Reducing the carbon footprint of hand trauma surgery under local anaesthetic.

INTRODUCTION: This study measured how much the carbon footprint and hospital costs could be reduced by using fewer surgical instruments and drapes in minor hand trauma surgery. METHODS: A retrospective cohort study of patients undergoing minor hand trauma procedures was carried out to compare carbon emissions between standard care and a reduced-waste protocol over a 4 month period. The size of drapes and instrument trays were reduced in the new protocol, having been identified as the most modifiable contributors to carbon footprint. Carbon emissions analysis and data on procedural costs and i

Comparison of Efficacy and Cost of Iodine Impregnated Drape vs. Standard Drape in Cardiac Surgery: Study in 5100 Patients.

We sought to examine the efficacy in preventing surgical site infection (SSI) in cardiac surgery, using two different incise drapes (not iodine-impregnated and iodine-impregnated). A cost analysis was also considered. Between January 2008 and March 2015, 5100 consecutive cardiac surgery patients, who underwent surgery in our Institute, were prospectively collected. A total of 3320 patients received a standard not iodine-impregnated steri-drape (group A), and 1780 patients received Ioban(®) 2 drape (group B). We investigated, by a propensity matched analysis, whether the use of standard incise

Antimicrobial Incise Drapes in Knee and Hip Arthroplasties: Meta-analysis of Randomized Controlled and Prospective Cohort Studies.

Traditional skin preparation and prophylactic antibiotics have not uniformly been successful in preventing surgical site infection (SSI) following total joint arthroplasties. Iodophor-impregnated adhesive dressings, such as Ioban, have shown promising effects in reducing the incidence of SSI. A systematic review and meta-analysis were conducted according to PRISMA checklist and the Cochrane Handbook for Systematic Reviews of Interventions. Non-stratified and stratified meta-analysis were conducted to test for confounding and biases. The methodological quality and risk of bias were analyzed and

[Lidocaine tape (Penles--a dressing tape based on 60% lidocaine--) reduces the pain of postherpetic neuralgia].

The treatment of postherpetic neuralgia (PHN) by topical administration of local anesthetics has a number of drawbacks. Lidocaine tape (Penles) is a 15 cm2 dressing tape based on 60% lidocaine used to anesthetize skin when an intravenous catheter is inserted. This study aims to evaluate the analgesic efficacy of lidocaine tape in patients with PHN by comparing the results with those of surgical drape (Tegaderm). In a single-blind, two session study, lidocaine tape or surgical drape was applied to the affected skin of 10 patients. In one session, lidocaine tape was applied to the painful skin a

Use of sterile adhesive film and polypropylene mesh in the construction of a temporary silo in the treatment of omphalocele.

When a primary closure is not feasible, a synthetic material must be used in patients with congenital abdominal wall defects. Sterile adhesive film (drape) is reinforced with polypropylene mesh, and then it is used as a prosthesis for constructing a temporary silo. The drape surface is applied to the herniated viscera, which provides a smooth surface that does not adhere to the bowel loops. The adhesive film (drape) is a sterile, impermeable, transparent, and flexible material, which is readily available in all operating rooms. When combined with polypropylene mesh, an effective prosthesis can

Single vs Repeat Surgical Skin Preparations for Reducing Surgical Site Infection After Total Joint Arthroplasty: A Prospective, Randomized, Double-Blinded Study.

BACKGROUND: Preventing surgical site infection (SSI) after total joint arthroplasty (TJA) is a high priority and is partly linked to the efficacy of surgical site preparation solutions (SPSs) in reducing the number of pathogens on the skin before incision. The aim of this study is to investigate the effectiveness of SRS reapplication after draping to reduce the incidence of SSI after TJA. METHODS: Six hundred patients undergoing primary TJA between 2010 and 2011 at a single institution were recruited and randomly assigned to one of 2 groups. The patients in the intervention group (n = 300) re

Green Cystoscopy: Does Minimizing the Use of Drapes Increase Infection Rates?

IMPORTANCE: The U.S. health care system has an enormous carbon footprint made worse by the escalating use of single-use supplies. Emerging evidence suggests that smaller surgical fields ("green" draping) may represent a safe alternative to traditional draping. OBJECTIVES: The aim of the study was to determine if the proportion of cases treated for culture-proven urinary tract infection (UTI) within 2 weeks of operating room cystoscopy after the green draping protocol implementation is noninferior to preprotocol cases. Secondary objectives included risk factors for UTI and waste and cost savin

Letter to the Editor regarding "Risk of surgical site infection associated with intraoperative smartphone use in a sterile cover during total knee arthroplasty".

PURPOSE: To critically examine the interpretation of the reported association between intraoperative smartphone use in a sterile cover and surgical site infection after total knee arthroplasty. METHODS: We appraised the study's infection definition and surveillance period, statistical precision, interpretation of nonsignificant findings, and reporting of the institutional infection-prevention bundle. RESULTS: Defining the outcome as periprosthetic joint infection within one year may capture events less plausibly attributable to intraoperative contamination. Reporting infections separately at

Minimizing Propionibacterium acnes contamination in shoulder arthroplasty: use of a wound protector.

