Intracranial Pressure.

Elevated intracranial pressure (ICP) is a critical condition that requires rapid diagnosis and intervention to prevent severe neurologic damage and mortality. While invasive ICP monitoring remains the gold standard, it carries significant risks in cases of coagulopathy, bleeding diathesis, or in unstable patients. Additionally, there is a risk of infection, particularly in immunocompromised patients. Transcranial ultrasonography offers a valuable, non-invasive bedside tool for indirectly assessing elevated ICP in critical care settings. There are 3 primary modes of detecting elevated ICP using

Pressure alopecia.

Postoperative or pressure alopecia (PA) is an infrequently reported group of scarring and non-scarring alopecias. It has been reported after immobilization of the head during surgery and following prolonged stays on intensive care units, and may be analogous to a healed pressure ulcer. This review presents a summary of cases published in pediatrics and after cardiac, gynecological, abdominal and facial surgeries. PA may manifest as swelling, tenderness, and ulceration of the scalp in the first few postoperative days; in other cases, the alopecia may be the presenting feature with a history of

Pressure sores.

Pressure, hygiene, spasticity and chronic infection are the important local factors in pressure ulceration. Hypoproteinemia, nutritional deficiency and anemia are the systemic factors. Careful attention to local wound care and dietary supplements is required. Muscle and myocutaneous flaps may provide better coverage than the traditional random pattern flaps. Flaps with sensibility have theoretic advantages but need further investigation.

Pressure ulcerations.

Pressure sore are caused by multiple mechanisms. Preventive management requires relief of pressure, skin care, control of spasms, release of contractures and treatment of infection. Conservative treatment succeeds only when pressure on the ulcer site is relieved. Surgical treatment involves primary, secondary and tertiary procedures. Benefits of surgical closure include avoidance of protein loss, prevention of osteomyelitis and other infections, improved appearance and prevention of carcinoma and amyloidosis. A coordinated team approach, incorporating patient education, is effective.

Gravitational effects on intraocular pressure and ocular perfusion pressure.

Changes in the gravitational vector by postural changes or weightlessness induce fluid shifts, impacting ocular hemodynamics and regional pressures. This investigation explores the impact of changes in the direction of the gravitational vector on intraocular pressure (IOP), mean arterial pressure at eye level (MAPeye), and ocular perfusion pressure (OPP), which is critical for ocular health. Thirteen subjects underwent 360° of tilt (including both prone and supine positions) at 15° increments. At each angle, steady-state IOP and MAPeye were measured, and OPP calculated as MAPeye - IOP. Experim

Analysis of Intracranial Pressure Pulse-Pressure Relationship: Experimental Validation.

The slope of linear relationship between the amplitude of pulsations in intracranial pressure (ICP) versus mean ICP has recently been suggested as a useful guide for selecting patients for shunt surgery in normal pressure hydrocephalus (NPH). To better understand how the pathophysiology of cerebral circulation influences this parameter, we aimed to study the relationship between mean pressure and pulsation amplitude in a wide range of conditions affecting cerebrovascular tone and ICP in experimental conditions. We retrospectively analysed experimental material collected previously. [Perioperative blood pressure management : What is the optimal pressure?].

BACKGROUND: Measurement of blood pressure is part of standard monitoring procedures in anesthesia, in addition to the other vital parameters of heart frequency and peripheral oxygen saturation. In recent years the relevance of the duration and extent of perioperative episodes of hypotension for the occurrence of postoperative complications or even increased mortality have become the focus of scientific investigations. OBJECTIVE: The aim of this review is to briefly recapitulate the physiological aspects of blood pressure and to describe the pathophysiology and risk factors of perioperative hy

Blood pressure variability of two ambulatory blood pressure monitors.

OBJECTIVE: There are no data on the evaluation of blood pressure (BP) variability comparing two ambulatory blood pressure monitoring monitors worn at the same time. Hence, this study was carried out to compare variability of BP in healthy untreated adults using two ambulatory BP monitors worn at the same time over an 8-h period. METHODS: An Accutorr device was used to measure office BP in the dominant and nondominant arms of 24 participants.Simultaneous 8-h BP and heart rate data were measured in 24 untreated adult volunteers by Mobil-O-Graph (worn for an additional 16 h after removing the Sp

Swallowing pressure and pressure profiles in young healthy adults.

OBJECTIVES/HYPOTHESIS: To measure the swallowing pressure (SP) of normal subjects using a 2.64-mm-diameter high-resolution manometry (HRM) catheter with 36 circumferential sensors. STUDY DESIGN: Repeated measures with subjects serving as controls. METHODS: Thirty healthy subjects swallowed water at different temperatures and volumes to examine the maximum SP at the velopharynx, meso-hypopharynx, upper esophageal sphincter (UES), and cervical esophagus, and the duration of lowered pressure at the UES. RESULTS: The maximum SP at any location was unaffected by the volume of water, whereas the

Iliac venous pressure estimates central venous pressure after laparotomy.

BACKGROUND: Central venous pressure (CVP) is traditionally obtained through subclavian or internal jugular central catheters; however, many patients who could benefit from CVP monitoring have only femoral lines. The accuracy of illiac venous pressure (IVP) as a measure of CVP is unknown, particularly following laparotomy. METHODS: This was a prospective, observational study. Patients who had both internal jugular or subclavian lines and femoral lines already in place were eligible for the study. Pressure measurements were taken from both lines in addition to measurement of bladder pressure, m

Pressure-dependent neuroretinal rim loss in normal-pressure glaucoma.

