Working the night shift: a necessary time for training or a risk to health and safety?

The European Working Time Directive (EWTD) limits excessive night shifts and restricts the working week to no more than 48 hours. The underlying rationale is to minimise the health risks to all workers. Here we debate the impact of night rotas for doctors-in-training on patient safety and medical education; when the EWTD was agreed these topics may not have been considered, either systematically or objectively. The impacts of diurnal rhythms on human functions affect all night workers, but the nature of rostered medical and surgical work has little precedent in other industries or even in the

Can Technology Use Alleviate Workforce Needs in Finnish Assisted Living Services? A Convergent Mixed-Methods Study of Automatic Medicine Dispensers and Night-Time Monitoring.

BACKGROUND: Workforce needs are growing in older people care, exacerbated by the aging population. Technological solutions offer potential means to address this challenge. However, their effects on work and worktime management remain unclear, especially in the setting of assisted living services. Our aim was to assess the potential of two established technologies, automatic medicine dispensing and night-time monitoring, in alleviating workforce needs in Finnish assisted living facilities. METHODS: The effects of the two technologies on workforce needs were studied using a convergent mixed-met

Coinfections associated with human immunodeficiency virus infection: workshop 1A.

BACKGROUND: When asked, a significant number of patients report having experienced night sweats. Those who do are more likely to report decreased physical health, mental health, and quality of life. In most cases the cause of night sweats is unknown. We therefore do not know how much to worry about patients with this symptom. The present study examined associations between night sweats and mortality. METHODS: We used logistic regression and proportional hazards analyses to investigate potential predictors of mortality, including night sweats reported at baseline, among 2 different cohorts of

Nocturnal aspects of narcolepsy with cataplexy.

Even though the most impressive manifestation of narcolepsy is excessive sleepiness, paradoxically a significant number of patients have trouble sleeping at night. A wide array of alterations can affect the night-time sleep of a narcoleptic patient, and the aim of this review is to increase awareness on this issue, thereby enhancing the care of narcoleptic patients by more specific approaches to their disturbed night sleep. This review covers a broad variety of nocturnal sleep features in narcolepsy. Starting from animal models and the clinical features of patients, the paper then discusses th

[Effect of birth timing on perinatal mortality].

Several reports show increased risk of perinatal/neonatal mortality for night time, weekend and holiday births. Intrapartum-related deaths were attributed to physical and mental fatigue, reduced availability of skilled and experienced staff and inadequate supervision by senior staff. The incidence of intrapartum and neonatal deaths related to intrapartum asphyxia is regarded to be a sensitive indicator of the quality of care during labor. Continuity of care is regarded as a major element in providing quality medical care. Several reports indicate the occurrences of potentially preventable adve

Unintended changes to the occlusion following the provision of night guards.

A review of the literature on night guards left questions unanswered about the risk and possible causes of occlusal derangement linked to night guard design and use. Cases are presented where night guard use resulted in anterior open bite and associated occlusal derangement. The aetiology of the occlusal changes observed is discussed. It is concluded that adverse occlusal effects linked to the use of night guards may be found to be more common than anticipated from existing literature. Patient follow-up is essential to monitor the use and effects of night guards, and regardless of appliance ty

The effect of cycled lighting in the intensive care unit on sleep, activity and physiological parameters: A pilot study.

Patients in intensive care suffer from severe illnesses or injuries and from symptoms related to care and treatments. Environmental factors, such as lighting at night, can disturb patients' circadian rhythms. The aim was to investigate whether patients displayed circadian rhythms and whether a cycled lighting intervention would impact it. In this pilot study (N=60), a cycled lighting intervention in a two-bed patient room was conducted. An ordinary hospital room functioned as the control. Patient activity, heart rate, mean arterial pressure and body temperature were recorded. All data were col

Use of Family Photographs Reduces Restlessness in Neurocritical Care Patients.

BACKGROUND: Patients admitted to the neuroscience intensive care unit often experience varying states of confusion and restlessness. The purpose of this study was to examine restlessness in acutely confused patients through use of familiar photographs. METHODS : This randomized prospective pilot study placed family photographs (photos) on the bedrail of confused patients during the night shift (8 pm to 4 am ) in a neuroscience intensive care unit. Wrist actigraphy was used to examine restlessness when patients were turned to face the photos versus when they were not facing the photos. RESULTS:

Multi-night measurement for diagnosis and simplified monitoring of obstructive sleep apnoea.

Substantial night-to-night variability in obstructive sleep apnoea (OSA) severity has raised misdiagnosis and misdirected treatment concerns with the current prevailing single-night diagnostic approach. In-home, multi-night sleep monitoring technology may provide a feasible complimentary diagnostic pathway to improve both the speed and accuracy of OSA diagnosis and monitor treatment efficacy. This review describes the latest evidence on night-to-night variability in OSA severity, and its impact on OSA diagnostic misclassification. Emerging evidence for the potential impact of night-to-night va

Midnight medicine: Empowering night shift to treat opioid use disorder (OUD).

