Neuroendoscopic aspiration of hematocephalus totalis: technical note.

OBJECTIVE AND IMPORTANCE: Massive intraventricular hemorrhage requires aggressive and rapid management to decrease intracranial hypertension. The amount of intraventricular blood is a strong prognostic predictor, and its fast removal is a priority. Neuroendoscopy may offer some advantages over more traditional surgical approaches. We describe here the technical details and clinical outcomes of the neuroendoscopic management of massive tetraventricular hemorrhage in 25 consecutive patients, highlighting the potential pitfalls and the advantages of the technique. CLINICAL PRESENTATION: Twenty-f

Purely neuroendoscopic transventricular management of cystic craniopharyngiomas.

INTRODUCTION: This paper reports our series of cystic craniopharyngiomas treated using a purely neuroendoscopic approach. METHODS: Drainage and wide marsupialization into CSF spaces (cysto-ventriculo-cisternostomy) were achieved to ensure mass effect control, continuous dilution, and reabsorption of the cyst's fluid. A steerable, flexible neuroendoscope and a pre-coronal approach were used in 7 patients harboring intra/paraventricular cysts (4 primary and 3 recurrent). RESULTS: These surgical goals were achieved in all but one case and no procedure-related complications were observed. During

Neuroendoscopic biopsy of tectal glioma: a case report.

OBJECTIVE AND IMPORTANCE: A type of intrinsic dorsal midbrain glioma known as benign tectal glioma can cause obstructive hydrocephalus. Because of its slow progress and relatively good prognosis, initial treatment should be cerebrospinal fluid diversion and biopsy. CLINICAL PRESENTATION: We report a 24-year-old man with obstructive hydrocephalus from a tectal glioma, who was admitted to the hospital for malfunction of a shunt placed 16 years previously. INTERVENTION: Magnetic resonance imaging demonstrated enlargement of the dorsal midbrain associated with increased signal intensity on T2-we

Navigation guidance in neuroendoscopic management of complex hydrocephalus.

Patients with multiloculated hydrocephalus have multiple, separate abnormal CSF collections with no communication between them. Causes include complications of neonatal meningitis, germinal matrix hemorrhage in neonates, head trauma, and intracranial surgery. Endoscopic fenestration with shunt insertion is a safe and less invasive technique as the initial treatment. In this video, the authors demonstrate a few cases in which electromagnetic navigation was used with a stylet inserted through the operating endoscope to guide the surgeon. Modalities such as insertion of intraventricular contrast

Neuroendoscopic surgery for neuroendocrine cancer of the skull base.

BACKGROUND: Neuroendocrine cancer is an extremely rare malignant tumor that originates from the nervous and the endocrine systems. This type of cancer can exist in almost any organ, although it is mainly found in the lung and gastrointestinal system according to current clinical reports. It is rarely found in the skull base, including primary and secondary neuroendocrine cancers. Therefore, diagnosis and treatment face a huge challenge. CASE DESCRIPTION: A 50-year-old female patient was introduced. She was admitted to the hospital due to persistent headaches, accompanied by nausea, vomiting,

[Neuroendoscopic surgery--current state and future development].

Recently, endoscopic neurosurgery progressed drastically, because of improvement of optical imaging and surgical instruments. Neuroendoscopic surgery reflects the tendency of modern neurosurgery to aim towards minimum invasiveness: that is, access and visualization through the narrowest practical surgical field and get maximum effectiveness. Transventricular approach using neuroendoscope allows the treatment of several pathologies inside the ventricular system, such as non-communicating hydrocephalus and intra/paraventricular tumors or cysts. Endoscopic endonasal trans-sphenoidal surgery as en
