Simultaneous Surgery for Multiple Facial Basal Cell Carcinoma Excision: Modified Hughes Procedure and Rieger Nasal Flap.

An 84-year-old female patient initially presented to the general ophthalmology outpatient clinic with a complaint of a mass on the left lower eyelid. At that time, only the eyelid lesion was noted. Upon referral to the oculoplastic unit, an expert oculoplastic surgeon identified, in addition, multiple lesions on the left nasal alar rim. The patient was subsequently referred to the Department of Otorhinolaryngology. Incisional biopsies confirmed basal cell carcinoma in both sites. Under general anesthesia, the ophthalmology and otorhinolaryngology teams performed simultaneous resections and rec

Composite endpoint for liver and colon simultaneous surgery: a proposed approach to reduce sample size of future clinical trials.

BACKGROUND: With expanding indications for resection of colorectal liver metastasis (CRLM), simultaneous resections for primary colon cancer (CC) with synchronous colorectal liver metastases (sCRLM) has increased. sCRLM remains debated, however, and only a handful, often underpowered, trials have evaluated this approach. We sought to develop a composite postoperative endpoint (composite endpoint for liver and colorectal simultaneous surgery [CELCSS]) combining colon- and liver-specific complications to reduce sample size requirements in future prospective randomized clinical trials. METHODS:

Simultaneous surgery for NMIBC and BOO: evaluating surgical safety to oncological benefit.

BACKGROUND: Current evidence demonstrates that simultaneous surgical management of bladder cancer (BCa) and bladder outlet obstruction (BOO) is oncologically safe, despite well-known concerns. This systematic review and meta-analysis primarily evaluates oncological outcomes of concurrent transurethral resection of bladder tumor (TURBT) and BOO surgery compared to TURBT alone or staged treatments; perioperative safety is descriptively summarized. METHODS: A systematic search of PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials was conducted from January 2000 to Mar

Simultaneous Surgery of the Clitoral Hood-Labia Minora Complex with the Central Neurovascular Pedicle Retained.

BACKGROUND: Hypertrophy of the labia minora with clitoral hood redundancy always occurs simultaneously and should be considered in its entirety. This study investigated a new simultaneous procedure for the clitoral hood-labia minora complex (CLC) that retains the central neurovascular pedicle. METHODS: This retrospective observational study, conducted between January 2021 and August 2022, included 29 patients with labia minora hypertrophy and clitoral hood redundancy who underwent a surgery wherein procedures on the CLC were performed simultaneously. A divisional design to separate the medial

Overlapping single-level lumbar fusion and adverse short-term outcomes.

OBJECTIVE: There is a paucity of research on the safety of overlapping surgery. The purpose of this study was to evaluate the impact of overlapping surgery on a homogenous population of exactly matched patients undergoing single-level, posterior-only lumbar fusion. METHODS: The authors retrospectively analyzed case data of 3799 consecutive adult patients who underwent single-level, posterior-only lumbar fusion during a 6-year period (June 7, 2013, to April 29, 2019) at a multihospital university health system. Outcomes included 30-day emergency department (ED) visit, readmission, reoperation,

Similarity index for intuitive assessment of three-dimensional facial asymmetry.

Evaluation of facial asymmetry generally involves landmark-based analyses that cannot intuitively assess differences in three-dimensional (3D) stereoscopic structures between deviation and non-deviation sides. This study tested a newly developed similarity index that uses a mirroring technique to intuitively evaluate 3D mandibular asymmetry, and characterised the resulting lower facial soft tissue asymmetry. The similarity index was used to evaluate asymmetry before and after surgery in 46 adult patients (27 men, 19 women; age, 22 ± 4.8 years) with skeletal Class III malocclusion and facial as

Investigating the Impact of Wound Edge Approximation With Skin Grafting on Hypertrophic Scar Reduction: A Randomized Controlled Clinical Trial.

