[Delusional disorder of the paranoid type and unruptured intracerebral aneurysm--is there an association?].

INTRODUCTION: Contrary to ruptured intracerebral aneurysm, according to our knowledge, not many papers have been published dealing with the relation of psychic/neuropsychological symptoms and unruptured intracerebral aneurysm (UIA). As to our knowledge, there is no published paper relating delusional disorder, paranoid type (DD-PT) and UIA, which can have important clinical implication. AIM, METHOD AND RESULTS: We describe a case of a woman who had DD-PT and UIA. The patient in her fifties, a typist with elementary school education, presented to the psychiatrist with the symptoms of irritabil

Paranoid personality traits in a panic disorder population: a pilot study.

To better understand the relationship between panic disorder and paranoid personality, panic disorder patients (N = 28) who were referred to an anxiety disorder clinic in a community mental health center were evaluated for paranoid personality traits on a standardized personality self-report instrument. Paranoid personality disorder was found in 54% of subjects. Paranoid subjects were found to have an earlier age of onset, longer duration of illness, and more psychopathology. Possible etiologies and implications for treatment of these findings are discussed.

Toward an operationalization of reactive paranoid psychoses (reactive delusional disorder).

Reactive psychoses are relatively often diagnosed in Norway, although they are not operationalized in any classificatory system and the reliability may be questioned. The aim of this study was to define a group of reactive paranoid psychoses and compare its long-term outcome to the group of nonreactive paranoid psychoses and to paranoid schizophrenia. Approximately 200 out of 301 delusional subjects earlier studied by Retterstöl have recently been interviewed semistructurally, making a total follow-up period of 22-38 years. Diagnoses at discharge have been assessed retrospectively before the p

Delusional (paranoid) disorders: etiologic and taxonomic considerations. II. A possible relationship between delusional and affective disorders.

Paranoid (delusional) disorders are usually thought to overlap with schizophrenic disorders, and there may be a continuum, especially with paranoid schizophrenia. There is also some recent evidence of an overlap with affective disorders. This article refers to the author's series of monodelusional disorders, emphasizing certain mood concomitants, and discussing the implications of these for delusional disorders in general.

Facial emotion recognition in paranoid schizophrenia and autism spectrum disorder.

Schizophrenia (SZ) and autism spectrum disorder (ASD) share deficits in emotion processing. In order to identify convergent and divergent mechanisms, we investigated facial emotion recognition in SZ, high-functioning ASD (HFASD), and typically developed controls (TD). Different degrees of task difficulty and emotion complexity (face, eyes; basic emotions, complex emotions) were used. Two Benton tests were implemented in order to elicit potentially confounding visuo-perceptual functioning and facial processing. Nineteen participants with paranoid SZ, 22 with HFASD and 20 TD were included, aged

Paranoid personality disorder: a synthesis of developmental, dynamic, and descriptive features.

Suspiciousness, feeling persecuted, and grandiosity constitute the classical triad associated with paranoid personality. However, a more important feature appears to be the discrepancy between the outer persona and the inner world of such individuals. The split is pervasive and involves self-concept, object-relations, affects, morality, sexuality, and cognitive style. Outwardly, paranoid individuals are demanding, arrogant, mistrustful, driven, unromantic, moralistic, and acutely vigilant towards the external environment. Internally, however, they are frightened, timid, self-doubting, gullible

Paranoid psychosis (delusional disorder) and schizophrenia. A family history study.

Family studies have played a central role in the controversy over the nosologic status of paranoid psychosis or delusional disorder (DD). In this study, 12 cases of DD were blindly rediagnosed from a cohort of 146 schizophrenics. The prevalence of schizophrenia in the relatives of the DD probands was significantly less than that found in the relatives of the schizophrenics. Affective illness was equally uncommon in both groups of relatives. Inferiority feelings clustered significantly in the relatives of the DD probands. From a familial perspective, DD is not closely related to schizophrenia o

Evolutionary systems therapy for paranoid personality disorder: A seven cases series.

Little is known about effective psychosocial treatments for paranoid personality disorder. This study explores the feasibility of a novel treatment, namely Evolutionary Systems Therapy, in supporting individuals diagnosed with paranoid personality disorder. Seven patients attended 10 months of individual therapy without receiving any psychopharmacological treatment. The primary outcome was the feasibility of the intervention, while the secondary outcomes were remission from the diagnosis and reliable changes in personality pathology and paranoid ideation. All recruited patients completed the i

In search of hidden threats: A scoping review on paranoid presentations in personality disorders.

Recent diagnostic developments suggest that paranoia is a transdiagnostic characteristic common to several personality disorders rather than a personality disorder per se. Nonetheless, empirical literature fails to provide comprehensive and univocal findings on whether and how paranoid presentations relate to different personality disorders. In the present scoping review, we map the empirical literature on paranoid presentations in personality disorders, considering the entire spectrum of paranoid manifestations (i.e., the paranoia hierarchy). In selecting original quantitative studies on para

Lithium-Induced Bradycardia and Cardiomyopathy in a Patient With Bipolar Disorder and Paranoid Schizophrenia.

