Racism in Inuit experiences in healthcare: findings from an environmental scan.

While the category of "race" has no scientific foundation, race and subsequent racism continue to be determinants of access to health for Indigenous communities globally, including for Inuit in Canada. This scan raised concerns from community members voicing racial inequities in healthcare. The aim of this review was to scan Canadian Inuit experiences of racism in healthcare. To date, no such work has been done, and contemporary Inuit experiences of racism in the healthcare context have not been systematically documented. We hope that this review will serve as a foundation for scholars, policy

Addressing the Impacts of Racism on American Indian and Alaska Native Child Health.

The legacy of racism toward Native Americans is far-reaching. We will review the topic using the conceptual model of racism as a form of violence as it is inherent in racism, as are prejudice and power. Using the basic frameworks of racism as internalized, interpersonal, institutional, and structural, we will discuss the many types of racism affecting Native Americans today. Racism is the bedrock of generations of trauma experienced in Native communities. The generational/historical trauma of racism has led to epigenetic-level changes affecting Native American people today. We will cover the h

Socioeconomic Status Partially Mediates the Effects of Structural Racism on Youth Tobacco Use Initiation.

BACKGROUND: Recent research has identified structural racism-systemic policies and practices that perpetuate racial inequalities-as a significant social determinant of population health. Studies utilizing data from the Adolescent Brain Cognitive Development (ABCD) study have shown an association between higher levels of state-level structural racism and increased tobacco use among youth in the United States. However, there has been limited exploration of the psychosocial mediators of this relationship, particularly in the context of youth aged 10-16 years. OBJECTIVE: This study aimed to asses

Promoting Equitable Engagement in Attention-Deficit/Hyperactivity Disorder Care.

Attention-deficit/hyperactivity disorder (ADHD) is a treatable pediatric condition, but children with racial-ethnic minority backgrounds often do not receive timely or consistent treatment. Understanding how systemic racism impacts care and learning from families of color about their experiences can provide critical insights for improving clinical practice and engaging patients equitably in ADHD care. We interweave a mother's experience navigating ADHD care for her son with commentary from an interprofessional team about what clinicians can do for families to reduce the impact of systemic raci

Racism, Not Race: A Physician Perspective on Anti-Black Racism in America.

In this commentary, which responds to the article "Anti-Black Racism as a Chronic Condition," by Nneka Sederstrom and Tamika Lasege, I draw on my experience as a physician who cares for a largely Black patient population. Physicians are trained to "first do no harm" and strive to treat patients to the best of their abilities. However well intentioned, many of us fall short of this goal and witness the consequences of health inequities that disproportionally impact the lives of Black patients. Recent years have brought increased acknowledgment and understanding of social determinants of health,

The Traumatic Impact of Structural Racism on African Americans.

Many African Americans in the United States have been impacted by structural racism since slavery and continue to experience trauma because of health disparities, economic disadvantages, and segregation. This article will define race, racism, and structural racism, which has perpetuated trauma for African Americans. The authors present a theory called Post Traumatic Slavery Syndrome (PTSS) by Dr. Joy DeGruy, a social work researcher, to explain why many African Americans continue to experience trauma. PTSS is a condition that exists as a consequence of multigenerational oppression of African a

The Impact of Racism on the Health and Wellbeing of Black Indigenous and Other Youth of Color (BIPOC Youth).

Black, Indigenous, and other Youth of Color (BIPOC youth) experience racism from a young age. These experiences have both immediate and long-term impacts on their health and wellbeing. Systemic racism contributes to the inequitable distribution of health resources and other social determinants of health, creating barriers to accessing care. Substance use disorders and sexual/nonsexual risk behaviors have been linked to experiences of racism in BIPOC youth. The legacy of generational racial trauma can frame behaviors and attitudes in the present, undermining health and survival in this group. B

The Association of Racism and Discrimination in Disparities of Hypertensive Disorders of Pregnancy in the United States: An Analysis of PRAMS Data.

BACKGROUND: Hypertensive disorders of pregnancy are a leading cause of maternal mortality. Racial disparities in maternal outcomes such as maternal mortality in the United States (US) are well-documented, but the relationship of racism and/or discrimination with one's risk of developing a hypertension during pregnancy (HDP) is not well-studied. METHODS: Data from 17 sites that asked questions regarding experiences with racism and/or discrimination during pregnancy via the Pregnancy Risk Assessment Monitoring System (PRAMS) Phase 8 (2016-2020) was used. Anxiety-Related Disorders in the Context of Racism.

PURPOSE OF REVIEW: The literature on racism and anxiety-related disorders, especially social anxiety, specific phobia, and generalized anxiety disorder, is notably lacking. This report aims to review recent evidence demonstrating the link between racial discrimination and various anxiety-related disorders. RECENT FINDINGS: Anxiety-related disorders were the most significant mediator for daily discrimination and suicidal thoughts, above both depression and substance use. Further, studies showed that racial discrimination promotes posttraumatic stress and racial trauma among people of color. Sy

Addressing Adultification of Black Pediatric Patients in the Emergency Department: A Framework to Decrease Disparities.

