Myasthenia gravis (MG) is an autoimmune disorder resulting from the production of antibodies against acetylcholine receptors leading to the destruction of the postsynaptic membrane at the neuromuscular junction. In the US there are about 18,000 people with MG. Myasthenia gravis crisis (MGC) is defined as any MG exacerbation necessitating mechanical ventilation. Most patients presenting with MGC have an identifiable risk factor. The diagnosis of MGC should be suspected in all patients with respiratory failure, particularly those with unclear etiology. Acute management of MGC requires supportive

Some myasthenia gravis (MG) patients do not have detectable acetylcholine receptor (AChR) antibodies and have been termed "seronegative" (SNMG) in many previous studies. A high proportion of patients with purely ocular symptoms, ocular MG, are seronegative; this may be because the sensitivity of the assay is insufficient to detect low levels of circulating AChR antibodies and because of intrinsic differences in the ocular muscles that make them more susceptible to circulating factors. Seronegative generalized myasthenia is proving to be heterogeneous both clinically and immunologically. Plasma

Treatment decisions for ocular myasthenia gravis (OMG) should be based on symptomatology. Local, nonpharmacologic treatment of ptosis or diplopia is successful in relatively few patients, and the majority of patients require drug therapy for satisfactory resolution of their symptoms. Response to anticholinesterase agents is variable, but should always be used as the first-line agent or adjunctive therapy in the treatment of OMG. It is unknown whether early treatment of OMG with corticosteroids or other immunosuppressive agents prevents or delays the development of generalized myasthenia, altho

Juvenile myasthenia gravis shares a similar pathophysiologic origin with adult myasthenia gravis, but there are important differences, mostly relating to epidemiology, presentation, and therapeutic decision making. Gender ratios and the proportion of seropositive patients differ in the pre- and postpubertal age groups. The diagnostic evaluation is similar to that in adults, although special techniques are sometimes necessary to perform single-fiber electromyography in younger patients. Therapeutic decisions in affected children and adolescents are complicated by the greater long-term consequen

Myasthenia gravis (MG) characteristically involves ocular; bulbar, and proximal limb muscles. MG with predominant distal limb involvement is uncommon. We present a case of MG with distal, asymmetrical, upper limb involvement, seven years after disease onset.

BACKGROUND: Myasthenia gravis, an autoimmune disease of young women, is due to the dysfunction of neuromuscular transmission. The newborn of a myasthenic mother inconstantly presents a transitory neonatal myasthenic syndrome. Maternal aggravation, or even myasthenic crisis with respiratory failure, can occur in the first three months post-partum.
CASE REPORT: Mrs. S., para two without appreciable medical history, delivered normally a boy weighing 4 kg with an Apgar score of 10/10. At 3 h of life the newborn was admitted to the neonatal care unit for grunting and axial hypotonia. Diagnoses of

Ten cases of thymoma associated myasthenia were seen in the last 7 years. They constitute 17.2% of all cases of Myasthenia gravis seen during the same period. Five of these patients presented in the third decade, all of them presenting with acute generalised Myasthenia (Osserman stage-III). Two patients presented with tumour related symptoms of chest pain, cough and dyspnea both of them having large, invasive and partially resectable tumours. Five patients underwent complete thymectomy and 3 patients underwent partial resection of tumour. Two patients underwent radiotherapy subsequently. Histo

Stucture of acetylcholine receptor protein (AChR) purified from Electrophorus electricus (eel) by affinity chromatography is described. AChR is detected in extracts from human muscle, rat muscle, and rat thymus. Rats immunized with eel AChR develop humoral antibodies, a small fraction of which recognize AChR from rat muscle. Rats immunized with AChR exhibit myasthenia, but those immunized with denatured AChR do not. Immunoglobulin fraction of antisera to eel AChR can block the activity of AChR in electroplaques. Sera from patients with myasthenia gravis contain antibodies to AChR from human mu

Out of 700 patients operated on account of myasthenia gravis, 144 cases with tumours (20.57%) were evaluated. Classification took place in five groups: 1. Thymoma and myasthenia gravis; 2. Thymic cysts and myasthenia gravis; 3. Thymoma and latent myasthenia gravis; 4. Paraneoplastic myasthenia-syndrome; 5. Thymoma, myasthenia gravis and myasthenia-syndrome. Up-to-date classification of thymomas is: epithelioma with minimal, marked or overwhelming lymphatic reaction. Dark-cell and light-cell epitheliomas equally associate with myasthenia. Concerning prognosis, location of the tumour to thymic c