BACKGROUND: Propionibacterium acnes may be transmitted from the subdermal tissues to the deeper tissues during shoulder arthroplasty surgery, resulting in deep infection. The aim of this prospective, clinical study was to determine whether the use of a wound protector drape can lower the incidence of P. acnes in the wound during shoulder arthroplasty surgery. METHODS: For a consecutive series of 47 patients undergoing shoulder arthroplasty, a wound protector drape was used during surgery, to isolate the subdermal layer from the surgeons' hands, retractors and other instruments. Delayed-onset contact dermatitis caused by olanexidine gluconate antiseptic solution: effect of wiping off the remaining drug solution in two cases.

BACKGROUND: Olanexidine glucuronide (Olanedine®), an antiseptic solution may cause skin dermatitis around one week after disinfection. Although removal after the procedure is recommended to avoid skin dermatitis, whether it is effective for preventing skin dermatitis has not been documented in detail in the literature. CASE PRESENTATION: We encountered two cases of delayed-onset contact dermatitis by Olanedine®. In both cases, the patient's back was disinfected with Olanedine® and was covered with a surgical drape for epidural catheterization. After catheterization and removal of the surgical

Environmental impact of single-use items and their contamination levels in elective orthopaedic surgery.

BACKGROUND: Single-use items have dominated operating theatres since the 1980s, initially perceived to carry a lower risk of surgical site infection and occupational blood exposure while maintaining hospital expenditure. The topic of reusable surgical gowns and drapes has naturally emerged due to discussions regarding climate change. Considering reusable alternatives is the next step in reducing the carbon footprint of operating theatres. METHODS: Clinical waste attributable to single-use items was collected from 93 cases in an elective orthopaedic hospital over four weeks, comprising knee ar

Does an Antimicrobial Incision Drape Prevent Intraoperative Contamination? A Randomized Controlled Trial of 1187 Patients.

BACKGROUND: The risk of periprosthetic joint infection (PJI), a serious complication after arthroplasty, has not changed for years. Interventions such as eradication of Staphylococcus aureus and antibiotic bone cement are used to diminish infection risk but despite these efforts, the percentage of infection in TKA remains constant. Antimicrobial drapes have a dual action, acting both as a physical and antimicrobial barrier to counter bacterial contamination of the surgical wound. To study the effect of antimicrobial drapes, we used intraoperative contamination as a proxy for infection in our i

Efficacy of wound protectors in reducing surgical site infections in patients undergoing open colorectal surgery: A systematic review and meta-analysis.

BACKGROUND: The incidence of surgical site infections (SSIs) has been reported to be significantly higher in colorectal surgeries compared to other surgical procedures. Wound protectors have been developed to help reduce the incidence of SSIs by creating a barrier between the incisional wound and the contaminated surgical field, but their efficacy remains uncertain. OBJECTIVE: To assess whether the use of wound protectors reduces the incidence of SSIs in patients undergoing open colorectal surgery. DATA SOURCES: A comprehensive literature review was conducted using PubMed, Embase, Web of Sci

Re-sterilization Prior to Implant Insertion in Prosthetic Breast Reconstruction: Is It Necessary?

PURPOSE: Breast operations have a surgical site infection rate of <1.5%, a figure that increases to 3% after implant-based reconstructions. We aimed to assess whether contamination occurred before implant insertion and could be related to post-operative infections. PATIENTS AND METHODS: The reconstruction team prepared the surgical field using a betadine solution and standard draping. After the surgical procedures, the antiseptic was re-applied around the incision, the surgical field re-draped, and the gloves changed before implant insertion. Immediately before the betadine application and re

A technical note to prevent superficial tumor cell dissemination at fungating soft tissue tumor resection.

Dissemination of the tumor within surgical field increases the risk of local recurrences. Fungating soft tissue tumors present a special risk of contamination in the attempt to perform wide resections. We hypothesized that adequate tumor coverage at resection could prevent tumor spilling and superficial dissemination in the operation field. For this purpose we used swabs to soak secretions. Incise drapes were put over swabs to seal the tumor. Double row skin staples were placed at the edge of the adhesive drapes for lift-off-prevention. We present eight patients treated with wide resection (3

A patient who was burned in the operative field: a case report.

Operating room fires occur very rarely. Nevertheless, a disaster can complicate almost any kind of surgery. The majority of operating room fires result from the use of alcohol- based surgical preparation solutions, electro-surgical equipment, or flammable drapes in an oxygen-rich environment. We report a patient with an ovarian cyst and uterine myomas who suffered a flame burn while undergoing gynecological surgery.

How to Manage Intraoperative Contamination.

UNLABELLED: Sterile technique is essential to protect patients and decrease the risk of surgical site infections. Despite efforts to maintain sterility, sterile breaches occur. Examples of such breaches include holes in surgical drapes, insects in the operating room, residual bioburden on surgical instruments, dropped grafts intraoperatively, and defective sterile tray wrappers. Intraoperative sterile breaches are stressful situations for the surgical team and require the surgeon to make quick decisions that have a large impact on the patient's infection risk and outcome. There is limited lite

Does Extent of Skin Preparation Influence Contamination During Direct Anterior Total Hip Arthroplasty? A Fluorescent Tracer Proof-of-Concept Study.

There is currently no evidence-based consensus regarding the optimal extent of skin preparation when performing total hip arthroplasty (THA) through the direct anterior approach using dedicated anterior-approach draping systems (DAA Drapes). The purpose of this study was to evaluate the contamination risk associated with different disinfection and draping strategies for anterior-approach THA using a proof-of-concept contamination model. Six commonly used skin preparation protocols were evaluated using a standardized fluorescent contamination model. Protocols ranged from extensive bilateral low
