Outcome of plate fixation of olecranon fractures.

OBJECTIVES: To evaluate the functional outcome of plate fixation for displaced olecranon fractures, both simple and comminuted. DESIGN: Retrospective patient, chart, and radiographic review. SETTING: Academic teaching hospital. PATIENTS: Twenty-five patients who underwent plate fixation of displaced olecranon fractures by two surgeons were independently reviewed at an average follow-up of thirty-four months (range, 15-69 months). MAIN OUTCOME MEASUREMENTS: Physical capability was assessed by measuring range of motion and isometric elbow strength. Patient-rated outcomes were evaluated using…

Fractures of the olecranon and radial neck in children.

Over one half of radial neck fractures are associated with other injuries about the elbow. Most radial neck fractures can be treated with closed reduction or with percutaneous probing, even if the other associated injuries require open reduction. The results are good, although radiologic imperfections may be seen after healing. In the older child, olecranon fractures that have significant displacement usually require open reduction with internal fixation.

Olecranon fractures: a review of 100 cases.

Olecranon fractures in the adult are a common upper extremity injury. There is still no universally accepted classification of these fractures and no uniformity of treatment. Results stated vary considerably, and support can be found in the literature for any of several forms of operative treatment. A review of the literature on olecranon fractures is presented. A system of classification of these fractures based on lateral roentgenograms is proposed; Type I, IIa, IIb, and III (depending on location in the olecranon). An assessment of the results of treatment of 100 patients with such fracture…

Loop Anchor Tension Band Fixation for Olecranon Fractures and Chevron Olecranon Osteotomy.

Although tension band wiring is effective for treating olecranon fractures, it is associated with a high reoperation rate because of hardware-related problems. The loop anchor tension band technique is a novel modification of the Arbeitsgemeinschaft für Osteosynthesefragen (AO)-modified tension band wiring technique. This technique is suitable for treating olecranon fractures without severe comminution and olecranon osteotomy. In the current case series, 2 intramedullary 1.25-mm Kirschner wires (K wires) were inserted as temporary fixation into the ulnar shaft from the olecranon after anatomic…

Posterior Olecranon Fracture Dislocations in Adults: A Systematic Review.

Posterior olecranon fracture dislocations (POFDs) were considered posterior Monteggia lesions, which were less described in the literature. The purpose of this study was to provide a systematic review of the diagnosis, treatment, prognosis, and complications of POFDs in adults. A systematic review was performed to identify all relevant studies on the POFDs in the PubMed, Web of Science, Embase, and MEDLINE databases. The methodological quality of the studies was scored using the Methodological Index for Non-Randomized Studies (MINORS). A total of 117 patients were identified in the nine studie…

Trans-olecranon fracture dislocations: How should we treat them?

BACKGROUND: Trans-olecranon fracture dislocations are the least frequent complex elbow instability. Proper surgical treatment should be performed to avoid postoperative complications. METHODS: A retrospective design study was performed. Patients that suffered from this injury, treated at our center from 2010 to 2016 were included. Fifteen patients were analyzed. Functional results were measured using DASH, MEPS and VAS scores. Average time from injury to first surgical treatment was 4.87 days. Radial head fracture was present in seven cases and coronoid process in three patients. Most frequen…

Outcome after olecranon fracture repair: Does construct type matter?

PURPOSE: This study compares clinical and functional outcomes of patients with displaced olecranon fractures treated with either tension band wiring (TBW) or a hook plate construct. METHODS: We performed a retrospective review of olecranon fractures operatively treated with either TBW or plate fixation (PF) using a hook plate over a 7-year period. Patient demographics, injury information, and surgical management were recorded. Fractures were classified according to the Mayo system. Measured outcomes included range of elbow motion, time to union, and development of postoperative complications.…

Ipsilateral olecranon and distal radius fracture: A case report.

INTRODUCTION: Concomitant ipsilateral olecranon and distal radius fracture are rare injuries. Their clinical presentation is unusual and investigation and management is poorly described. PRESENTATION OF CASE: We present a 55-year-old woman patient who fell off sustaining a concomitant distal radius and olecranon fracture in the same extremity. On examination, there was gross swelling of the proximal and distal forearm and no neurovascular deficit. Radiographs confirmed distal radius and olecranon fracture. Patient was treated with open reduction and anatomic locking plate for olecranon and a …

Median nerve palsy after operative treatment of olecranon fracture.