PURPOSE: To evaluate whether, in normal-pressure glaucoma, the level of intraocular pressure is correlated with the degree of glaucomatous optic nerve damage. METHODS: Color stereo optic disk photographs of 98 eyes with the focal type of normal-pressure glaucoma, 17 eyes with the highly myopic type of normal-pressure glaucoma, and 36 eyes with the age-related atrophic type of primary open-angle glaucoma were morphometrically evaluated. RESULTS: In all three study groups, neuroretinal rim area declined significantly (P < .05) with increasing maximal intraocular pressure values. CONCLUSIONS:

Increased intracranial pressure in so-called 'normal-pressure hydrocephalus'.

Five patients exhibited an apparently communicating hydrocephalus -- type of disorder, with ventricular reflux on the cisternograms, lack of air-encephalographic parasagittal air-filling and clearly enlarged temporal horns. The group seemed heterogeneous, with regard to age distribution; possible etiological factors and duration of the hydrocephalic process. In all 5 patients increased intracranial/intrathecal pressure was observed: in 2 patients, only with intrathecal measurement in the lumbar area, but in the remaining 3 patients with epidural/ventricular fluid pressure monitoring. Optimizing Initial Shunt Pressure in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND: Although setting a low initial opening pressure in a programmable valve with a gravitational unit seems to be the gold standard treatment for idiopathic normal pressure hydrocephalus (iNPH) patients, there are no studies in the literature describing the optimal initial low-pressure setting. Here, we sought to describe the optimal initial opening pressure setting for iNPH in terms of both efficacy and safety. MATERIALS AND METHODS: Patients with probable iNPH underwent shunt surgery with a programmable valve with a gravitational unit, using either an opening pressure of 3 or 6.5 cm

Blood Pressure-Lowering Medications, Sodium Reduction, and Blood Pressure.

BACKGROUND: Both blood pressure-lowering medication and sodium reduction are effective in hypertension control, but whether the effect of sodium reduction differ across blood pressure-lowering medications is unclear. This study aims to evaluate the dose-response effect of sodium intake reduction on blood pressure in treated hypertensive individuals and the impact of different classes of blood pressure-lowering drugs. METHODS: We searched multiple databases and reference lists up to July 9, 2024. Randomized controlled trials with a duration of ≥2 weeks comparing the effect of different levels

Alternating pressure (active) air surfaces for preventing pressure ulcers.

BACKGROUND: Pressure ulcers (also known as pressure injuries, pressure sores, decubitus ulcers and bed sores) are localised injuries to the skin or underlying soft tissue, or both, caused by unrelieved pressure, shear or friction. Alternating pressure (active) air surfaces are widely used with the aim of preventing pressure ulcers. OBJECTIVES: To assess the effects of alternating pressure (active) air surfaces (beds, mattresses or overlays) compared with any support surface on the incidence of pressure ulcers in any population in any setting. SEARCH METHODS: In November 2019, we searched the

Lower body negative pressure to safely reduce intracranial pressure.

KEY POINTS: During long-term missions, some astronauts experience structural and functional changes of the eyes and brain which resemble signs/symptoms experienced by patients with intracranial hypertension. Weightlessness prevents the normal cerebral volume and pressure 'unloading' associated with upright postures on Earth, which may be part of the cerebral and ocular pathophysiology. By placing the lower body in a negative pressure device (LBNP) that pulls fluid away from cranial compartments, we simulated effects of gravity and significantly lowered pressure within the brain parenchyma and

Does pressure cause liver cirrhosis? The sinusoidal pressure hypothesis.

Independent of their etiology, all chronic liver diseases ultimately lead to liver cirrhosis, which is a major health problem worldwide. The underlying molecular mechanisms are still poorly understood and no efficient treatment strategies are available. This paper introduces the sinusoidal pressure hypothesis (SPH), which identifies an elevated sinusoidal pressure (SP) as cause of fibrosis. SPH has been mainly derived from recent studies on liver stiffness. So far, pressure changes have been exclusively seen as a consequence of cirrhosis. According to the SPH, however, an elevated SP is the ma

Blood pressure modifies retinal susceptibility to intraocular pressure elevation.

Primary open angle glaucoma affects more than 67 million people. Elevated intraocular pressure (IOP) is a risk factor for glaucoma and may reduce nutrient availability by decreasing ocular perfusion pressure (OPP). An interaction between arterial blood pressure and IOP determines OPP; but the exact contribution that these factors have for retinal function is not fully understood. Here we sought to determine how acute modifications of arterial pressure will affect the susceptibility of neuronal function and blood flow to IOP challenge. Anaesthetized (ketamine:xylazine) Long-Evan rats with low (

Toe pressure measurements compared to ankle artery pressure measurements.

The Trans-Atlantic Inter-Society Consensus (TASC)-recommended absolute toe pressure is < 30-50 mm Hg for definition of chronic critical limb ischemia (CLI). Toe pressures can be measured by different techniques. The authors analyzed the clinical use of the Doppler technique and an automatic device with optical sensors and estimated their value in documentation of chronic critical limb ischemia compared to ankle artery pressures. Three different investigations were performed: (1) In 16 healthy subjects the digital artery pressures were measured by using 3 different optical sensors (transmission

Blood pressure and high blood pressure. Aspects of risk.

This report deals with three aspects of risk related to blood pressure and high blood pressure. The first aspect of risk concerns distributions of systolic blood pressure (SBP) and diastolic blood pressure (DBP) in the adult population and their relation to long-term risk of morbidity and mortality. By middle age, only a minority (about 20%) of Americans have optimal SBP and DBP levels, less than 120 mm Hg and less than 80 mm Hg, respectively. For the majority with higher levels, risks of major clinical events, including death from cardiovascular diseases and from all causes, are markedly incr
