyositis, rheumatoid arthritis (RA) and with unusually high titers of circulating antinuclear antibody with specificity for nuclear ribonucleoprotein (RNP) are considered to have mixed connective tissue disease (MCTD). The overlap was described by Sharp and colleagues in 1972. During the post 20 years many studies exposed the clinical cor

[Mixed connective tissue disease (Sharp's syndrome)--a case report].

A case of a 45-year-old woman with a prolonged disease (more than 16 years) is reported. The initial manifestations were those of Raynaud's syndrome. Later a very complex combination of symptoms and syndromes developed which included: arthritis, arthralgia, skin eruptions, lymphadenopathy, hepatosplenomegaly, a tendency toward sclerodermic changes of the face and the distal parts of the fingers, Sjogren's syndrome, slight anemia, transient leucopenia, fast ESR, increased serum immunoglobulin G and A levels and presence of high antibody titres against a nuclear ribonucleoprotein.
