Intensity-modulated radiotherapy for soft tissue sarcoma of the thigh.

PURPOSE: Fracture of the femur is one of the late complications of adjuvant radiotherapy for patients with soft tissue sarcomas of the thigh, who receive external beam irradiation after limb-sparing surgery. When the target volume approximates the femur, it is often inevitable that a large segment of the femur will receive full prescription dose with conventional radiation techniques. We report the dosimetric feasibility of intensity- modulated radiation therapy (IMRT) techniques to achieve adequate target coverage and bone sparing. METHODS AND MATERIALS: Treatment planning was performed usin

Algorithms and functionality of an intensity modulated radiotherapy optimization system.

The main purpose of this paper is to describe formalisms, algorithms, and certain unique features of a system for optimization of intensity modulated radiotherapy (IMRT). The system is coupled to a commercial treatment planning system with an accurate dose calculation engine based on the kernel superposition algorithm. The system was designed for use for research as well as for routine clinical practice. It employs dose- and dose-volume-based objective functions. The system can optimize IMRT plans with multiple target volumes simultaneously. Each target volume may be assigned a different presc

Dysphagia-optimised intensity-modulated radiotherapy versus standard intensity-modulated radiotherapy in patients with head and neck cancer (DARS): a phase 3, multicentre, randomised, controlled trial.

BACKGROUND: Most newly diagnosed oropharyngeal and hypopharyngeal cancers are treated with chemoradiotherapy with curative intent but at the consequence of adverse effects on quality of life. We aimed to investigate if dysphagia-optimised intensity-modulated radiotherapy (DO-IMRT) reduced radiation dose to the dysphagia and aspiration related structures and improved swallowing function compared with standard IMRT. METHODS: DARS was a parallel-group, phase 3, multicentre, randomised, controlled trial done in 22 radiotherapy centres in Ireland and the UK. Participants were aged 18 years and old

Intensity-modulated radiotherapy and volumetric-modulated arc therapy for malignant pleural mesothelioma after extrapleural pleuropneumonectomy.

Radiotherapy reduces the local relapse rate after pleuropneumonectomy of malignant pleural mesothelioma (MPM). The optimal treatment technique with photons remains undefined. Comparative planning for intensity-modulated radiotherapy (IMRT) and volumetric-modulated arc therapy (VMAT) was performed. Six MPM patients with significant postoperative intrathoracic air cavities were planned with IMRT and VMAT. A dose comparison for the targets and organ at risks (OAR) was performed. Robustness was assessed in respect to the variation of target dose with change in volume of air cavities. VMAT reduced

Optimization of Fluence Map for CyberKnife Raster Scanning Intensity Modulated Radiotherapy.

BACKGROUND/AIM: Stereotactic body radiotherapy for prostate cancer using CyberKnife with circular cone requires a long treatment time. Raster scanning intensity modulated radiotherapy (RS-IMRT) has a potential of improving treatment efficacy, introducing shorter treatment time, better target dose uniformity, and lower organ at risk (OAR) dose. The purpose of the study was to develop a fluence optimization system for RS-IMRT. PATIENTS AND METHODS: RS-IMRT plans were created for five prostate cancer patients treated with the Novalis system and parameters were compared to the Novalis treatment p

Patterns of nodal failure after intensity modulated radiotherapy for nasopharyngeal carcinoma.

OBJECTIVES: To evaluate the sites of nodal failure (NF) of nasopharyngeal carcinoma (NPC) treated with intensity-modulated radiation therapy (IMRT). STUDY DESIGN: Retrospective chart review. METHODS: We reviewed the records of 165 patients with nonmetastatic NPC treated with IMRT between July 1998 and April 2011 at our institution. Recurrent nodes were delineated on imaging and coregistered with the original treatment planning computed tomography. Failures were assessed as in-field, out-field, or marginal based on the relative volumes of the recurrent nodes covered by the original dose distr

Endorectal balloon reduces anorectal doses in post-prostatectomy intensity-modulated radiotherapy.

BACKGROUND AND PURPOSE: To investigate the effect of an endorectal balloon (ERB) on anal wall (Awall) and rectal wall (Rwall) doses in high-dose post-prostatectomy intensity-modulated radiotherapy (IMRT). MATERIALS AND METHODS: For 20 patients, referred for salvage IMRT after prostatectomy for prostate cancer, two planning CT-scans were performed: one with and one without an air-filled ERB. A planning target volume (PTV) was defined, using international guidelines. Furthermore, the Awall and Rwall were delineated. In both the scans, IMRT plans were generated with a prescribed dose of 70 Gy. T

New approach for treatment of vertebral metastases using intensity-modulated radiotherapy.

