Bidirectional Psychosocial Burden in Autoimmune Hepatitis: Mental Health, Caregiver Strain, and the Potential Role of Patient Advocacy Programming.

BACKGROUND: Autoimmune hepatitis (AIH) is associated with substantial psychosocial burden, yet mental health challenges remain poorly characterized and caregiver experience has not been systematically described. AIMS: We aimed to (1) characterize mental health burden among AIH patients; (2) provide the first systematic description of caregiver burden and unmet needs in AIH; (3) compare patient and caregiver intervention priorities; and (4) explore whether attendance at a disease-specific educational conference was associated with changes in self-reported psychological distress. METHODS: Pati

Identifying and addressing the needs of caregivers of patients with cancer: evidence on interventions and the role of patient advocacy groups.

As the number of people with cancer increases, so does the number of informal caregivers. These caregivers frequently have multiple unmet needs and experience numerous burdens. Here we explore the crucial roles of these caregivers and categorize their unmet needs into four areas: information, relationship and communication, emotional support, and practical or financial needs. We provide evidence on emerging interventions aimed at supporting caregivers, including patient/caregiver assessments, education, collaborative care, financial assistance, wellness, informational programs, and an integrat

The Role of Patient Advocacy and the Declining Rate of Clitoroplasty in 46,XX Patients With Congenital Adrenal Hyperplasia.

Prior to the 1990s, most 46,XX infants with clitoromegaly secondary to congenital adrenal hyperplasia were treated with feminizing genitoplasty to make their cosmetic appearance congruent with their genotypic sex. A 2006 consensus statement for the management of intersex disorders accepted input from patient advocates and did not support purely cosmetic surgery for clitoromegaly. This study examined the extent to which the desired change was implemented in practice. Retrospective chart review was performed at a single Midwestern tertiary care medical center for patients born between 1979 and 2

"We are champions": an investigation on how cancer support group propels patient advocacy, voices, education, mentorship, and support.

PURPOSE: With over 127,000 incidents and 79,000 deaths, cancer is now officially declared a notifiable disease of public interest in Nigeria. Most cancer control efforts have targeted prevention, diagnosis, and treatment, often overlooking supportive care, survivorship, and patient-centered aspects of care. This study investigated survivors' experiences with cancer support groups (CSG), exploring their activities, effectiveness, challenges, and benefits to address these critical gaps. METHODS: Participants (N = 14) who were cancer survivors and members of a CSG were recruited. Data collection

Advocacy for health rights with a group of patients with chronic diseases in a low-income neighbourhood in Ecuador.

OBJECTIVE: We describe an advocacy process developed with a diabetes patient club from a low-income area, using participatory research methods aimed at securing governmental support and recognition of peer networks as a fundamental principle of health promotion for chronic patients. RESULTS: This participatory process, led by local facilitators from the Chimbacalle Diabetes Patients' Club, followed three key steps: social diagnosis (using participatory tools such as a sociogram and SWOT analysis), development of a community narrative through digital storytelling, and process evaluation. Self-advocacy experiences in coping with dysphagia among patients with oral cancer: A qualitative study.

OBJECTIVE: This study aimed to explore the experiences of patients with oral cancer in coping with dysphagia from the perspective of self-advocacy. METHODS: A descriptive qualitative research design was adopted. From March to May 2024, face-to-face, in-depth semi-structured interviews were conducted with 21 patients with oral cancer at a tertiary cancer hospital. The interview guide was developed based on the self-advocacy framework, and data were analyzed using the directed content analysis method. RESULTS: Findings were organized within the self-advocacy framework, yielding three main cate

Patient Advocacy Group Leaders' Perceptions on Primary Care's Role in Caring for Patients With a History of Breast Cancer.

BACKGROUND: Cancer advocacy groups engage patients, families and caregivers in navigating the cancer landscape, with a focus on early detection/screening and providing psychosocial and financial support during and after treatment. These groups are influential among their communities, funders and policymakers. OBJECTIVE: We wished to understand perceptions of breast cancer advocacy group leaders on primary care's role in breast cancer survivorship care, given limited primary care engagement despite endorsement by the National Cancer Institute (NCI). METHODS: As part of a larger NCI-funded stu

The National Marrow Donor Program's symposium on patient advocacy in cellular transplantation therapy: addressing barriers to hematopoietic cell transplantation.

Although hematopoietic cell transplantation (HCT) is an effective treatment option for patients with life-threatening blood, immune system, or genetic disorders, many barriers besides a lack of suitably matched donors exist and can have an adverse impact on access and outcomes of HCT. In 2008, the National Marrow Donor Program, through its Office of Patient Advocacy, convened a diverse group of experts and transplantation survivors to identify persistent patient barriers throughout the transplantation process and to make recommendations for programs and initiatives to address these barriers, i

Cancer Patient Advocacy in the Postoperative Intensive Care Unit: The Experience of Nurses and the Voice of Older Adult Patients.

