Retrograde ureteropyeloscopic treatment of 2 cm. or greater upper urinary tract and minor Staghorn calculi.

PURPOSE: Upper urinary tract calculi that are too large to treat with extracorporeal shock wave lithotripsy are most commonly cleared with percutaneous endoscopic techniques. In a select group of patients who were poor candidates for percutaneous nephrostolithotomy we used retrograde endoscopic lithotripsy, and define the safety and efficacy of this modality in treating large, noninfectious stone burdens (2 cm. or greater). MATERIALS AND METHODS: A total of 51 patients with 66 large (2 cm. or greater) upper urinary tract stones were chosen for retrograde ureteroscopic surgery. Many of these p

Retrograde intrarenal surgery with intelligent control of renal pelvic pressure for staghorn calculi: a case report.

Percutaneous nephrolithotomy is the gold standard treatment for staghorn calculi. However, this study reviews a case of an almost complete removal of staghorn calculi following one session of retrograde intrarenal surgery with intelligent control of renal pelvic pressure (RIRS-ICP). A 45 years-old female patient with an 8.3 × 4.5 cm complete staghorn stone was infected with Proteus mirabilis. Two sensitive antibiotics, piperacillin tazobactam and etimicin, were administered for 3 days. Semirigid 7/8.4 Fr ureteroscope was used to treat the renal pelvis and upper calyceal calculi for 57 min. A 5

Extracorporeal shock wave lithotripsy as monotherapy for staghorn calculi--is reduced renal function a relative contraindication?

Extracorporeal shock wave lithotripsy as monotherapy for staghorn calculus is not without complications. We describe a case in which, due to markedly reduced renal function following lithotripsy for a staghorn calculus, broken fragments of calculi forming steinstrasse became cemented together to form a solid calcified tube extending from the renal pelvis to the ureteric orifice. This resulted in further loss of kidney function. The patient eventually required nephro-ureterectomy. Extreme care should be taken when using extracorporeal shock wave lithotripsy to treat staghorn calculi in kidneys

Mini-percutaneous nephrolithotomy with high-power holmium YAG laser in pediatric patients with staghorn and complex calculi.

INTRODUCTION: Shock wave lithotripsy (SWL) is the treatment of choice for most cases of renal lithiasis in children. Some cases, however, are refractory to SWL and may be associated with anatomic and metabolic changes or a large stone burden. In these circumstances, mini-percutaneous nephrolithotomy (mini-PCNL) is an option. OBJECTIVE: The aim was to assess the safety and efficacy of high-power holmium YAG (Ho:YAG) laser in mini-PCNL for staghorn calculi. PATIENTS AND METHODS: The clinical records relating to 35 mini-PCNLs (Table) performed between January 2008 and December 2012 in 33 patien

Complete staghorn calculi: random prospective comparison between extracorporeal shock wave lithotripsy monotherapy and combined with percutaneous nephrostolithotomy.

PURPOSE: We determined the preferred treatment of staghorn calculi. MATERIALS AND METHODS: Between January 1992 and December 1994 we performed a prospective, randomized, single center study involving 50 kidneys with complete staghorn calculi: 27 renal units were treated with extracorporeal shock wave lithotripsy (ESWL) monotherapy (group 1) and 23 were treated with combined (initial) percutaneous nephrostolithotomy with ESWL (group 2). The 2 treatment groups were compared regarding stone size, grade of collecting system dilatation and urine culture at presentation. The number of treatment ses

[Removal of staghorn calculi from the urinary tract with extracorporeal shock wave lithotripsy and endourologic treatment methods].

ESWL has been accepted as a method for treatment of urinary tract calculosis. In most cases with urinary tract calculi, the method has replaced the classic surgical procedure. Staghorn calculi are still too large to be simply managed with classic procedure; however, they may be successfully disintegrated (crushed) with the available lithotriptors, particularly with second generation lithotriptors such as LITHOSTAR. Technological innovations which appeared during the last two decades have induced sudden changes in the treatment of urinary tract calculosis. They were enabled by extracorporeal sh

Ureteroscopic fragmentation followed by extracorporeal shock wave lithotripsy: a treatment alternative for selected large or staghorn calculi.

Ureterorenoscopic fragmentation followed by extracorporeal shock wave lithotripsy (ESWL) is proposed as an addition to the alternatives of percutaneous nephrostolithotomy alone or with ESWL, or ESWL monotherapy for treatment of large (greater than 500 mm.2) or staghorn calculi. Six patients who failed percutaneous tube placement and 2 additional patients, all of whom had fragile-appearing, large volume (greater than 500 mm.2) renal calculi, were treated by primary ureteroscopy with semirigid or flexible instruments, fragmentation of the stone with the pulsed dye laser, ureteral stenting and su

Supine percutaneous nephrolithotomy for bilateral complete staghorn calculi in an L-shaped cross-fused renal ectopic anomaly.

A 68-year-old woman was diagnosed to have a left-to-right L-shaped crossed-fused kidney with staghorn calculi in both renal moieties on evaluation for abdominal pain. She underwent supine percutaneous nephrolithotomy using 3 tracts in 3 stages. By using 3-dimensional reconstructed computed tomography urogram, proper preoperative planning, appropriate patient positioning, ultrasound-guided access, intraoperative traction, and flexible nephroscopy, complete stone clearance can be achieved in seemingly difficult stones in anomalous kidneys.

