Quantitative assessment of vestibular otopathology in granulomatosis with polyangitis: A temporal bone study.

OBJECTIVE: To investigate the temporal bone histopathology of vasculitis, especially in the vestibular organs, in granulomatosis with polyangitis (GPA). METHODS: Using light and differential interference contrast microscopy, we examined 12 human temporal bones from six deceased GPA patients and 12 histopathologically normal human temporal bones from six deceased age-matched patients. RESULTS: In the GPA group, three patients had undergone tympanostomy tube placement. Two of them had suffered mixed hearing loss; one, sensorineural hearing loss; and one, conductive hearing loss. Of the 12 spec

Pathogenesis of experimental endolymphatic hydrops.

The protein content and the d.c. potential of the endolymph differs in the various parts of the endolymphatic space (cochlea, utricle, semicircular canals and endolymphatic sac) as also does the ion composition (chloride, potassium, sodium). 12 months after obliteration of the endolymphatic sac and duct in guinea pigs the d.c. potential falls, whereas, the sodium activity increases. The endolymphatic hydrops is not caused by an increased colloid osmotic pressure. The increased water-binding capacity of the cochlear endolymph is correlated with the increased Na+ activity.

Magnetic resonance imaging of the temporal bone in patients with Ménière's disease.

Ménière's disease (MD) is still controversial in several aspects. The vestibular aqueduct, the osseous channel that carries the endolymphatic duct and sac, has previously been studied by tomography and computed tomography, with shortening and narrowing of this structure observed. These findings are apparently correlated to the development of the endolymphatic hydrops present in MD and related to its episodic symptoms. In studying the endolymphatic duct, the key structure in the pathology of this disease, magnetic resonance imaging (MRI) studies of the temporal bone were performed in 12 patient

Congenital anomalies of the ear resulting from cyclophosphamide treatment in the rat.

A single dose of cyclophosphamide (20 mg/kg) administered on day 12 of gestation to CF rats resulted in microtia in 97.5% of fetuses at term. The ears of affected fetuses were low set and dorsally placed. Histological examination revealed persistence of meatal plug, branching of the primordium of external acoustic meatus, periotic hemorrhages, narrowing of tympanic cavity, presence of only one to two primordia of middle ear ossicles, hypoplasia of stapedial artery, a maximum of two turns of cochlea and under differentiation of the organ of Corti and the semicircular ducts. Hemorrhages in and a

Three-dimensional Fourier transformation constructive interference in steady state magnetic resonance imaging of the inner ear in patients with unilateral and bilateral Menière's disease.

OBJECTIVE: In this study, three-dimensional Fourier transformation constructive interference in steady state (3DFT-CISS) magnetic resonance imaging was used to quantify the distance between the vertical part of the posterior semicircular canal and the posterior fossa as a measure of the endolymphatic sac and duct in patients with Menière's disease. Differences in this distance between affected and unaffected ears, as well as differences between unilaterally and bilaterally affected patients, were studied and compared with a control group. Also, possible correlations between the measured distan

A theoretical consideration of the mechanisms underlying auditory symptoms in patients with superior semicircular canal dehiscence syndrome.

BACKGROUND: We were astonished after reading reports that superior semicircular canal dehiscence can cause auditory hypersensitivity such as hearing sounds of eye movements. The mechanisms underlying auditory hypersensitivity remain incompletely understood. OBJECTIVES: The aim of this hypothesis-generating, theoretical consideration was to propose a new theory regarding the mechanisms responsible for auditory symptoms in patients with superior semicircular canal dehiscence and to explore related aspects of auditory physiology. METHOD: Auditory symptoms were analyzed through 23 published repo

BACKGROUND: Sclerosing sweat duct carcinoma (SSDC) is a rare variant of sweat gland carcinomas. We report a case of SSDC of the eyelid. CASE: A 33-year-old woman presented with a painless, slowly enlarging nodule in her left lower eyelid. Incisional biopsy revealed proliferated ducts lined by two layers of epithelium embedded in a dense fibrous stroma. Cytologic atypia was not evident. Initial histopathological diagnosis was a syringoma or an inflammatory reaction. OBSERVATIONS: Four years after initial biopsy, the mass had enlarged slightly. Madarosis was observed thereafter. The lesion was

Hypothetical mechanism for vertigo in Meniere's disease.

Past theories that have been proposed to account for the attacks of vertigo during the course of Meniere disease are reviewed. In the past, vascular theories and theories of perilymph and endolymph mixing due to ruptures or leakages were proposed. Recent research concerning the basic mechanisms of the inner ear anatomy and function cast doubt on these theories. The anatomy, physiology, and pathophysiology of the inner ear, and in particular of the endolymphatic sac and endolymphatic duct are reviewed. Recent studies suggest that in people the endolymph ionic content is replenished without any

Dlx5 and Dlx6 homeobox genes are required for specification of the mammalian vestibular apparatus.

The mammalian inner ear is a complex organ that develops from a surface ectoderm into distinct auditory and vestibular components. Congenital malformation of these two components resulting from single or multiple gene defects is a common clinical occurrence and is observed in patients with split hand/split foot malformation, a malformation which is phenocopied by Dlx5/6 null mice. Analysis of mice lacking Dlx5 and Dlx6 homeobox genes identified their restricted and combined expression in the otic epithelium as a crucial regulator of vestibular cell fates. Otic induction initiates without incid

Cell specificity of aquaporins 0, 3, and 10 expressed in the testis, efferent ducts, and epididymis of adult rats.

