Non-Recurrent Laryngeal Nerve: Surgical Risk of Injury in Comparison With the Normal Recurrent Laryngeal Nerve-A Systematic Review and Meta-Analysis.

INTRODUCTION: The non-recurrent laryngeal nerve (NRLN) is a known anatomical variation of the normal recurrent laryngeal nerve (RLN). Its prevalence is estimated to be < 1% and has a higher risk of iatrogenic injury. The risk during thyroid surgery has been reported variably in current literature, from 0% to 12%. This study aimed to systematically review the incidence of NRLN injury and present the results through meta-analysis. The incidence of injury to NRLN and RLN was compared statistically to determine its significance. METHODS: The PRISMA protocol was followed for an online search of pr

Effect of Melatonin Administration on Nerve Regeneration after Recurrent Laryngeal Nerve Injury.

Recurrent Laryngeal Nerve (RLN) injury is a complication in neck surgery. The aim of this study is to evaluate the effect of primary suture repair with melatonin treatment on nerve regeneration after RLN damage. After the RLN damage, nerve repair was performed in the first and fourth groups. The third and fourth groups were given intraperitoneal melatonin therapy daily for six weeks. EMG was applied to all subjects and vocal cord movements were evaluated endoscopically. At the end of the sixth week, all subjects were sacrificed, and their larynx were examinedhistologically. Vocal cord paralysi

Recurrent Laryngeal Nerve Injury In Total Versus Subtotal Thyroidectomy.

BACKGROUND: Both Total and Subtotal Thyroidectomy are correct treatment options for symptomatic Euthyroid Multinodular Goitre. The choice depends upon surgeon's preference due to consideration of disadvantages like permanent hypothyroidism in Total Thyroidectomy and high chances of recurrence in Subtotal Thyroidectomy. Many surgeons believe that there is a higher incidence of Recurrent Laryngeal nerve injury in Total Thyroidectomy which affects their choice of surgery. This study aimed to compare the incidence of recurrent laryngeal nerve injury in total versus subtotal thyroidectomy. METHODS

Synkinesis following recurrent laryngeal nerve injury: A computer simulation.

OBJECTIVES/HYPOTHESIS: When the recurrent laryngeal nerve (RLN) is injured, functional recovery may be limited by the number of axons that regrow across the site of injury, and by the proportions of these axons that reinnervate the antagonistic muscle (synkinesis). This process was investigated in a computer model of RLN recovery. STUDY DESIGN: Computer simulation. METHODS: The developed computer program accepted as inputs: number of RLN axons; proportions of axons originally innervating adductor versus abductor, fraction of axons transected, fraction of axons that grow back, and width of 1

Recurrent laryngeal nerve monitoring during mediastinoscopy: predictors of injury.

BACKGROUND: Recurrent nerve injuries occur during mediastinoscopy despite assiduous technique. We evaluated mediastinoscopy by monitoring laryngeal nerve stimulation during the surgery. These techniques utilize sensing electrodes on laryngeal masks to evaluate stimulus of the larynx, and are used to identify recurrent nerves during redo neck surgery. METHODS: Fifteen patients were monitored during the entire mediastinoscopy. The laryngeal sensor was placed just before intubation. All patients had a suprasternal incision, digital dissection along the anterior wall of the trachea, and harvest o

Malpractice litigation after thyroid surgery: the role of recurrent laryngeal nerve injuries, 1989-2009.

BACKGROUND: Recurrent laryngeal nerve injuries remain a complication that is a source of concern to both surgeons and patients. RLN monitoring has gained popularity in recent years despite a lack of evidence showing decreased rates of recurrent laryngeal nerve injury when nerve monitoring is used. We sought to explore malpractice litigation in thyroid surgery with respect to recurrent laryngeal nerve monitoring. With increased public awareness and surgeon use of recurrent laryngeal nerve monitoring, we hypothesize an increase in its use in malpractice litigation in the area of thyroid surgery.

New insights into the mechanism of injury to the recurrent laryngeal nerve associated with the laryngeal mask airway.