In modern burn care, the focus extends beyond mere patient survival to encompass long-term functional and cosmetic outcomes. Research suggests that the technique and manner of suturing during skin grafting play a significant role in scar formation. This study aimed to explore the effectiveness of wound edge approximation with skin grafting compared to the conventional approach, where the graft edge exclusively interacts with the wound edge, in reducing hypertrophic scar development. Seventy-four burn unit patients eligible for grafting were randomly allocated into 2 groups: those receiving gra

Postoperative Three-Dimensional Evaluation of Mandibular Contouring Surgery Using Computer-Assisted Simulation Planning and a Three-Dimensional-Printed Surgical Guide.

Mandibular contouring surgery was performed using computer-assisted simulation planning (CASP) and 3-dimensional printed surgical guide. The outcome of the surgery was evaluated by overlapping preoperative image. The patient underwent mandibular contouring surgery according to CASP for his residual facial asymmetry of the mandibular angle and mental area. The overall facial aesthetic of the patient was improved. In the overlapping image, the left mandibular border area was slightly overcorrected. However, the other portion was operated as planned. The overcorrection was due to the improper ada

Selection of two intrathoracic anastomosis methods for transabdominal radical surgery for Siewert type II adenocarcinoma of the esophagogastric junction.

INTRODUCTION: The most common intrathoracic anastomosis techniques for Siewert type II adenocarcinoma of the esophagogastric junction (AEG) are the overlap and transorally inserted anvil (OrVil) methods. However, the criteria for choosing between these two methods require further study. AIM: This retrospective study aimed to compare the efficacy and safety of overlap versus OrVil anastomosis in transabdominal radical surgery for Siewert type II adenocarcinoma of the esophagogastric junction. MATERIAL AND METHODS: A total of 34 patients with Siewert type II AEG who underwent transabdominal ra

Prognostic factors of reirradiation for recurrent head and neck cancer.

We analyzed outcomes and prognostic factors to assess the value of reirradiation for recurrent head and neck cancer. Forty-four patients with recurrent squamous-cell carcinoma had undergone external beam reirradiation with cumulative dosing of more than 80 Gy. Six and 38 cases exhibited recurrent stage III and stage IV cancers, respectively. The complete response rate was 32%. Median relapse-free survival time was 4 months, and the 5-year survival was 6%. The major prognostic factor was anatomical location (p < 0.0001). Favorable sites were the nasopharynx, larynx, or oropharynx; unfavorable s

An Evaluation of Muscle Repair Techniques: Implications in Musculoskeletal Healing and Corollaries in Oral-Facial Clefting.

We performed an animal study to identify the techniques associated with the best muscle healing outcomes in cleft lip/palate surgery. The right triceps of thirty adult male Sprague-Dawley rats were cut and repaired by three different suture techniques: simple (n = 10), overlapping (n = 10), and splitting sutures (n = 10). Muscle tissues were isolated from 5 rats per group 1 and 8 weeks postoperation. The inflammatory response and muscle fiber healing were evaluated by hematoxylin and eosin (H&E) staining, Western blotting, immunohistochemistry for TNF-α and IL-1β, and immunofluorescence for la

Obstetric anal sphincter injuries (OASIS) and secondary repair (overlapping sphincteroplasty) in a colorectal unit: case series.

BACKGROUND: Obstetric anal sphincter injuries (OASIS) represent a significant complication of childbirth, with global variation in incidence. Although overlapping sphincteroplasty is widely regarded as the gold-standard surgical treatment, managing delayed or inadequately repaired cases remains challenging. Prevention through improved obstetric practices and early recognition of injuries is considered the optimal approach to minimizing the burden of OASIS. OBJECTIVES: This case series aimed to evaluate the clinical outcomes of overlapping sphincteroplasty in patients presenting with delayed o

Interictal High Gamma Oscillation Regularity as a Marker for Presurgical Epileptogenic Zone Localization.

BACKGROUND: To ensure that epilepsy surgery is effective, accurate presurgical localization of the epileptogenic zone is essential. Our previous reports demonstrated that interictal high gamma oscillation (30-70 Hz) regularity (GOR) on intracranial electroencephalograms is related to epileptogenicity. OBJECTIVE: To examine whether preoperative GOR analysis with interictal high-density electroencephalography (HD-EEG) improves the accuracy of epileptogenic focus localization and enhances postoperative seizure control. METHODS: We calculated GOR from 20 seconds of HD-EEG data for 21 patients wi