Thymoma is the most common tumor of the anterior mediastinum. Approximately 20%-30% of patients with a thymoma develop myasthenia gravis (MG), and an additional one third may possess positive acetylcholine receptor (AChR) antibodies without MG. Historical estimates suggest that thymoma associated MG (TAMG) represents roughly 15%-20% of MG patients, although the proportion of TAMG has been decreasing. Virtually all TAMG patients are seropositive for AChR antibodies. In comparison to MG without thymoma, TAMG is mostly generalized and more refractory to treatment. Chest CT with contrast is the im

Myasthenia gravis is an autoimmune neurological disease characterized by a disorder of nerve transmission at the neuromuscular junction. In this disease, antibodies attack acetylcholine receptors and their associated molecules, disrupting nerve transmission and impairing normal muscle contraction. Myasthenia gravis is diagnosed through physiological and neurological examinations, blood antibody tests, and electronic methods. Various approaches are used for symptom management, including immunosuppressive drugs, acetylcholinesterase inhibitors, and thymectomy. Previous literature has focused pri

MG epidemiology may provide clues to disease etiology and pathogenesis. It is crucial for health planning and to estimate societal needs for health care resources. Recent high-quality studies have found an annual MG incidence of approximately 20 per million person-years, and a prevalence of approximately 25 per 100,000 population. Geographical variation reflects differences in case finding and diagnostic tools, but also a multitude of genetic and environmental etiological and risk factors. MG prevalence has increased in recent years because of more elderly people in the general population, imp

Autoimmune Myasthenia Gravis (MG) is a disease characterized by fatigable muscle weakness and autoantibodies. It can be divided by the presence of serum autoantibodies into two major categories where Immunoglobulin G (IgG) against either the acetylcholine receptor (AChR), or muscle specific kinase (MuSK) causes fatigable muscle weakness. The clinical relevance of Low-density lipoprotein-receptor related protein-4 IgG (LRP4) is debated. These antibodies disrupt neuromuscular transmission via different mechanisms: AChR antibodies, mostly of IgG1 and IgG3 subclass, can activate complement leading

The role of non-pharmacological management in MG is currently underdeveloped and underutilized. Exercise training has been shown to be both feasible and beneficial in improving function and strength in individuals with MG. Observational studies indicate that physical activity is associated with reduced perceived fatigue. Barriers to exercise persist in MG, and should be addressed by healthcare professionals. Maintaining or increasing muscle strength and endurance as well as cardiorespiratory fitness should be prioritized regardless of current level of function. In the general population, exerc

Myasthenia Gravis (MG) often affects young women, in whom reproductive issues are of importance, including the possibility of MG exacerbation during pregnancy, potential obstetric risks in pregnancy and treatment-related risks to the fetus. Apart from pregnancy, hormonal changes throughout a woman's lifespan may also affect MG symptoms. In this chapter, we discuss the role of sex hormones in MG and the disease burden in women. We also review the effects of pregnancy on MG, the evidence regarding pregnancy outcomes and current recommendations regarding MG treatments during pregnancy.

RATIONALE: Immunoglobulin has long been used to treat neuroimmunological conditions, including myasthenia gravis (MG). However, the evidence supporting its benefits in MG remains uncertain.
OBJECTIVES: To assess the benefits and harms of immunoglobulin in people with generalized MG. Specifically, we aimed to compare 1) intravenous immunoglobulin (IVIg) versus subcutaneous immunoglobulin (SCIg), and 2) immunoglobulin administered via either route (irrespective of treatment duration, dosage, and regimen) versus no treatment, placebo, plasma exchange (PLEX), corticosteroids, acetylcholinesterase

RATIONALE: Myasthenia gravis (MG) is an autoimmune disease which causes muscle weakness due to disruption in neuromuscular transmission. Rituximab is a medication increasingly used in the treatment of MG, but its potential benefits, and optimal use in terms of patient subgroup and dosing, are unclear. It is important to bring together high-quality evidence to determine how rituximab would be best used in treatment algorithms for MG.
OBJECTIVES: To assess: - the effects of rituximab (including biosimilar variants) for the treatment of MG in adults; and - the benefits and harms of rituximab in