Hospitalizations for opioid use disorder (OUD) are rising, yet overnight clinicians rarely receive targeted OUD education. Our interprofessional initiative implemented two overnight training sessions specifically for night-shift physicians, nurses, pharmacists, and advanced practice providers, emphasizing buprenorphine initiation, stigma reduction, harm reduction, and patient-centered communication. Participants in the program demonstrated increased confidence in OUD management and a self-reported rise in overnight buprenorphine initiation, highlighting the impact of this tailored night-shift

Experiences and perceptions of nurses working night shift: a qualitative systematic review.

OBJECTIVE: The objective of this review was to examine the available evidence on the experiences and perceptions of nurses working the night shift within any specialty in the acute care, subacute, or long-term care setting. INTRODUCTION: Nurses are required for around-the-clock patient care. Night shift nurses can experience detrimental effects because of their work hours, which disrupt their normal circadian rhythm. Understanding nurses' experiences and perceptions when working night shift will facilitate the development of strategies to minimize the potential negative effects of working at

[Home mechanical ventilation: indications and pathophysiological limitations].

INTRODUCTION: Domiciliary assisted ventilation (DAV) may be undertaken invasively or non-invasively. Non-invasive DAV is used for patients suffering from alveolar hypoventilation due to restrictive pathology. Invasive DAV is reserved for "indications of necessity" that is when non-invasive ventilation is contraindicated due to the absence of adequate cough and for alveolar hypoventilation leading to hypercapnoea during spontaneous ventilation. STATE OF THE ART: The main pathophysiological limitation to non-invasive ventilation is the interference of the glottis. In this mode the glottis impos

The impact of time and day on the presentation of acute coronary syndromes.

BACKGROUND: The frequency of acute myocardial infarction (AMI) peaks on Mondays and in the mornings. However, the distribution of the types of acute coronary syndromes (ACS), including unstable angina (UA), has not been systematically evaluated. HYPOTHESIS: The distribution of the types of ACS and clinical presentations varies by time and day of admission. METHODS: A retrospective cohort study was conducted in 1,946 consecutive nontransfer ACS admissions (1999-2004) to a tertiary-care academic center to assess presenting clinical variables in patients admitted on days versus nights (6 P.M.-6

Escalation of Care: Association of Weekend and Nighttime Postoperative ICU Admission with Poor Outcomes After Gastrointestinal Cancer Surgery.

BACKGROUND: Postoperative intensive care unit (ICU) admission represents an important escalation of care after surgery. We evaluated the association between ICU admission timing and outcomes after gastrointestinal (GI) cancer surgery. METHODS: Adults undergoing GI cancer surgery with postoperative ICU care were identified in Epic Cosmos from 2016 to 2025 and classified as weekday, weekend-day, weeknight, or weekend-night admission. Outcomes included complications during the index hospitalization and within 30 days, 30-day mortality, readmission, discharge home, and prolonged length of stay (L

Nocturnal positive pressure ventilation via nasal mask.

Five patients with chronic respiratory failure from neuromuscular disease and symptomatic worsening nocturnal hypoventilation were treated with nocturnal ventilation. Home ventilation at night was provided by a volume-cycled positive pressure ventilator attached to a nasal mask originally designed to administer nasal continuous positive airway pressure (CPAP) for obstructive sleep apnea. The device was well tolerated. Symptoms of headache, insomnia/somnolence, and impaired intellectual capacity rapidly disappeared with nocturnal ventilatory support. Daytime arterial PO2 and PCO2 improved after

Night eating in Cushing's disease: Disentangling psychological, sleep, and endocrine contributions.

OBJECTIVE: Cushing's disease (CD) is characterized by chronic exposure to excess cortisol and disruption of circadian cortisol rhythms. Given cortisol's role in affective regulation and circadian synchronization, eating disturbances-particularly those linked to mood and sleep-may represent an underexplored aspect of CD. We investigated night eating symptoms and chronotype in CD and examined their clinical, psychological, sleep-related, and endocrine correlates. METHODS: Ninety-eight participants were included (22 active CD, 33 remission, 43 healthy controls). Night eating symptoms were assess

Nonelective surgery at night and in-hospital mortality: Prospective observational data from the European Surgical Outcomes Study.

BACKGROUND: Evidence suggests that sleep deprivation associated with night-time working may adversely affect performance resulting in a reduction in the safety of surgery and anaesthesia. OBJECTIVE: Our primary objective was to evaluate an association between nonelective night-time surgery and in-hospital mortality. We hypothesised that urgent surgery performed during the night was associated with higher in-hospital mortality and also an increase in the duration of hospital stay and the number of admissions to critical care. DESIGN: A prospective cohort study. This is a secondary analysis of

The Experiences of the Nursing Professional Development Practitioner on the Night Shift-A Qualitative Study.

BACKGROUND: The nursing shortage continues to grow and is expected to intensify in the near future. Health care organizations need to implement strategic ways to support nurses. First-year new graduate nurses have a high turnover rate. It is critical to have educational support on the night shift, where most new nurses begin their careers. OBJECTIVE: The study aimed to gain insight into how nursing professional development (NPD) practitioners or nursing educators who work the night shift perceive their roles and explore their experiences. Limited literature exists on having NPD on the night s