Lithium is primarily known to cause neurological and gastrointestinal side effects, however, cardiac effects have been rarely reported. We present a unique case of lithium cardiotoxicity causing bradyarrhythmia and cardiomyopathy. A 68-year-old man with a history of paranoid schizophrenia and bipolar disorder presented with altered mental status. On examination, the patient was lethargic, afebrile, with dry oral mucosa, and a regular pulse of 42 bpm. Labs revealed acute kidney injury and elevated lithium levels. Electrocardiogram (ECG) revealed a junctional escape rhythm with a right bundle mo

An exploratory analysis of neurocognition in methamphetamine-induced psychotic disorder and paranoid schizophrenia.

OBJECTIVE: This exploratory study compared the neurocognitive performances of individuals with methamphetamine-induced psychotic disorder (MA psychosis) and paranoid schizophrenia. BACKGROUND: MA psychosis presents very similarly to paranoid schizophrenia and the relationship between the 2 disorders is not well-understood. Neurocognitive functioning is one potential factor for comparison, as deficits are documented in both schizophrenia and methamphetamine dependence. METHOD: Participants were selected through chart review. Neurocognitive test scores for participants with paranoid schizophre

When the Others Are Dangerous: Paranoid Presentations in Subclinical Forms of Personality Disorders.

The discriminant validity of paranoid personality disorder has been recently questioned, and paranoid presentations are now conceived of as transdiagnostic features of personality disorders (PDs). However, empirical results are inconsistent. This study investigated the link between subclinical personality disorders (except paranoid PD) and paranoid presentations, exploring how the severity of personality functioning affects this relationship. Nonclinical participants (N = 270, females: n = 194; 71.9%) completed self-report measures of the constructs of interest. In multiple regression analyses

Social anxiety and premorbid personality disorders in paranoid schizophrenic patients treated with clozapine.

The concept of anxiety as a distinct comorbid disorder in schizophrenia has recently been rediscovered after having been neglected for a long period of time due to both theoretical and clinical approaches adopted from the appearance of the first edition of the Diagnostic and Statistical Manual of Mental Disorders in 1950. This rediscovery was accentuated by the fact that the concept of comorbidity in various psychiatric disorders has recently won widespread favor within the scientific community, and that the use of atypical neuroleptic medication to treat patients with schizophrenia has been r

Treatment outlines for paranoid, schizotypal and schizoid personality disorders. The Quality Assurance Project.

Treatment outlines for paranoid, schizotypal and schizoid personality disorders were developed by having nominated experts consider their own views in the light of the treatment literature and the responses of practising psychiatrists. In the detailed recommendations it is clear that while patients with all three disorders often present for treatment in a crisis and often see no issue other than the resolution of the crisis, patients with schizoid personality disorder can use long-term psychotherapy to develop and change to the extent of no longer being handicapped.

Taxometric evidence for the dimensional structure of cluster-C, paranoid, and borderline personality disorders.

Despite a lively debate about the dimensional vs. categorical nature of Personality Disorders (PDs), direct empirical tests of the underlying structure are missing for most PDs. Taxometrics can be used to investigate whether latent structures are categorical or dimensional. We investigated the latent structure underlying Avoidant, Dependent, Obsessive-Compulsive, Depressive, Paranoid, and Borderline PD by means of three types of taxometric analyses. SCID-II based DSM-IV PD criterion scores from 1,816 patients from Mental Health and Forensic Institutes, and 63 nonpatients, were analyzed with th

Evidence for Distinguishable Treatment Costs among Paranoid Schizophrenia and Schizoaffective Disorder.

BACKGROUND: Schizophrenia spectrum disorders result in enormous individual suffering and financial burden on patients and on society. In Germany, there are about 1,000,000 individuals suffering from schizophrenia (SZ) or schizoaffective disorder (SAD), a combination of psychotic and affective symptoms. Given the heterogeneous nature of these syndromes, one may assume that there is a difference in treatment costs among patients with paranoid SZ and SAD. However, the current the national system of cost accounting in psychiatry and psychosomatics in Germany assesses all schizophrenia spectrum dis

A comparative study of cognitive deficits in patients with delusional disorder and paranoid schizophrenia.

BACKGROUND: Very few studies have evaluated the neurocognitive functions of patients with persistent delusional disorder. AIM: To study the neurocognitive profile of patients with delusional disorder and compare it with those of patients with paranoid schizophrenia and healthy control subjects. MATERIALS AND METHODS: Attention concentration, executive functions, memory, and IQ were assessed in 20 patients with delusional disorder and were compared with 20 patients with paranoid schizophrenia and 20 healthy controls. All three groups were matched on age, sex, and level of education. The two p
