Update on materials used in intracoronal bleaching.

Intracoronal bleaching has emerged as a popular technique to bleach discoloured, endodontically treated anterior teeth. Although hydrogen peroxide and sodium perborate have traditionally been used in intracoronal bleaching, some concern has been expressed regarding the use of hydrogen peroxide, mainly because of the suspected "cause and effect" relationship between hydrogen peroxide and invasive cervical root resorption. Other complications associated with use of hydrogen peroxide alone or in combination with sodium perborate include increased dentine permeability, alteration in the chemical s

Long-term Success of Nonvital, Immature Permanent Incisors Treated With a Mineral Trioxide Aggregate Plug and Adhesive Restorations: A Case Series from a Private Endodontic Practice.

This case series evaluated the long-term clinical outcome of nonvital immature teeth treated with mineral trioxide aggregate (MTA) as an apical barrier and an adhesive restoration with or without a fiber post. Eighty-three teeth in 72 patients were treated by the first author with an apical MTA plug and an adhesive restoration of composite resin and in 45 of the 83 teeth 1 or more fiber posts. All of the patients had been referred to the first author's private endodontic practice with at least 1 immature tooth with signs of pulpal necrosis and subsequent apical periodontitis that had been caus

Periapical Granuloma: A Case Report.

A periapical granuloma is a fleshy mass of tissue invaded by varied numbers of chronic inflammatory cells, namely, epitheloid cells (modified tissue macrophages), mast cells, lymphocytes, and plasma cells, usually attached to the apex of a nonvital carious tooth as a sequelae of periapical infection from the diseased pulp. The growth of periapical granulomas develops in spurts amidst recurrent acute exacerbations. Periapical granulomas may form as the initial periapical pathosis or as a result of a quiescent periapical abscess. These lesions might not always be static. Radiographic changes are

Role of cementoenamel junction on the radicular penetration of 30% hydrogen peroxide during intracoronal bleaching in vitro.

Intracoronal bleaching of nonvital, teeth with 30% hydrogen peroxide is occasionally associated with external cervical root resorption. The exact mechanism by which bleaching induced root resorption occurs is not yet fully understood. The relationship of cementum to the enamel at the cementoenamel junction may have clinical significance. Seventeen single rooted human mandibular premolars extracted atraumatically for orthodontic reasons were used. The radicular hydrogen peroxide penetration in each tooth was measured in vitro by an indirect colorimetric method. Thereafter, the teeth were examin

Present status and future directions - Managing discoloured teeth.

Managing tooth discolouration involves a range of different protocols for clinicians and patients in order to achieve an aesthetic result. There is an increasing public awareness in the appearance of their teeth and management of tooth discolouration may be inter-disciplinary and involve both vital and nonvital teeth. Vital teeth can be easily treated with low concentration hydrogen peroxide products safely and effectively using an external approach and trays. For endodontically treated teeth, the walking bleach technique with hydrogen releasing peroxide products is popular. However, there is

Outcome of intentional replantation of endodontically treated teeth with periapical pathosis: A systematic review and meta-analysis.

The purpose of the present review was to examine success, survival and failure following intentional replantation of endodontically treated teeth with existing periapical pathosis and to determine the factors that might affect the outcome of replantation. Clinical trials, longitudinal studies, case series with >10 cases and at least 1-year follow-up were included. The average rate of success following intentional replantation was 77.23%. Meta-analysis revealed the mean weighted survival to be 85.9% (95% CI: 79.6-91.2) Common complications include inflammatory root resorption (0%-27%) and ankyl

Apexification of an Immature Permanent Incisor with the Use of Calcium Hydroxide: 16-Year Follow-Up of a Case.

Apexification is a process of forming a mineralized apical barrier and had been performed by using calcium hydroxide paste, due to its biological and healing performances in cases of existent trauma. This clinical report aims to report the results of a 16-year follow-up study of an apexification treatment applied to nonvital tooth 22 of a healthy 8-year-old male after a trauma. Clinical inspection of the tooth showed fractures of the incisal edge and mesial angle, absence of coronal mobility, and negative pulp vitality under cold testing. Radiographic analysis of the root revealed incomplete a

Orthokeratinized odontogenic cysts presenting as a periapical lesion: report of a case and literature review.

INTRODUCTION: Inflammatory cysts, granulomas, abscesses, and fibrous scars represent most periapical radiolucencies. However, other less common lesions, such as orthokeratinized odontogenic cysts (OOCs), can be found at this region, and they deserve to be discussed because the prognosis for an OOC is different from that expected for the ordinary inflammatory periapical diseases. METHODS: An interesting case of OOC associated with a nonvital tooth in a 40-year-old woman is described. After a previous clinical diagnosis of a radicular cyst, the tooth was extracted, and the lesion was enucleated

Radiographic features of vertically fractured, endodontically treated maxillary premolars.