With the popular use of the laryngeal mask airway (LMA), recurrent laryngeal nerve injury associated with the LMA, which might cause serious complications, has recently been reported. The true mechanism, however, remains unclear, which makes effective prevention and treatment of this disorder difficult. On the basis of the anatomical relationship of the laryngopharynx and the laryngeal mask and the progressive recovery in most reported cases, we hypothesize that demyelinating neuropraxia resulting from direct mechanical compression contributes to the recurrent nerve injury associated with the

Influence of Netrin-1 on reinnervation of laryngeal muscles following recurrent laryngeal nerve injury.

Following recurrent laryngeal nerve (RLN) injury, recovery results in poor functional restitution of the paralyzed vocal fold. Netrin-1 has been found to be upregulated in the rat posterior cricoarytenoid muscle (PCA) during nerve regeneration. We evaluated the effect of ectopic Netrin-1 in the PCA during RLN reinnervation. The right RLN was transected and Netrin-1 was injected into the PCA (2.5, 5, 10, 15, 20μg/ml). At 7 days post injury fluorescent retrograde tracer was injected into the PCA and Thyroarytenoid (TA) muscles. At 9 days tissues were harvested. Immunostaining showed reinnervatio

Unintentional recurrent laryngeal nerve injuries following thyroidectomy: Is it the surgeon who pays the bill?

BACKGROUND: Thyroidectomy is one of the most common intervention in general surgery and, after the turn of the century, its rate has sharply increased, along with a worldwide increased incidence of differentiated thyroid cancers. Therefore, injuries of the recurrent laryngeal nerve have become one of the most frequent cause of surgical malpractice claims, mostly following surgery for benign pathology. MAIN BODY: Even if the incidence of definitive paralysis is generally lower than 3%, during the last 20 years in Italy, the number of claims for damages has sharply raised. As a consequence, a l

Characteristics of Nerve Injury, Regeneration, and Vocal Fold Movement After Recurrent Laryngeal Nerve Electrocautery Injury.

OBJECTIVE: To investigate the temporal patterns of regeneration and functional recovery following a unilateral recurrent laryngeal nerve (URLN) electrocautery injury. METHODS: Unilateral recurrent laryngeal nerve(RLN) injury rabbit models (n = 12) were established. At 2, 4, 8, and 12 weeks post-injury, RLN and neuromuscular junction regeneration were evaluated via transmission electron microscopy and immunofluorescence. Thyroarytenoid muscle histopathology was assessed by hematoxylin and eosin staining, and vocal fold (VF) movement was quantified via laryngoscopy. Correlation between neuroele

Nerve monitoring decreases recurrent laryngeal nerve injury risk for neoplasm-related thyroidectomy.

OBJECTIVE: Conflicting reports exist regarding the benefit of intraoperative neuromonitoring (INM) for patients undergoing thyroidectomy. We hypothesized that in a national sample, the risk of mild and severe RLNi is decreased for patients undergoing neoplasm-related disease (NRD) thyroidectomy with INM compared to patients without INM. METHODS: The database was queried for patients that underwent total thyroidectomy for NRD with and without INM. A multivariable logistic regression model was used to determine the associated odds of RLNi. RESULTS: From 6942 patients, 4269 (61.5%) had INM duri

Continuous intraoperative vagus nerve stimulation for identification of imminent recurrent laryngeal nerve injury.

BACKGROUND: Conventional intraoperative nerve monitoring, predicated on intermittent stimulation, can predict recurrent laryngeal nerve (RLN) palsy only after the damage has been done. METHODS: Fifty-two patients (52 nerves at risk) who underwent continuous intraoperative nerve monitoring (CIONM) for thyroid surgery via vagus nerve stimulation had their electromyographic (EMG) tracings recorded and correlated with surgical maneuvers and postoperative RLN function. RESULTS: There was 1 imminent loss of signal (LOS) with intraoperative signal recovery and there were 4 losses of signal with cor