Treatment of complete staghorn calculi by extracorporeal shock wave lithotripsy monotherapy with special reference to internal stenting.

Extracorporeal shock wave lithotripsy monotherapy was used to treat 41 patients with complete staghorn calculi. In 20 patients polyethylene angiographic pigtail catheters were inserted prophylactically as ureteral stents and the other 21 patients did not receive stents. Prophylactic stenting reduced the incidence of complications and the need for percutaneous nephrostomy tubes to relieve subsequent ureteral obstruction. Internal stenting maintained the sterility of the urinary tract and reduced the average hospital stay by a third. Residual stone fragments representing less than 5 per cent of

No staghorn calculi and none/mild hydronephrosis may be risk factors for severe bleeding complications after percutaneous nephrolithotomy.

BACKGROUND: To explore the risk factors for severe bleeding complications after percutaneous nephrolithotomy (PCNL) according to the modified Clavien scoring system. METHODS: We retrospectively analysed 2981 patients who received percutaneous nephrolithotomies from January 2014 to December 2020. Study inclusion criteria were PCNL and postoperative mild or severe renal haemorrhage in accordance with the modified Clavien scoring system. Mild bleeding complications included Clavien 2, while severe bleeding complications were greater than Clavien 3a. It has a good prognosis and is more likely to

An experimental model for extracorporeal renal surgery: ex situ removal of artificially induced renal staghorn calculi in ambiothermia.

An experimental model for the evaluation of surgical techniques and preservation in extracorporeal surgery is described: staghorn calculi were induced in 15 dogs by the injection of methylmethacrylate into the renal pelvis. Removal of the calculi was carried out 3-4 weeks later on a workbench, using microsurgical techniques and ex situ radiographies. 15 min of warm ischaemic and 6 h of ambiothermic preservation were well tolerated as could be demonstrated by determination of the serum creatinine following autotransplantation and immediate contralateral nephrectomy.

Incidentally detected squamous cell carcinoma of renal pelvis in patients with staghorn calculi: case series with review of the literature.

Squamous cell carcinoma of the renal pelvis is a rare neoplasm, often unsuspected clinically due to its rarity and ambiguous clinical and radiological features, and hence patients present at advanced stages resulting in poor prognosis. We report here four cases of incidentally diagnosed primary renal squamous cell carcinoma, treated at our hospital over a short span of one year, and review the relevant literature. Mean age of the patients (3 males, 1 female) was 60 years. All suffered from staghorn stones. Interestingly, renal carcinoma was unsuspected clinically in all patients. In one case,

[Transperitoneal nephrectomy of crossed ectopic kidney without fusion associated with stenosis of the pyeloureteral junction and pyelocaliceal staghorn calculi].

The authors report their experience on a case of crossed ectopic kidney without fusion associated with stenosis of the pyeloureteral junction and secondary pyelocaliceal staghorn calculosis. After a brief summary of the various types of kidney ectopia and related principles of operation they describe their case report underlying the rarity of their case and the surgical approach. They consider the anterior transperitoneal approach as the most indicated for this type of malformation as it permits a better surgical view of the kidney and of the vascular structures.

Single upper-pole percutaneous access for treatment of > or = 5-cm complex branched staghorn calculi: is shockwave lithotripsy necessary?

BACKGROUND AND PURPOSE: Percutaneous nephrolithotomy for staghorn calculi is reported to have a residual stone rate of 28%, while shockwave lithotripsy alone results in residual stones in approximately 50% of cases. Combination therapy, sandwich therapy, and multiple percutaneous accesses have also been advocated for staghorn stones. We believe these stones can often be removed with a staged procedure via a single upper-pole percutaneous access using flexible nephroscopy and the holmium:YAG laser. Our experience is reviewed. PATIENTS AND METHODS: The hospital records of patients having a cumu

X-Ray Free Ultrasound-Guided Percutaneous Nephrolithotomy with Endoscopic Combined Intrarenal Surgery Is Feasible, Effective, and Safe for Staghorn Calculi.

Background: Percutaneous nephrolithotomy under ultrasound-guided access (US-PCNL) has multiple advantages compared with fluoroscopy. No prospective data currently examine the efficacy of this technique for the management of staghorn stones. Our objective was to determine the safety and effectiveness of US-PCNL for the management of staghorn calculi through a prospective cohort study, using strict definitions of stone free based on postoperative computed tomography (CT) imaging. Methods: Utilizing a prospective cohort of US-PCNLs performed by a single surgeon between October 2020 and September

First case report of staghorn calculi successfully removed by mini-endoscopic combined intrarenal surgery in a 2-year-old boy.

Less-invasive therapy for pediatric urolithiasis is available due to the miniaturization of equipment and improved optics; however, surgical treatment strategies, especially for large calculi, remain controversial. We describe here our experience of treating a 2-year-old boy with left renal staghorn calculi with a single session of mini-endoscopic combined intrarenal surgery in the prone split-leg position with pre-ureteral stenting and the directional enhanced flow imaging ultrasound technique. This is the first report of successful pediatric mini-endoscopic combined intrarenal surgery withou