Systematic racism and structural discrimination in the United States are factors that have negatively affected the health of Black individuals and families. One form of racism that often goes unrecognized and yet has a profound negative impact on the health and safety of Black children is adultification. Adultification occurs when children are perceived, or treated, as being older than they are. We implemented the Racism as a Root Cause framework to identify ways health care providers and health care systems can work to dismantle inequities and address the adultification of Black children in t

Structural Racism, the Social Determination of Health, and Health Inequities: The Intersecting Impacts of Housing and Mass Incarceration.

Public health researchers have directed increasing attention to structural racism and its implications for health equity. The conceptualization of racism as historically rooted in systems, structures, and institutions of US society has important implications for addressing social determinants of health (SDOH). It requires theorizing SDOH as embedded in and expressions of racially oppressive historical structures that are manifested in and maintained by policies, programs, and practices in multiple domains that dynamically intersect to reinforce and reproduce in new ways: race inequities in hea

Toward a Theory of the Underpinnings and Vulnerabilities of Structural Racism: Looking Upstream from Disease Inequities among People Who Use Drugs.

Structural racism is increasingly recognized as a key driver of health inequities and other adverse outcomes. This paper focuses on structural racism as an "upstream" institutionalized process, how it creates health inequities and how structural racism persists in spite of generations of efforts to end it. So far, "downstream" efforts to reduce these health inequities have had little success in eliminating them. Here, we attempt to increase public health awareness of structural racism and its institutionalization and sociopolitical supports so that research and action can address them. This pa

Depressive and Insomnia Symptoms Sequentially Mediate the Association Between Racism-Based Discrimination in Healthcare Settings and Clinical Pain Among Adults With Sickle Cell Disease.

Racism-based discrimination in healthcare settings has been associated with clinical pain in adults living with sickle cell disease; however, no studies have examined depressive and insomnia symptoms as mechanisms that may drive this relationship. This secondary data analysis examined associations between depressive and insomnia symptoms, racism-based discrimination, and clinical pain. Seventy-one adults with sickle cell disease (70% female, Mage = 38.79) provided baseline reports of racism-based discrimination, depressive symptoms, insomnia symptoms, and pain (severity, interference, catastro

Is doing better science enough?: An autoethnographic study of a multidisciplinary collaboration to measure structural racism.

OBJECTIVES: Addressing structural racism remains a critical challenge in population health and social sciences, as traditional quantitative approaches often fail to disrupt long-standing racialized health hierarchies. This study critically interrogates the process of a multidisciplinary scientific collaboration attempting to operationalize and measure life course structural racism while enacting an anti-racist praxis. DESIGN: Utilizing a collaborative analytic autoethnography framework grounded in social cognitive theory and anti-racist praxis, our team - comprising biostatisticians, nurse sc

"Cell Lines" and "Syphilitics": A Critical Investigation into Medical Racism, Rhetorical Dehumanization, and Health Genres.

This essay investigates rhetorical dehumanization in the US related to systemic racism, sexism, and classism. While I revisit well-known cases of medical exploitation in the stories of Henrietta Lacks and the Tuskegee Syphilis Study, I examine them through a new lens by undertaking a rhetorical analysis of medical journal articles written on them, demonstrating how the genre of scientific writing, with its emphasis on objectivity and neutrality, mitigated concerns about the suffering and mortality of Black Americans and overlooked exploitation in the name of scientific advancement. I conclude

Race and ethnic variation in college students' allostatic regulation of racism-related stress.

Racism-related stress is thought to contribute to widespread race/ethnic health inequities via negative emotion and allostatic stress process up-regulation. Although prior studies document race-related stress and health correlations, due to methodological and technical limitations, they have been unable to directly test the stress-reactivity hypothesis in situ. Guided by theories of constructed emotion and allostasis, we developed a protocol using wearable sensors and daily surveys that allowed us to operationalize and time-couple self-reported racism-related experiences, negative emotions, an

The Association between State-Level Structural Racism and Alcohol and Tobacco Use Behaviors among a National Probability Sample of Black Americans.

BACKGROUND: Structural racism is how society maintains and promotes racial hierarchy and discrimination through established and interconnected systems. Structural racism is theorized to promote alcohol and tobacco use, which are risk factors for adverse health and cancer-health outcomes. The current study assesses the association between measures of state-level structural racism and alcohol and tobacco use among a national sample of 1,946 Black Americans. METHODS: An existing composite index of state-level structural racism including five dimensions (subscales; i.e., residential segregation a

Conceptualizing the Impacts of Racism on Racialized Midwives in Ontario: An Alert to the Profession.

INTRODUCTION: There is a research gap on how racism impacts the mental health of midwives in Ontario. Our aim was to conceptualize the impact of racism on racialized midwives in Ontario. METHODS: Informed by constructivist grounded theory, we analyzed data contributed by racialized midwives in Ontario who participated in focus groups and interviews as part of a larger study about mental health. Participants had practiced midwifery within the past 15 months. RESULTS: Seven participants from 2 focus groups and one individual interview were included.