PURPOSE: To perform aggressive radiotherapy for vertebral metastases. Using very steep dose gradients from intensity-modulated radiotherapy (IMRT), a protocol based on the concept of partial volume dose to the spinal cord was evaluated. PATIENTS AND METHODS: 50 patients with vertebral metastases were treated using IMRT. In previously unirradiated cases, where a prescribed dose of 80 Gy (BED10) was delivered, the constraint to the spinal cord should be less than 100 Gy (BED2). For previously irradiated cases, on the other hand, the dose is the same as in the previously unirradiated case; howev

The effect of intensity-modulated radiotherapy on radiation-induced second malignancies.

PURPOSE: To compare intensity-modulated radiotherapy (IMRT) with three-dimensional conformal radiotherapy (3D-CRT) in terms of carcinogenic risk for actual clinical scenarios. METHOD AND MATERIALS: Clinically equivalent IMRT plans were generated for prostate, breast, and head-and-neck cases treated with 3D-CRT. Two possible dose-response models for radiocarcinogenesis were generated based on A-bomb survivor data corrected for fractionation. Dose-volume histogram analysis was used to determine dose and its distribution to nontargeted tissues within the planning CT scan volume and thermolumines

Intensity-modulated radiotherapy for previously irradiated, recurrent head-and-neck cancer.

The purpose of this work is to evaluate our initial experience in treating previously irradiated, recurrent head-and-neck cancers using intensity-modulated radiotherapy (IMRT). Between July 1997 and September 1999, 12 patients with previously irradiated, locally recurrent head-and-neck cancers were treated with IMRT. These included cancers of the nasopharynx, oropharynx, hypopharynx, larynx, paranasal sinus, skin of the head-and-neck region, and malignant melanoma. Five of these 12 patients had received radiation as the primary treatment, with doses ranging from 66.0 to 126.0 Gy, and the remai

Fractionated stereotactic intensity-modulated radiotherapy (FS-IMRT) for small acoustic neuromas.

Eight patients with acoustic neuromas were treated using a novel method developed at our institution for delivering fractionated stereotactic intensity-modulated radiotherapy (FS-IMRT). We present treatment parameters, dosimetry analysis, and preliminary clinical outcome. The method incorporates high-precision invasive fixation, obliquely-oriented tomotherapy arcs, and reduced dimension pencil beams. The delivered dose distributions for the 8 patients treated from April 1999 to May 2001 were assessed for dose conformality, homogeneity, and doses to organs at risk. Total doses prescribed were 5

[Intensity modulated radiotherapy with dynamic multileaf collimator. Technique and clinical experience].

Since early 1997, dynamic multileaf collimators (DMLCs) have been used in our division for intensity modulated radiotherapy (IMRT). We have used IMRT to: irradiate concave targets (head and neck, paraspinal tumors); combine beams with shallow hinge angles (mediastinum, lung tumors); deliver intentionally inhomogeneous dose distributions (prostate, paranasal sinuses, brain tumors). IMRT is now our standard treatment for locoregional relapse (after high-dose radiotherapy) for head and neck cancer and for radical treatment of localized prostate cancer. For a variety of other tumors, conventional

Intensity-modulated radiotherapy for paranasal sinuses and base of skull tumors.

Paranasal sinus and skull base tumors are rare aggressive head and neck cancers, and typically present in the locally advanced stages. As a result, achieving wide surgical resection with clear margins is a challenge for these tumors, and radiotherapy is thus usually indicated as an adjuvant modality following surgery to optimize local control. Given the integral role of radiotherapy in the management of this subgroup of head and neck tumors, the advent of intensity-modulated radiotherapy (IMRT) has led to substantial improvement of clinical outcomes for these patients. This is primarily driven

[Intensity-modulated radiotherapy in head and neck cancer: ethics and methodology].

Numerous studies have shown that intensity-modulated radiation therapy is the standard technique for the radiation treatment of head and neck cancers. Intensity-modulated radiation therapy reduces side effects (xerostomia, dysphagia, fibrosis, etc.) and improves the results for cancer localizations with highly complex shapes such as the cavum or nasal cavity. Intensity-modulated radiation therapy is also a costly technique that necessitates a numerous staff, highly trained, with regular practice. If this technique cannot be available (understaffing, overwork, etc.) the choice between entrustin