Aquaporins (AQPs) are transmembrane protein channels that allow the rapid passage of water across an epithelium at a low energy requirement, though some also transport glycerol, urea, and solutes of various sizes. At present, 11 members of the AQP family of proteins have been described in mammals, with several being localized to the testis (AQP-7 and AQP-8), efferent ducts (AQP-1 and AQP-9), and epididymis (AQP-1 and AQP-9) of adult rats. With the discovery of expression of multiple AQPs in different tissues, we undertook a systematic analysis of several other members of the AQP family on Boui

Computerized tomography of the petrous bone in Menière's disease.

High resolution transverse axial computerized tomograph (CT) encompassing the lateral semicircular canal (LSC) was used to measure the size of the petrous bone in ears with bilateral (n = 13) and unilateral (n = 38) Menière's disease, and in ears with unilateral chronic otitis media (n = 25) as a control group. Measurements were made of the following 4 distances in the petrous bone; A: the distance between the posterior semicircular canal (PSC) and the posterior petrous surface (PPS) (P-P distance), B: the distance between the PSC and the LSC (P-L distance), C: the distance between the vestibu

The winged helix transcription factor Fkh10 is required for normal development of the inner ear.

Fkhl0 is a member of the forkhead family of winged helix transcriptional regulators. Genes encoding forkhead proteins are instrumental during embryogenesis in mammals, in particular during development of the nervous system. Here we report that mice with a targeted disruption of the Fkh10 locus exhibit circling behaviour, poor swimming ability and abnormal reaching response-all common findings in mice with vestibular dysfunction. These animals also fail to elicit a Preyer reflex in response to a suprathreshold auditory stimulation, as seen in mice with profound hearing impairment. Histological

A Revised Surgical Approach to Induce Endolymphatic Hydrops in the Guinea Pig.

Endolymphatic hydrops is an enlargement of scala media that is most often associated with Meniere's disease, though the pathophysiologic mechanism(s) remain unclear. In order to adequately study the attributes of endolymphatic hydrops, such as the origins of low-frequency hearing loss, a reliable model is needed. The guinea pig is a good model because it hears in the low-frequency regions that are putatively affected by endolymphatic hydrops. Previous research has demonstrated that endolymphatic hydrops can be induced surgically via intradural or extradural approaches that involve drilling on

Gap junctional connections between hair cells, supporting cells and nerves in a vestibular organ.

The pattern of gap-junctional connections between cells in the vestibular neuroepithelium of the posterior semicircular duct of the alligator lizard are described based upon the study of freeze fracture replicas and ultrathin sections with a transmission electron microscope. Both type I and type II hair cells are coupled to adjacent supporting cells by a series of small macular gap junctions located in a ring around the hair cell at the level of the apical circumferential belt of actin filaments. Adjacent supporting cells are extensively interconnected by gap junctions. A few cases of gap junc

Cellular distribution of parchorin, a chloride intracellular channel-related protein, in various tissues.

The cellular distribution of parchorin, a new chloride intracellular channel family member, was investigated in rabbit tissues by immunohistochemistry using an antibody recognizing the sequence containing a parchorin-specific repeat. Parchorin preferentially resides in the epithelium of the ducts of the lacrymal, parotid, submandibular, and mammary glands and the pancreas, prostate, and testis. In the trachea and lung, parchorin was found in the airway epithelium and type II alveolar cells. In the kidney, parchorin was distributed mainly from the thick ascending limb to the distal convoluted t

Pathogenesis and pathophysiology of Meniére's disease.

Meniére's disease, neither spontaneous nor inducible in animals, is studied only in patients. Natural history (epidemiology) shows a triad of major symptoms: vestibular, auditory, and aural pressure. One in 3 patients has bilateral Meniére's; over full lifespans, bilaterality approaches 50%. Aural pressure (74.1%) and positional vertigo during/between attacks (85.9%) are common. Clinical variants can persist for 25+ years. All forms have delayed onset and can occur years after incitement, after otosclerosis, infections like otitis media, syphilis, or trauma. Endolymphatic hydrops is found in a

Photon-counting detector computed tomography for temporal bone: does higher resolution matter?

PURPOSE OF REVIEW: Photon-counting detector computed tomography (PCD-CT) represents a major technological advancement in computed tomography, offering improved spatial resolution, reduced image noise, and increased dose efficiency compared with conventional energy-integrating detector CT (EID-CT). Because many clinically relevant structures within the temporal bone are submillimeter in size, the temporal bone represents an ideal region for determining whether these technical improvements translate into meaningful clinical benefits. This review evaluates the emerging literature on PCD-CT in tem

High-resolution 7-Tesla magnetic resonance imaging and post-processing for 3-dimensional reconstruction of the membranous labyrinth in healthy adults.

The membranous labyrinth of the inner ear is a complex network of endolymph-filled structures critical for auditory and vestibular function. Pathological distension of these spaces, termed endolymphatic hydrops (EH), is associated with disorders such as Ménière's disease (MD). However, diagnosing inner ear pathologies remains challenging due to limitations in traditional imaging techniques, which lack the spatial resolution required to assess these intricate structures. Advances in 7-Tesla (7T) magnetic resonance imaging (MRI) now allow for high-resolution visualization of the inner ear. In th