Olecranon fracture is not an uncommon fracture in clinical practice. Simple olecranon fracture usually heals quite well without any types of iatrogenic complications. Despite close proximity of the fracture to the nerve, median nerve palsy after operative treatment of olecranon fracture is a rare complication. To the authors' knowledge, this complication has not been previously reported in the Thai or English literature. The authors present a patient who had median nerve palsy after tension-band wiring for olecranon fixation. Intraoperative finding revealed that the median nerve was injured by…

Outcome of olecranon osteotomy in the trans-olecranon approach of intra-articular fractures of the distal humerus.

BACKGROUND: The trans-olecranon approach has been suggested to improve the visualization of complex intra-articular distal humerus fractures. Significant osteotomy complications have prompted a search for alternative approaches. The purpose of this series was to study the outcome of the olecranon osteotomy in terms of union and complications and the ultimate outcome of the fracture. METHODS: Ninety-four patients with intra-articular fractures of the distal humerus (type C3) were treated by open reduction and internal fixation using the trans-olecranon approach. The patients were followed from

Trans-olecranon fracture-dislocation of the elbow in children.

OBJECTIVES: Trans-olecranon fracture-dislocations are rare in children. To our knowledge, only 12 cases have been described in children till now and the treatment strategy for this injury in children remains unclear. To provide a clear clinical description and more accurate treatment options, we retrospectively reviewed cases with this kind of injury in our institution. METHODS: From 2002 to 2019, eleven cases diagnosed with trans-olecranon fracture-dislocation of the elbow were identified, and their medical charts and radiographs were obtained. All patients underwent open reduction and inter…

Adult Monteggia and Olecranon Fracture Dislocations of the Elbow.

Monteggia fractures and olecranon fracture dislocations represent complex injuries with distinct patterns of bony and soft tissue involvement. Fractures of the proximal ulna and olecranon process may lead to disruption of the proximal radioulnar joint and/or ulnohumeral joint. The keys to treatment are recognition of the pattern of injury and formation of an algorithmic surgical plan to address all components of the injury process. Complications are common and may be related to the injury spectrum itself and/or inadequate fracture alignment or fixation.

Fracture separation of the olecranon ossification center in adults.

Traumatic disruption of an incompletely fused olecranon physis in adults has been reported on only one occasion by O'Donoghue in 1942, while stress fractures of the olecranon physis have been well-recognized and reported. A series of three adult males with traumatic disruption of an incompletely fused olecranon physis was studied. Two were competing in football at the time of the injury, while a third patient sustained the injury during a fall. All of our patients were former throwing athletes and all sustained a direct blow to the dominant elbow at the time of the fracture. In our study, as w

Olecranon Fractures: Patterns, Fixation Types, and Outcomes.

BACKGROUND: Prominent metalwork remains a common source of postoperative discomfort following fixation of olecranon fractures and often leads to elective implant removal despite successful union. This study aimed to assess the incidence and pattern of symptomatic hardware prominence among different fixation methods involving tension band wiring (TBW), plate fixation, and suture constructs in a tertiary trauma center. METHODS: A retrospective review was conducted of adult patients who underwent operative fixation for olecranon fractures at the Queen Elizabeth Hospital, Birmingham, between Janu…

Compression Screw Fixation for Pediatric Olecranon Fractures.

BACKGROUND: Pediatric olecranon fractures can be treated with several methods of fixation. Though postoperative outcomes of various fixation techniques, including cannulated intramedullary screws, have been described in adults, functional and radiographic outcomes of screw fixation in pediatric patients are unclear. In this study, we assessed clinical, radiographic, functional, and patient-reported outcomes of pediatric olecranon fractures treated with compression screw fixation. METHODS: We retrospectively identified 37 patients aged 16 years or younger with a total of 40 olecranon fractures…

Olecranon fractures in children: treatment of a rare entity.

BACKGROUND: Olecranon fractures are a rare entity in children. The classification and treatment strategies are still discussed controversially. METHODS: A retrospective chart review of all patients < 17 years admitted with an olecranon fracture at a Level I Trauma Center between 2005 and 2017 has been performed. 46 subjects were included. For classification of olecranon fractures in children the AO Pediatric Comprehensive Classification of Long Bone Fractures (AO-PCCF) was used. Fractures were classified along the fracture line, dislocation, joint involvement and affection of the apophysis. F…
