Journal of the American College of Clinical Pharmacy : JACCP2026eng
BACKGROUND: Alcohol use disorder (AUD) remains undertreated in the United States despite the availability of effective pharmacotherapies. Pharmacist-led education and clinical decision support embedded in the electronic medical record (EMR) was provided to primary care providers (PCPs) to address barriers to prescribing medications for AUD. This study evaluated the impact of the pharmacist-led intervention on pharmacologic treatment rates for patients with AUD managed by PCPs.
METHODS: This was a retrospective, fixed cohort, pre-post study conducted across a network of 10 clinics within a sin

BACKGROUND: To increase access and use of pre-exposure prophylaxis (PrEP), medications which serve as an effective prophylaxis for HIV, several U.S. states have permitted pharmacists to provide PrEP to patients independent from other providers. States have implemented this policy either by allowing pharmacists to serve as stop-gap providers by prescribing PrEP to patients for a limited duration, or by allowing pharmacists to fully provide these medications to patients without a time limitation.
OBJECTIVES: This study tested whether the introduction of these policies is associated with increas

While deprescribing benzodiazepines and related drugs (BZRDs) is crucial for preventing prolonged use and their associated adverse effects, it presents challenges from a healthcare perspective, because of limited resources and time. Recently, a pharmacist-led deprescribing of BZRDs in the treatment of insomnia was introduced in Helsinki's primary care health centres. To explore pharmacists' and physicians' insights, qualitative semi-structured interviews were conducted via Microsoft Teams with physicians and pharmacists involved in a pharmacist-led deprescribing. The interviews were recorded,

Pharmacist-led deprescribing is an approach of addressing polypharmacy in the long-term care (LTC) setting. However, the sustainability of this practice has not been widely studied. This study describes facilitators and barriers to pharmacist-led deprescribing from pre- and post-surveys completed as part of a randomized controlled antihypertensive deprescribing trial in Alberta, Canada. The surveys, targeting facilitators and barriers to deprescribing, were developed based on current evidence and designed through an iterative process with pharmacist input and consist of open- and closed-ended

BACKGROUND: Although opioid prescribing has recently trended downward, opioid-related overdoses and deaths have drastically increased. Community pharmacists are accessible health care providers who are well positioned to disseminate information on opioid safety and to educate and counsel on medication use, managing adverse events, and proper medication disposal. Patient callbacks facilitate appropriate medication usage. We developed an opioid callback program that provides a framework for pharmacists to follow up with patients with an opioid prescription.

BACKGROUND: The improper disposal of unwanted or unused medications is a pressing issue that can lead to drug misuse and environmental contamination. Pharmacists play a crucial role in promoting safe drug disposal by the public. This study explores pharmacists' perceptions of the causes of unwanted and unused medications, their practices, and the barriers to promoting safe medication disposal among the public in the United Arab Emirates.
METHODS: This cross-sectional study included pharmacists working in the UAE's community and hospital settings with direct patient contact. A convenience samp

PURPOSE: Pharmacist prescribing of HIV pre-exposure (PrEP) and post-exposure prophylaxis (PEP) is an emerging strategy to expand access to HIV prevention, but real-world evidence on its implementation remains limited. We assessed national and regional trends in pharmacist-prescribed dispensing records of PrEP (2020-2025) and PEP (2020-2024) in Brazil.
METHODS: Nationwide time series analysis using databases from the Ministry of Health. The primary outcome was pharmacist-prescribed PrEP and PEP dispensing records per 100 000 adults per year. Joinpoint regression was used to estimate average an

PURPOSE: Pharmacists are critical clinical partners for patients managing complex medication regimens, particularly those with chronic disease, polypharmacy, or heightened vulnerability. This commentary aims to highlight how these same principles such as careful dose titration, vigilant monitoring, and patient-centered counseling are indispensable in medical cannabis care and to illustrate the impact of pharmacist integration using New York State's tightly regulated medical cannabis program as a primary example.
METHODS: Drawing on nearly a decade of practice in a specialty pharmacy and medic

BACKGROUND: Community pharmacists often need patient health information such as laboratory results to provide safe care. However, access to this information varies by pharmacy setting. While health system outpatient pharmacists have access to electronic health records (EHRs), most independent and chain pharmacies rely on limited dispensing software data.
OBJECTIVES: To characterize the frequency and characteristics of patient health information requests made by community pharmacists in independent and health system outpatient pharmacies during routine care delivery and to compare findings acr

BACKGROUND: Community pharmacists dispensing antibiotics without a prescription (DAwP) remain a contributing factor to antimicrobial resistance (AMR) in low- and middle-income countries (LMICs). Multiple structural, behavioral, and systemic factors influence this practice, yet the diversity and interaction of these drivers remain insufficiently explored. OBJECTIVE: This systematic literature review aims to identify and synthesize the internal and external factors contributing to DAwP among community pharmacists in LMICs, integrating evidence from both qualitative and mixed-method studies to of

INTRODUCTION: The management of drug-drug interactions (DDIs) is a complex process in which risk-benefit assessments should be combined with the patient's perspective.
OBJECTIVE: The aim of this study was to determine patients' and pharmacists' preferences regarding DDI management.
METHODS: We conducted a choice-based conjoint survey about a fictitious DDI concerning the combination of a cardiovascular drug and an antibiotic for pneumonia. Patients and pharmacists had to choose 12 times between two management options. The options were described by five attributes, including risk, benefit and
