Herpes zoster and radiation therapy: what radiation oncologists need to know about diagnosing, preventing, and treating herpes zoster.

Herpes zoster, a viral disease that is characterized by a painful and blistering eruption in the skin, represents reactivation of latent varicella zoster (chickenpox) virus infection. In high-risk groups such as elderly or immunocompromised patients, the incidence of zoster can be as high as 50%. Radiation oncologists are likely to see zoster because cancer, and cancer therapy, can adversely affect immune function. A few reports suggest that radiation therapy is a risk factor for zoster, and that the skin eruption is often in or near the radiation treatment field. The diagnosis is typically ma

Effect of practice integration between urologists and radiation oncologists on prostate cancer treatment patterns.

PURPOSE: National attention has focused on whether urology-radiation oncology practice integration, known as integrated prostate cancer centers, contributes to the use of intensity modulated radiation therapy, a common and expensive prostate cancer treatment. MATERIALS AND METHODS: We examined prostate cancer treatment patterns before and after conversion of a urology practice to an integrated prostate cancer center in July 2006. Using the SEER (Statistics, Epidemiology and End Results)-Medicare database, we identified patients 65 years old or older in 1 statewide registry diagnosed with nonm

Radiation Oncologists' Perspectives on Oligometastatic Prostate Cancer: A Survey from Korean Oligometastasis Working Group.

BACKGROUND: Interest in the oligometastatic prostate cancer (OMPC) is increasing, and various clinical studies have reported the benefits of metastasis-directed radiation therapy (MDRT) in OMPC. However, the recognition regarding the adopted definitions, methodologies of assessment, and therapeutic approaches is diverse among radiation oncologists. This study aims to evaluate the level of agreement for issues in OMPC among radiation oncologists. METHODS: We generated 15 key questions (KQs) for OMPC relevant to definition, diagnosis, local therapies, and endpoints. Additionally, three clinical

Treatment of recurrent prostate cancer following radical prostatectomy: the radiation-oncologists point of view.

Recurrence of prostate cancer after radical prostatectomy is a common event. Salvage radiation therapy (RT) is the mainstay of treatment in cases with recurrence defined as PSA failure, offering the chance of cure. Multiple studies showed that the lower the PSA level at the beginning of salvage RT, the better the treatment outcome. There is evidence that higher radiation doses are associated with improved PSA relapse free rates. Four different recurrence patterns exist: 1) local recurrence in the prostatectomy bed only; 2) loco-regional metastases in the pelvic lymph nodes; 3) distant metastas

Treatment of parotid gland oncocytoma: a case report from a radiation oncologist's perspective.

BACKGROUND: Parotid oncocytoma is a rare benign neoplasm for which surgical excision remains the standard treatment. Although the recurrence rate is generally low, it increases significantly in cases with multifocal lesions or incomplete surgical excision. In such scenarios, postoperative radiotherapy (RT) has been shown to improve local control, reduce recurrence rates, and minimize the need for reoperation. This report presents a representative case of parotid oncocytoma, with a focused discussion on therapeutic decision-making from a radiation oncologist's perspective. CASE REPORT: We repo

You Are Already Trained for This: The Hematologist/Oncologist's Role in Radiation Mass Casualty Response.

A large-scale radiation injury or nuclear incident requires an immediate, coordinated medical response that vastly exceeds traditional disaster management frameworks. While specialized networks exist, the initial surge of casualties will inevitably place a heavy burden on regional healthcare systems. Hematologists and oncologists possess the core clinical competencies necessary to manage the hematologic complications of acute radiation syndrome (ARS), yet barriers to active engagement persist. This manuscript reviews current operational strategies for disaster readiness, emphasizing the utiliz

Should internal mammary lymph nodes in breast cancer be a target for the radiation oncologist?

PURPOSE: The elective treatment of internal mammary lymph nodes (++IMNs) in breast cancer is controversial. Previous randomized trials have not shown a benefit to the extended radical mastectomy or elective IMN irradiation overall, but a survival benefit has been suggested by some for subgroups of patients with medial tumors and positive axillary lymph nodes. The advent of effective systemic chemotherapy and potential for serious cardiac morbidity have also been factors leading to the decreased use of IMN irradiation during the past decade. The recent publishing of positive trials testing post

The Evolving Role of Radiation Oncology in the Management of Uterine Cervical Carcinoma: A State-of-the-Art Review for Non-Radiation Oncologists.

Cervical cancer is one of the most common gynecological tumors globally. When diagnosed, treatment decisions should be based on a risk-benefit analysis of each treatment modality to obtain a cure with minimum complications. The optimal approach for management should consider clinical factors such as age, menopausal status, medical comorbidities, histological type, tumor size, and the extent of disease. Radiotherapy is the cornerstone for successful management in almost all clinical stages of this disease. Options for primary treatment in patients with early cervical cancer may include radical

A Radiation Oncologist's Guide to Axillary Management in Breast Cancer: A Walk Through the Trials.

PURPOSE OF REVIEW: The axilla is the most common site for breast cancer nodal metastases. Aggressive management includes axillary lymph node dissection (ALND), radiotherapy, and systemic therapy, but carries the risks of lymphedema and "overtreatment". We review the clinical trials that led to de-escalation of axillary management and their nuances that are often overlooked. RECENT FINDINGS: With the rise of sentinel lymph node biopsy, several trials conclude that ALND can be omitted in specific populations. However, the subtleties in those trials, such as the role of chemotherapy and radiothe

Tumor Board Shadowing: A Unique Approach for Integrating Radiation Oncologists Into General Medical Student Education.

PURPOSE: Radiation oncology is often overlooked in US medical school curricula, with few opportunities for most students to learn about the specialty or the value of radiation therapy in cancer care. Tumor boards represent a potential avenue not only to increase students' exposure to radiation oncologists but also to provide a fundamental understanding of the multidisciplinary nature of cancer care and effective collaboration in clinical practice. METHODS AND MATERIALS: In this study, we evaluated a novel radiation oncologist-driven tumor board shadowing experience at 3 medical schools in the

Technical aspects of SBRT for therapy-refractory ventricular tachycardia: a systematic review for radiation oncologists.

BACKGROUND: Ventricular tachycardia (VT) is a potentially life-threatening arrhythmia, that can lead to sudden cardiac death. While conventional treatments include antiarrhythmic drugs, catheter ablation, and ICD implantation, recent studies suggest that stereotactic body radiotherapy (SBRT) offers a non-invasive alternative for the treatment of VT. The objective of this systematic review is to summarize the current evidence on SBRT for refractory VT from a radiotherapeutic and technical perspective and to assess its safety and effectiveness. METHODS: he systematic search was conducted accordi
