BACKGROUND: Melanotic neuroectodermal tumors of infancy are rare, benign neoplasms predominantly affecting the craniofacial region, and they are typically managed through resection with minimal need for reconstruction. However, in exceptional cases, larger or more complex tumors may necessitate open craniofacial approaches, with limited literature detailing the surgical strategies for these scenarios. The authors report a distinctive case of an aggressively expanding melanotic neuroectodermal tumor of infancy in a pediatric patient, describing their approach for the tumor's resection.

We present the case of a woman referred to our department at 34 weeks of pregnancy with a fetal ultrasonographic scan showing a mass that had developed within the right maxilla with invasion of the orbit. A retrospective examination showed that this tumor had been present since the 12th week of pregnancy. At 39+4 weeks of gestation, a boy was born. He presented a black firm aspect in the maxilla. A computed tomographic scan and magnetic resonance imaging revealed a soft tissue swelling over the right maxilla, extending into the orbit but without invasion of the globe. Surgical biopsy confirmed

BACKGROUND: Melanotic neuroectodermal tumor of infancy (MNTI) is a rare, typically benign but locally aggressive tumor that most commonly affects the maxilla in infants under 1 year of age. Although surgical resection is the standard treatment, cases requiring total maxillectomy and immediate reconstruction have rarely been reported.
CASE PRESENTATION: Here, we report a 6-month-old male infant who presented with a rapidly enlarging maxillary mass. Imaging revealed a 60 × 57 × 50 mm lesion with extensive bone destruction and displacement of the primary teeth. A biphasic tumor composed of small

We report an infant melanotic neuroectodermal tumor in the anterior part of the upper gingiva, which was associated with high urinary excretion of vanilmandelic acid. Serum levels of alpha-fetoprotein were abnormally high (200 ng/ml). They returned to normal after removal of the tumor (10 ng/ml). The tumor was characterized histologically and ultrastructurally by the presence of melanocytic and astrocytic cells containing glial fibrillary protein. Intracellular alpha-fetoprotein was demonstrated by the immunoperoxidase method (PAP) in scattered foci of interstitial cells.

Extra-testicular scrotal masses may be seen in young children, spanning a broad spectrum of lesions from benign entities to aggressive malignancies. Though rare, melanotic neuroectodermal tumor of infancy (MNTI) is a very important differential diagnosis of scrotal masses in infants due to its close histologic resemblance to more aggressive malignant small round cell tumors involving this region. An 18-month-old baby presented with a 10-month history of a scrotal mass, which was surgically excised and initially misdiagnosed as rhabdomyosarcoma, with a plan for post operative chemotherapy. Melanotic neuroectodermal tumor of infancy or melanotic progonoma is an uncommon tumor, presenting in infants. The epididymis and testis are rare sites of occurrence. This tumor can be confused with round cell tumor (RCT). Since the treatment modality and prognosis of melanotic progonoma and RCT are very different, it is imperative to make a correct diagnosis.

Embryonal tumor with abundant neuropil and true rosettes (ETANTR) is a rare variant of central nervous system primitive neuroectodermal tumor occurring exclusively in the pediatric population. We report a unique case of a 6-month male child presenting with a large intraventricular lesion. Histological examination revealed a tumor composed of primitive neuroectodermal cells in dense aggregates, interspersed by hypocellular areas containing small round cells widely dispersed in neuropil-like material. Few ependymal and occasional ependymoblastic rosettes were appreciated. Focal melanotic neuroep

February 2005. Case report of a 10-month-old boy with a large tumor located in the pineal gland, consisting of glia, ganglion cells, pigmented neuroepithelium and striated muscle, without immature components. The combination of neuroectodermal and mesenchymal constituents includes entities as pineal anlage tumor (melanotic neuroectodermal tumor of infancy, MNTI), ectomesenchymoma, medullomyoblastoma, and teratoma in the differential diagnosis. Lack of immature elements in this case, however, eliminates ectomesenchymoma and medullomyoblastoma from the differential diagnosis. Retinal anlage tumo

Melanotic neuroectodermal tumor of infancy is a rare maxillofacial tumor, usually presenting within the first year of life, often a benign locally aggressive lesion, with wide local excision providing the best opportunity of cure. In past reports, direct tissue visualization guided resection, with occasional need for reoperation to clear margins. In this case report, a method is presented to achieve clear margins in surgery using virtual surgical planning and surgical stealth navigation.

