Why can't we fund supported employment?

Work is one of the most valued social rules in modern society, contributing to a person's sense of economic well-being, self-esteem, personal identity, and social status. Conversely, the inability to work or sustain employment due to a psychiatric condition is the primary factor in determining eligibility for disability benefits, such as Social Security Disability Income or Social Security Supplemental Income. Just as work is valued strongly by society, it is also important to people with serious mental illness. In this editorial, we contemplate why we cannot "do the right thing" and fully fun

Severe mental retardation in children from socially disadvantaged families.

Children receiving ESN(S) education, attending special day schools, and living at home, were categorized into those whose handicap had been medically recognized in infancy and those of unknown aetiology, 'non-specific'. Information obtained from interviews with their families was used to identify a subgroup of those who were relatively socially disadvantaged. Both factors, medical and social, are shown to have a significant effect on early provisions for special needs. The non-specific cases from disadvantaged families tended to have been recognized at a later age than the rest; their parents

Wearing many hats: The art of addiction psychiatry in private practice.

ObjectiveTo argue that contemporary addiction psychiatry, particularly in private and mixed-sector practice, is best understood as an integrative clinical craft rather than a narrow detoxification or prescribing role.ConclusionsAddiction psychiatrists increasingly work in rooms where trauma, ambivalence, psychiatric comorbidity, physical illness, family distress, medication complexity, social adversity, and crisis arrive together. Good care requires more than technical knowledge. It requires trauma-informed engagement, careful listening, psychotherapy fluency, formulation, medical competence,

Predictors and course of vocational status, income, and quality of life in people with severe mental illness: a naturalistic study.

Due to high unemployment rates, people with mental illness are at risk of poverty and are deprived of the social and psychological functions of work, such as the provision of social support, structuring of time, and self-esteem, with a negative effect on their perceived quality of life (QoL). Two distinct processes are held responsible for the low work force participation of people with mental illness: 'Social underachievement' and 'social decline'. Social underachievement signifies that, due to early illness onset, the educational attainment of people with mental illness is low and entry to t

Domestic Factors as Determinant of Sickness Absence with Psychiatric Disorders: A Scoping Review of Nordic Research Published between 2010-2019.

Uneven division of domestic factors may contribute to sex differences in sickness absence with psychiatric disorders. The aim of this scoping review was to compile current Nordic research on domestic factors and sickness absence with psychiatric disorders. A systematic search was performed to identify studies from the Nordic countries published between 1 January 2010 and 31 December 2019. Twelve studies were included. Marital status, family situation, work-home interference (in both directions), social affiliation, and loss of child/young adult (suicide, accident, or natural death) were identi

[Madness in freedom: experiences in Crato-CE (1930-1970)].

This study aims at identifying the most meaningful characters in the social imaginary and narrating its experience. This is a social-historical study in which oral (semi-structured interviews) and written documental sources were used. The interpretation of data was based on the archeological studies by Foucault on the history of madness and on other authors who worked with the themes: psychiatric madness and psychiatric reform. The narration of madness in the scenery of Crato indicates the existence of a multiplicity that reveals diversity, transcending the traditional psychiatric paradigm, wh

Psychiatric morbidity after myocardial infarction.

Using a standardized interview, psychiatric morbidity was diagnosed in 35 our of 100 consecutive male patients one week after admission to hospital following a first acute myocardial infarction. Sixteen of these patients had been psychiatrically ill before the infarction and their psychiatric symptoms and social difficulties persisted throughout the 12 month period of observation. In contrast, patients whose psychiatric morbidity had been precipitated by the infarction tended to have transient symptoms and fewer problems of social adjustment. Measures of psychiatric morbidity one week after th

Self- and proxy-reported impaired social interaction in young adults with simple congenital heart defects.

BACKGROUND: Simple Congenital Heart Defects such as septal defects constitute a large proportion of Congenital Heart Defects. New research has demonstrated more co-morbidities than previously thought. In particular, co-morbidities involving neurocognitive, psychiatric, and social difficulties have been described. Neurocognitive and psychiatric morbidities affect social interaction. Social interaction is important in everyday social life (education, work life, family life). In this study, we investigated social interaction through self- and proxy-answered Social Responsiveness Scale 2 (SRS-2) i

The impact of prior psychiatric medical treatment on return to work after a diagnosis of breast cancer: A registry based study.

Purpose: Breast cancer and psychiatric disorders negatively impact work life, both positively associated with unemployment and early retirement. Our purpose was to assess whether being prescribed psychiatric medication, 2-4 yrs prior to a diagnosis of breast cancer, could impact the likelihood of returning to work after cancer therapy. Methods: 16,868 self-supporting women, diagnosed with breast cancer in Denmark from 2000 to 2012, were identified from a population-based clinical database, then cross-referenced to data held for psychiatric medication usage, sociodemographics, and labour-market

Work Accommodations and Natural Supports for Employees with Severe Mental Illness in Social Businesses: An International Comparison.

Little is known about the types of work accommodations and natural supports that are useful for people experiencing severe mental illness working in social businesses. We conducted an exploratory, descriptive and cross-sectional investigation in Australia, Canada and Italy to study the nature of work accommodations and natural supports available in social businesses. Study findings are drawn from survey responses of a convenience sample of 90 employees with self-reported psychiatric disabilities. Results showed that, regardless of the country, social businesses provide many work accommodations

The inpatient psychiatric unit as both a safe and unsafe place: implications for absconding.

Absconding from acute psychiatric inpatient units is a significant issue with serious social, economic, and emotional costs. A qualitative study was undertaken to explore the experiences of people (n = 12) who had been held involuntarily under the local mental health act in an Australian inpatient psychiatric unit, and who had absconded (or attempted to abscond) during this time. The aim of the study was to explore why people abscond from psychiatric inpatient units, drawing on published work from health geography on the significance of the person-place encounter, and in particular the concept

Relationship of binge drinking to alcohol dependence, other psychiatric disorders, and behavioral problems in an American Indian tribe.

The hypothesis that binge drinking is a benign behavior not associated with alcohol dependence, other psychiatric disorders, or problem areas, in American Indians, was tested in a sample of 582 adult Southwestern American Indian males and females in large multigenerational pedigrees. All information was obtained from semistructured psychiatric interviews that were independently blind-rated for DSM-III-R diagnoses. Three main outcome measures were used: the relationship between binge drinking and (1) alcohol dependence and other psychiatric disorders, (2) substance abuse treatment, and (3) four

Cooperation and contention in psychiatric work.

This article discusses the social organization of psychiatric work in the psychiatric emergency department of a public general hospital located in New York City, based on ethnographic research conducted from 1999 to 2001. Case studies of the care of two patients with ambiguous symptoms are discussed. The analysis applies the ''differences approach'' developed by Mol and colleagues which focuses on the way different professions provide divergent explanations and ontologies for symptoms and illness. The cases illustrate the ways in which social structural constraints are compelling psychiatry to
