Comparison of transbronchial biopsy techniques using needle and forceps biopsies in lung cancer for molecular diagnostics: a prospective, randomized crossover trial.

BACKGROUND: In lung cancer, molecular testing and next-generation sequencing (NGS) are needed to identify therapeutic targets and are increasingly being used in earlier stages of the disease. Despite its longstanding use, it remains unclear whether transbronchial needle aspiration (TBNA) of peripheral lung lesions provides as adequate material for genetic testing as transbronchial forceps biopsies (TBFBs). In this study, we aim to analyze the use of TBNA using median viable cell area (MVCA) as a surrogate parameter to analyze sample quality. METHODS: This prospective single-center study analy

Molecular Assays on Cutaneous Swabs as an Effective, Non-Invasive Diagnostic Technique for Cutaneous Leishmaniasis: Results from a Retrospective Study Conducted in Italy.

BACKGROUND: The case confirmation of CL relies on the direct demonstration of the parasite in clinical specimens from skin tissues. Despite most research efforts focusing on biopsy samples as the preferred diagnostic specimen for the detection of Leishmania spp., the use of non-invasive sampling, such as cutaneous swabs, combined with the use of molecular assays, has shown promising results. METHODS: We conducted a retrospective study aimed at comparing the performance of different invasive and non-invasive diagnostic techniques, employed for the diagnosis of CL, in an Italian tertiary care c

Diagnostic accuracy of a 16S ribosomal DNA gene-based molecular technique (RT-PCR, microarray, and sequencing) for bacterial meningitis, early-onset neonatal sepsis, and spontaneous bacterial peritonitis.

The diagnostic accuracy of a 16S ribosomal DNA (rDNA) gene-based molecular technique for bacterial meningitis (BM), early-onset neonatal sepsis (EONS), and spontaneous bacterial peritonitis (SBP) is evaluated. The molecular approach gave better results for BM diagnosis: sensitivity (S) was 90.6% compared to 78.1% for the bacterial culture. Percentages of cases correctly diagnosed (CCD) were 91.7% and 80.6%, respectively. For EONS diagnosis, S was 60.0% for the molecular approach and 70.0% for the bacterial culture; and CCD was 95.2% and 96.4%, respectively. For SPB diagnosis, the molecular app

The new millennium laboratory: molecular diagnostics goes clinical.

Applications of nucleic acid testing in most areas of the clinical laboratory have increased rapidly. The advantages of nucleic acid testing include enhanced specificity and sensitivity, ease of sample procurement, and more rapid turnaround time compared to conventional laboratory testing methods. However, the cost of testing is usually higher due to the need for additional laboratory space, specialized equipment, safety apparel, and the need for highly trained personnel. Most nucleic acid techniques currently used in a clinical setting can be categorized as either hybridization or amplificati

Loop-mediated isothermal amplification (LAMP) test for diagnosis of uncomplicated malaria in endemic areas: a meta-analysis of diagnostic test accuracy.

BACKGROUND: The global malaria decline has stalled and only a few countries are pushing towards pre-elimination. The aim of the malaria elimination phase is interruption of local transmission of a specified malaria parasite in a defined geographical area. New and improved screening tools and strategies are required for detection and management of very low-density parasitaemia in the field. The objective of this study was to synthesize evidence on the diagnostic accuracy of loop-mediated isothermal amplification (LAMP) test for the detection of malaria parasites among people living in endemic a

[Laboratory diagnosis of mycobacterial infections].

Each year, tuberculosis claims nearly 2 million lives, especially in developing countries. Laboratory diagnosis of mycobacteria remains to be based on conventional approaches such as staining, culture and proportional methods for testing drug susceptibility. In recent years, new molecular methods have been proposed for mycobacterial diagnosis. Whereas the conventional methods for identification and detecting drug resistance are lengthy and cumbersome, the new assays are rapid, highly sensitive and specific and may be used for detecting resistance to antituberculosis drugs. The review describes

A rapid and specific loop-mediated isothermal amplification-lateral flow dipstick (LAMP-LFD) method for detecting Melissococcus plutonius, the causal agent of European foulbrood in honeybees.

Melissococcus plutonius, a gram-positive bacterium, causes European Foulbrood (EFB) disease in honeybee larvae, leading to reduced yields in various countries. This study developed a rapid detection method for M. plutonius using colorimetric loop-mediated isothermal amplification with a lateral flow dipstick (cLAMP-LFD), based on the sequences of gyrase subunit B (gyrB) and superoxide dismutase (sodA) genes, giving three primer sets (gyrB1, gyrB2, and sodA). The cLAMP-LFD using gyrB1 was capable of detecting M. plutonius specifically and sensitively under the optimal condition of 63°C for 50 m

[Molecular diagnosis of non-syndromic craniosynostosis: new challenges for real clinical practice].

Craniosynostoses (CS) are a congenital malformation caused by absence or premature closure of skull sutures. This leads to deformation and impaired growth of skull. Traditionally, syndromic and non-syndromic forms are distinguished. For a long time, it was believed that molecular genetic mechanisms are significant exclusively for syndromic CS. However, clear data on involvement of somatic and germinal mutations in pathogenesis of non-syndromic forms were accumulated in recent years. Currently, there are no unified clinical guidelines defining the strategy for molecular genetic diagnosis for no

Molecular diagnosis of inheritable neuromuscular disorders. Part II: Application of genetic testing in neuromuscular disease.

Molecular genetic advances have led to refinements in the classification of inherited neuromuscular disease, and to methods of molecular testing useful for diagnosis and management of selected patients. Testing should be performed as targeted studies, sometimes sequentially, but not as wasteful panels of multiple genetic tests performed simultaneously. Accurate diagnosis through molecular testing is available for the vast majority of patients with inherited neuropathies, resulting from mutations in three genes (PMP22, MPZ, and GJB1); the most common types of muscular dystrophies (Duchenne and

The future of fungal diagnostics: What's next in laboratory testing for invasive fungal infection?

Invasive fungal infection (IFI) is responsible for millions of deaths globally each year. Prompt diagnosis of IFI is critical, with delay associated with increased mortality. Traditional, culture-based diagnostics are relatively slow and insensitive. Novel molecular techniques, biomarkers and point-of-care tests hope to overcome these limitations. However, these newer methods come with issues of cost, accessibility, and standardisation. This review briefly lays out the current diagnostic landscape, highlights important changes to standards of care, then explores new developments and the future

Systematic Review and Meta-Analysis of Urine-Based Molecular Testing for Urogenital Tuberculosis.

BACKGROUND: Early confirmation of urogenital tuberculosis (UGTB) remains challenging in routine practice because conventional culture is slow and has variable sensitivity. Urine-based molecular testing has emerged as a rapid alternative, but its performance estimates vary across studies. This systematic review evaluated the diagnostic value of urine-based molecular testing, including Xpert Mycobacterium tuberculosis/rifampicin, multiplex polymerase chain reaction and loop-mediated isothermal amplification, for UGTB. METHODS: Risk of bias was assessed using Quality Assessment of Diagnostic Acc