Embryonal tumors are the most common brain tumors in infants less than 36 months. Histologically characterized as undifferentiated small, round cell tumors with divergent patterns of differentiation, these include medulloblastoma, the most common form of embryonal tumor, as well as supratentorial primitive neuroectodermal tumor, medulloepithelioma, ependymoblastoma, medullomyoblastoma, melanotic medulloblastoma, and atypical teratoid/rhabdoid tumor. All are similarly aggressive and have a tendency to disseminate throughout the central nervous system. Because of efforts to avoid craniospinal ir

BACKGROUND: The pineal anlage tumor is a very infrequent malign neoplasm. Even though it has been documented in literature, it is not listed yet in the World Health Organization's last nervous system classification (2007). It is a primitive pineal tumor with neuroepithelial and ectomesenchyme differentiation. Due to its low frequency, the understanding of its biological behavior and a suitable treatment are incomplete. In a search performed in PubMed with the term pineal anlage tumor, only seven informed cases were identified between 1989 and 2011.
CLINICAL CASE: An 8-month-old infant was bro

A wide spectrum of benign and malignant tumors of peripheral nervous system origin can arise in the mediastinum. These neoplasms are more frequent in the posterior mediastinum and can develop from peripheral nerves, sympathetic and parasympathetic ganglia, and neural tube embryonic remnants. The clinicopathologic features of mediastinal schwannomas, melanotic schwannomas, neurofibromas, ganglioneuromas, granular cell tumors, malignant tumors of peripheral nerve sheath origin, malignant melanocytic tumors of peripheral nerve sheath origin, neuroblastomas, ganglioneuroblastomas, and pigmented ne

Ependymoblastoma developed in a 28-month-old girl whose epileptic mother took diphenylhydantoin and methylphenobarbitone throughout pregnancy. The child was also shown to be a genetic carrier for ornithine transcarbamylase deficiency, an x-linked inborn error of urea cycle metabolism. The possibility of transplacental carcinogenesis should be considered, as other juvenile embryonic tumors such as neuroblastoma, melanotic neuroectodermal tumor, and mesenchymoma have been reported in offspring after diphenylhydantoin use by the mother during pregnancy.

A primitive tumor of the pineal region in a 9-year-old girl led to death after 5 months of increased intracranial pressure with occlusive hydrocephalus. Histological investigation including immunocytochemistry revealed a broad spectrum of neuroectodermal differentiation with neuroblastic, neuronal, ependymal and retinoblastomatous features. Melanotic epithelial formations were similar to those of the so-called retinal anlage tumor and of the developing pineal gland. Striated muscle fibers indicated additional ectomesenchymal differentiation reminiscent of the close topographical relationship b

Pineal anlage tumor (PAT) is an extremely rare tumor of paediatric population. It is considered as a subtype of pineoblastoma having ectomesenchymal/ rhabdomyoblastic and cartilaginous differentiation. PAT is presumed to have an aggressive behaviour with propensity for craniospinal fluid (CSF) spread due to histologically resemblance with pineoblastoma, thus requiring intensive multimodality treatment with craniospinal irradiation and chemotherapy. Here we report a case of PAT in a 35 years old lady along with clinical and pathological review. To the best of our knowledge only less than ten ca

A one year old boy was found to have a large tumor encompassing the pineal region and extending towards the third and lateral ventricles and quadrigeminal plate. The tumor was composed mostly of small, undifferentiated cells. Some small cells were arranged in Flexner-Wintersteiner rosettes and a few displayed fleurettes. The tumor contained immature cartilage and skeletal muscle and numerous clusters of pigmented epithelial-like cells which, histologically, resembled those found in melanotic neuroectodermal tumors of infancy (retinal anlage tumors) and retinal or ciliary epithelium. As dermatopathologists, we routinely diagnose melanocytic nevi, melanomas, and occasionally melanocytomas in our daily clinical practice. However, it is now clearly established that the presence of melanocytic differentiation in a tumor does not necessarily indicate any of the aforementioned diagnoses. Tumors such as clear cell sarcoma, malignant melanotic nerve sheath tumor, PEComa, melanotic neuroectodermic tumor of infancy, and even certain translocation-associated renal cell carcinomas all share the common characteristic of melanin synthesis. Over the past two decades, with the advent of m

Melanotic neuroectodermal tumour of infancy (MNTI) is a rare benign tumour of neural crest origin that was first described by Krompecher in 1918.1 It is predominantly found in infancy, with about 92% of cases below the age of 12 months and 82% below the age of 6 months. The predominant site of origin is in the premaxilla though it is reported at other sites also including the skull, the mandible, the epididymis and the brain.2 The lesions often have areas of bluish discolouration on the surface and are characterised by displacement of the involved tooth bud and local aggressiveness. This article reviews odontogenic and developmental oral lesions encountered in the gnathic region of pediatric patients. The process of odontogenesis is discussed as it is essential to understanding the pathogenesis of odontogenic tumors. The clinical presentation, microscopic features, and prognosis are addressed for odontogenic lesions in the neonate (dental lamina cysts/gingival cysts of the newborn, congenital (granular cell) epulis of the newborn, melanotic neuroectodermal tumor, choristoma/heterotopia, cysts of foregut origin), lesions associated with unerupted/erupting teeth (hyperplast
