INTRODUCTION: The American College of Surgeons (ACS) has recently highlighted the need for emergency departments to maintain pediatric readiness, regardless of pediatric trauma center verification. Since sufficient emergency department volume is often required to maintain pediatric competency, a more complete understanding of pediatric trauma volume at centers without ACS verification would help inform efforts to improve pediatric trauma readiness.
METHODS: We conducted a retrospective descriptive analysis of public data sets from 2017 through 2023 from the Trauma Quality Improvement Program

Backgound The scaphoid is the most commonly fractured carpal bone and often presents as a diagnostic challenge. Fractures can often go unnoticed on initial radiographic evaluation and clinical presentation can vary significantly among patients. Awareness of high-risk cohorts assists practitioners in making the appropriate clinical diagnosis of acute scaphoid fracture. Materials and Methods The National Trauma Data Bank is the world's largest trauma data repository. Utilizing the 2016 public use file, we isolated scaphoid fractures by anatomic fracture location by utilizing International Clas

PURPOSE: Open lower limb fractures are resource intensive injuries. Regardless of the financing model, the cost of treatment is an important consideration for any healthcare provider.
METHODS: Open lower limb fractures treated at our centre were identified over a six-month period. Isolated open femur or tibia fractures were included as well as cases with multiple fractures. Direct inpatient care costs were calculated and income was reviewed for each case according to 'Healthcare Resource Group' (HRG) cost codes.
RESULTS: A total of 41 open lower limb fractures (32 patients) were identified.

BACKGROUND: State health departments and the American College of Surgeons focus on the availability of optimal resources to designate hospitals as trauma centers, with little emphasis on actual delivery of care. There is no systematic information on clinical practices at designated trauma centers. The objective of this study was to measure compliance with 22 commonly recommended clinical practices at trauma centers and its association with in-hospital mortality.
STUDY DESIGN: This retrospective observational study was conducted at 5 Level I trauma centers across the country. Participants were

To determine patient demographics, referral patterns, and problems with trauma transfer, records of 412 patients received over a 1-year period at the Vancouver General Hospital, a Level I Trauma Center, were reviewed. The average Injury Severity Score (ISS) was 15 (range: 1 to 75), with 142 patients (34%) having an ISS of 16 or greater. One hundred ninety-six (47%) of the transfers were accomplished by fixed-wing aircraft, 15 (4%) by helicopter, and 201 (49%) by land. Seventy-two percent were transported within 24 hours of injury. The injury mechanisms were as follows: 193 (47%) vehicular, 108

BACKGROUND: The American College of Surgeons Committee on Trauma level verification system directs severely injured patients to specialized centers, a strategy that has been associated with improved outcomes. The correlation between outcome of pediatric trauma patients requiring surgical procedures and verification status of the treating institution is not well established. This study aims to examine the association between outcomes following isolated severe traumatic brain injury (iSTBI) and pediatric trauma center (PTC) verification status.
METHODS: This study analyzed data from the America

BACKGROUND: Economic, social, and psychologic stressors are associated with an increased risk for abusive injuries in children. Prolonged physical proximity between adults and children under conditions of severe external stress, such as witnessed during the COVID-19 pandemic with "shelter-in-place orders", may be associated with additional increased risk for child physical abuse. We hypothesized that child physical abuse rates and associated severity of injury would increase during the early months of the pandemic as compared to the prior benchmark period.
METHODS: We conducted a nine-center

IMPORTANCE: The National Pediatric Readiness Project is a US initiative to improve emergency department (ED) readiness to care for acutely ill and injured children. However, it is unclear whether high ED pediatric readiness is associated with improved survival in US trauma centers.
OBJECTIVE: To evaluate the association between ED pediatric readiness, in-hospital mortality, and in-hospital complications among injured children presenting to US trauma centers.
DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study of 832 EDs in US trauma centers in 50 states and the District of Columb

BACKGROUND: Pediatric pelvic fractures are a significant source of morbidity for children in the United States. In the era of specialized care, the relationship between trauma center designation and outcomes remains unknown. We hypothesized that there would be no difference in patient outcomes when treated at adult trauma centers (ATCs), pediatric trauma centers (PTCs), or dual trauma centers (DTCs).
MATERIALS AND METHODS: We used the National Trauma Data Bank to identify pediatric (≤14 y) patients suffering pelvic fractures in 2013-2015. DTCs were defined as centers with level I or II trauma