Background/Objectives: Older adults with cancer in the postoperative environment face complex vulnerability, exacerbated by the frailty of ageing and the aggressiveness of surgical treatment. In this highly demanding context, nurses play a crucial role as patient advocates. However, there is a knowledge gap regarding how advocacy is perceived and experienced by the nurse-patient dyad. This qualitative study aims to explain the practice of advocacy by comparing the perspectives of nurses and patients in order to construct part of a substantive theory on the subject. Methods: The Grounded Theory

Postoperative Disparities Facing Patients Experiencing Homelessness: Opportunities for Advocacy From an Anesthesiology Perspective.

Patients experiencing homelessness (PEH) continue to face significant disparities across the world. Many of these disparities not only have downstream medical consequences but also housing-based consequences, including propagation of chronic homelessness. The postoperative period is a time of great medical vulnerability for PEH. Without targeted intervention, PEH are at risk for progression of disparate outcomes and chronic vulnerability. As integral players in the patient's care team during this period, anesthesiologists are uniquely positioned to address these disparities and drive change fo

Using Federal, State, and Grassroots Advocacy to Effect Change in Cancer Care Delivery and Patient Outcomes in the United States.

Health policy influences health and well-being for patients with cancer. Advocating for evidence-based health policies that aim to prevent cancer, promote health, and improve efficiencies in the health care system is crucial to improving care delivery and cancer outcomes nationwide. The role of the ASCO's Government Relations Committee (GRC) is to advocate for policies at a federal and state level that promote high-quality and equitable cancer care, and ASCO volunteer engagement is critical for the success of creating, changing, and adopting policies that support patients with cancer and their

Microvascular tissue transfer in cleft palate patients: advocacy of the prelaminated radial free forearm flap.

The closure of wide palatal clefts and recurrent oronasal fistulae may be challenging. After repeated failure of conventional techniques, microvascular tissue transfer may be indicated in the closure of such fistulae. Depending on the location and the size of the palatal fistula, different tissues are required to sufficiently close the palatal gaps. A subdivision of common flaps into mucosa, muscular, bony, skin, and fascia flaps was carried out to analyze their suitability for alveolar, hard, and soft palate reconstruction. Furthermore, the bulk of flaps and the length of the vascular pedicle

Is self-advocacy universally achievable for patients? The experiences of Australian women with cardiac disease in pregnancy and postpartum.

PURPOSE: Patient self-advocacy is valued and promoted; however, it may not be readily accessible to all. This analysis examines the experiences of women in Australia who had cardiac disease in pregnancy or the first year postpartum through the lenses of self-advocacy and gender, specifically seeking to elaborate on the contexts, impacts, barriers, and women's responses to the barriers to self-advocacy. METHOD: A qualitative study design was used. Twenty-five women participated in semi-structured in-depth interviews. Data were analysed using thematic analysis. RESULTS: Analysis of findings ge

Acceptability of a Mobile-Health Living Kidney Donor Advocacy Program for Black Wait-Listed Patients.

Marked racial disparities exist in rates of living donor kidney transplantation (LDKT). The Living Organ Video Educated Donors (LOVED) program is a distance-based, mobile health program designed to help Black kidney transplant wait-list patients advocate for a living donor. This study reported on the acceptability outcomes to aid in future refinements. Participants were randomized to LOVED (n = 24, mean age = 50.9 SD (9.2) years), male = 50%) and usual care groups (n = 24 (mean age 47.9 SD (10.0), male 50%). Four LOVED groups completed an eight-week intervention that consisted of six online vi

Impact of perception of socioeconomic burden on advocacy for patient autonomy in end-of-life decision making: a study of societal attitudes.

We investigated the impact of perception of socioeconomic burden on beliefs regarding a patient's autonomy in end-of-life (EOL) decision making. We also sought to identify the characteristics of individuals who advocate patient autonomy and their attitudes toward other EOL issues. A total of 1055 individuals from the Korean general population were interviewed through a telephone survey using a structured questionnaire that was designed to investigate public attitudes toward various EOL issues. Of 1019 individuals included in the analysis, 635 (62.3%) specified the patient and 221 (21.7%) the f

Management of patients with rare diseases in the Middle East: challenges & opportunities - insights from the Rare Advocacy Council.

BACKGROUND: Rare diseases affect a small percentage of the population but collectively impact millions worldwide. In the Middle East, the challenges are intensified by regional factors such as high rates of consanguinity, sociocultural stigma, limited diagnostic capacity, and inadequate healthcare infrastructure. These challenges often lead to delayed diagnoses, restricted access to treatment, and poor quality of life for affected individuals and their families. METHODS: The Rare Advocacy Council conducted two 1.5-hour virtual expert panels involving 14 regional and international stakeholders