OBJECTIVE: The purpose of this study was to evaluate the most frequent radiographic appearance of bony lesions associated with vertically fractured roots of endodontically treated maxillary premolars. STUDY DESIGN: The radiographic features of 102 endodontically treated teeth and their periradicular areas (51 with and 51 without vertically fractured roots) were evaluated and compared. RESULTS: The predominant appearance of the periradicular area in the teeth with vertically fractured roots was the "halo" lesion (57%); by contrast, in the non-vertically fractured roots group, a "periapical" r

Immense potential for self-healing with conservative management of a large radicular cyst associated with pulpectomized primary tooth: A 40-month follow-up.

Radicular cysts are one of the most common cysts of inflammatory origin involving the maxillofacial region that arise from the remnants of Herwig's epithelial root sheath. These are often diagnosed as incidental findings on radiographs, which are seldom symptomatic and are slowly progressive. Decompression could be used as a conservative treatment approach for the management of large radicular cysts in children to have satisfactory healing and minimum damage to adjacent vital structures, simultaneously allowing the eruption of succedaneous tooth. This paper discusses the management of a large

Atypical radiographic presentation of a horizontal mid-root fracture in a maxillary central incisor tooth.

Prognosis of a horizontal mid-root fracture is favorable, primarily because the dental pulp tends to maintain its vitality and the fracture segments are completely intraalveolar. Healing usually occurs with deposition of calcified tissue. However, if the segment coronal to the fracture becomes nonvital and infected, healing occurs by interposition of granulation tissue. This report describes a case of a horizontal mid-root fracture in a right maxillary central incisor tooth, where the apical fractured segment was significantly displaced in a linear direction. This was attributed to the pressur

A Comparison of Volume of Tissue Removed and Biomechanical Analysis of Different Access Cavity Designs in 2-rooted Mandibular First Molars: A Multisample 3-dimensional Finite Element Analysis.

INTRODUCTION: The aim of this study was to compare the biomechanical properties and the amount of coronal tissue removed among the different access cavities with a multisample 3-dimensional finite element analysis in the mandibular first molar. The correlation between the amount of tissue removed and the fracture resistance of the teeth was also analyzed. METHODS: Micro-computed tomography data from 20 2-rooted mandibular first molars were included in this study as 3-dimensional modeling prototypes. Models of untreated molars and molars treated with the traditional access cavity (TradAC), the

Periodontal and Periapical Outcomes of Surgical Extrusion: A Prospective Clinical Volumetric Study.

INTRODUCTION: Outcome predictors for the restoration of endodontically treated teeth mainly include cavity wall integrity and the presence of a 1.5-2 mm ferrule. However, in some clinical situations, the aforementioned predictors cannot be achieved. Three different techniques have been proposed to obtain an adequate ferrule respecting the supracrestal tissue attachment: surgical crown lengthening, orthodontic forced eruption, and surgical extrusion. There are little published prospective data evaluating periodontal changes after surgical extrusion. Therefore, the aims of this study were to rep

[Partsch's chronic granulomatous inflammation, the cutaneous manifestation of a dental cause].

Dentogenous inflammatory diseases can lead to typical dermatological facial symptoms with formation of cutaneous sinuses. Partsch's chronic granulomatous inflammation can result from conducted inflammation of a nonvital tooth via a chronic apical inflammation. In this rare disease, the granulomatous tissue perforates the bone, channels through the overlying skin, and drains via cutaneous or oral sinuses. A frequent localization of the cutaneous sinus is the skin inferior to the body of the mandible, and it is caused by an inflammation of the lower molars. Treatment consists of identifying the

Median palatine cyst.

Median palatine cysts are rare, nonodontogenic lesion of the hard palate that do not involve the palatine papilla or incisive canal and that usually present as asymptomatic, fluctuant swellings. They may involve the floor the nasal cavity but are treated by simple enucleation, without recurrence. They are composed histologically of a fibrous collagenous tissue wall, with infiltration of chronic inflammatory cells, and lined by stratified squamous and/or respiratory epithelium. They are differentiated from the nasopalatine and other anterior maxillary cysts by the following diagnostic criteria:

Accelerated healing of endodontically treated teeth with a periapical lesion as a result of orthodontic extrusion: evaluation and rationale.

Resolution of a periapical periodontal lesion as determined by radiographic examination may take a year or longer. Orthodontic movement is known to have a positive effect on the bone topography and morphology. The literature presents no information about the effect of orthodontic movement on the rate of periapical endodontic healing following a root canal treatment. The aim of this report was to present the use of orthodontic root extrusion in cases with a periapical lesion, where other options are ruled out. Three case reports involving endodontically treated teeth with a periapical lesion a