Questionnaires were mailed to 620 U.S. "trauma surgeons" to determine a consensus regarding indications for inferior vena caval (IVC) filter placement; 210 (34%) responded. Eighty-seven percent of respondents practiced in Level I trauma centers; 78% were in urban areas and 75% reported more than 1,000 trauma admissions per year. One-half (52%) of those responding were "trauma directors" at their centers. Filter insertion was done by radiologists at 81% of centers, by trauma surgeons at 34%, by vascular surgeons at 33%, and by general surgeons at 13%. Each month, 60% of trauma centers inserted

Clinical data on the nature and severity of injuries was linked with data from police crash reports for 3675 car or truck drivers admitted to trauma centers. Different patterns of injuries were noted for drivers in frontal compared with left lateral collisions. Injuries to the face and lower extremities were significantly greater in frontal collisions; thorax, abdominal, and pelvic injuries were significantly greater in lateral collisions. In addition, drivers in lateral collisions were found to have significantly more multiple injuries to the abdomen and thorax. Despite no difference in mean

Children treated at facilities prepared to provide emergency pediatric care have improved outcomes, but access to pediatric-ready care remains inconsistent. A multidisciplinary spotlight session at the 2026 American College of Surgeons Committee on Trauma Annual Meeting examined barriers to pediatric readiness across US trauma systems and practical strategies to address them. Participants identified limited dedicated personnel, financial and equipment constraints, fragmented regional coordination, inconsistent requirements, and barriers to completing and acting on pediatric readiness assessmen

OBJECTIVE: Ohio is consistently ranked as one of the worst states for opioid overdose deaths. Traumatic injury has been linked to opioid overdose deaths, yet the location of trauma centers has not been explored. We examined whether geospatial clustering occurred between county-level opioid overdose deaths (OODs) and trauma center levels.
METHODS: We obtained 2019 county-level data from the Ohio Department of Health for fatal overdoses from prescription opioids. We obtained the total number of opioid doses prescribed in 2019 per county from the Ohio Automated Rx Reporting System and American C

The aim of this study was to compare different treatment modalities for different severities of pediatric radial neck fractures in a large cohort of patients and determine prognostic factors. A retrospective, comparative study was conducted of all children treated for different severities of radial neck fractures at two level-1 pediatric trauma centers between 1990 and 2007. Pertinent data were collected, and the outcome was measured by the Tibone criteria. Several variables were compared to determine the prognostic value. Seventy-eight children were identified, 35 male and 43 female, at an av

BACKGROUND: Trauma is the leading cause of death for individuals under age 45 and disproportionately affects underserved populations. Trauma recidivism is defined as recurrent hospital admissions for injury-a substantial burden on healthcare systems. In regions such as Allegheny County, Pennsylvania, which has a large, aging population, recidivism is often influenced by social, behavioral, and structural determinants. This study examines patterns of trauma recidivism at an urban tertiary trauma center and identifies associated risk factors to guide targeted interventions in both hospital and c

INTRODUCTION: Subway-related trauma is an understudied category of urban injury. Prior work has focused on high-acuity train-contact events, with less attention to more common mechanisms and the roles of psychiatric illness and substance use. We analyzed a large contemporary cohort to define epidemiology, injury patterns, and outcome predictors across the full spectrum of subway trauma.
METHODS: We performed a retrospective cohort study of adults presenting with subway-related injury to a Level I trauma center (2018-2024). Registry data and manual chart review captured demographics, mechanism

PURPOSE: The purpose of this study is to delineate transfer patterns to a rural Level 2 pediatric trauma center by identifying common diagnoses associated with potentially avoidable transfers (PAT) to a higher level of care with the end goal of optimizing resource utilization in the center's catchment area.
METHODS: Using trauma registry data, every pediatric trauma patient (< 15 years old) transferred to our Level 2 trauma center from January 2017 to December 2023, was included. "Potentially avoidable transfer" was characterized as a patient who, upon arriving at the trauma center, did not u

BACKGROUND: The management of geriatric trauma patients is challenging because of the altered physiology and co-existent medical conditions. To study the in-hospital mortality profile of geriatric trauma victims and the parameters associated with the mortality, we conducted this retrospective analysis.
METHODS: In a retrospective review of geriatric trauma admissions (above 60 years) over a 3-year period, we studied the association of age, gender, comorbidities, mechanism of injury (MOI), Glasgow coma score (GCS), injury severity score (ISS), systolic blood pressure, and hemoglobin (Hb) level
