[Stapled transanal rectal resection (STARR) in the treatment of rectocele: personal experience].

Rectocele is an organic cause of chronic constipation, with a prevalence ranging from 8.95% to 12% in Europe and United States. Necessarily, the approach for rectocele repair is a surgical operation. Stapled transanal rectal resection (STARR) is safe and effective in the treatment of obstructed defecation syndrome. The authors' experience suggests that the surgical operation needs to be combined to rehabilitation exercises, before and after the surgical treatment, in order to strengthen the muscles of the pelvic pavement. From January 2005 to January 2007, 20 patients with outlet obstruction u

Functional results after laparoscopic rectopexy for rectal prolapse.

We investigated the functional results after laparoscopic rectopexy for rectal prolapse in 29 patients at least 12 months postoperatively. Twenty patients were evaluated completely pre- and postoperatively (median 22 months postoperatively, range 12 to 54 months). Six patients were interviewed by telephone, two patients were lost to follow-up, and one patient died of causes unrelated to rectal prolapse. Patients underwent a proctologic examination, anoscopy, rigid sigmoidoscopy, fluoroscopic defecography, and anorectal manometry pre- and postoperatively, and an additional standardized intervie

[Advances in functional assessment and bowel rehabilitation following intersphincteric resection for low rectal cancer].

Intersphincteric resection (ISR) has been performed as an ultimate sphincter-sparing strategy in selected patients with low rectal cancer. Accumulating evidence suggests that ISR may be an interesting alternative to abdominoperineal resection to avoid a permanent stoma without compromising oncological outcomes. However, bowel dysfunction is a most common consequence of ISR not to be neglected. To date, limited clinical research has reported functional and quality of life outcomes according to patient-reported outcome measures. Also, data concerning management of low anterior resection syndrome

Chronic Constipation in the Elderly Patient: Updates in Evaluation and Management.

Chronic constipation (CC) is a common disorder in the elderly population globally and is associated with comorbidities and negative implications on the quality of life. Constipation prevalence varies in different studies, primarily owing to the nonuniformity of the diagnostic criteria. However, 15%-30% of individuals aged >60 years are diagnosed with CC. Primary care physicians are the main healthcare providers that manage constipation in elderly patients in parallel with increased population aging and increased prevalence of constipation. Physical inactivity, polypharmacy, chronic medical con

[Domestic violence and constipation: an integrative review].

OBJECTIVE: To search the literature for evidence of a relationship between domestic violence and constipation. METHODS: An integrative review was conducted using the six-step approach and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). We examined articles published between 2005 and 2015 investigating the correlation between domestic violence and constipation. The search was performed in September and October of 2015 in PubMed, MEDLINE, Scopus, and Web of Science. The selection of articles was performed in three stages: search, pre-selection, and inclusion. R

Dynamic magnetic resonance imaging of the pelvic floor in children and adolescents with vesical and anorectal malformations.

BACKGROUND: Magnetic resonance imaging (MRI) of the pelvic floor allows better assessment of pelvic pathology and has a potential as an adjunct for therapy planning. In complex congenital malformations of the pelvic floor and continence organs, it plays a major role in assessing urinary and fecal incontinence or constipation, especially when performed as a dynamic investigation such as MRI defecography. PATIENTS AND METHODS: Twenty-three patients (3-21 years old) with urinary and/or fecal incontinence or constipation attributable to congenital malformations of the pelvic region presented at o

Rectoanal inhibition and rectocele: physiology versus categorization.

Some authors divide rectoceles into those with chronic evacuatory difficulty and normal genital position (type 1) and those with associated pelvic organ prolapse (type 2). This study assessed whether there are physiological differences between these two clinical rectocele types. Female patients were assessed by conventional anorectal manometry, vector manometry, parametric assessment of the rectoanal inhibitory reflex (RAIR), and defecography. Subjects included 33 volunteer controls without anorectal disease, 14 patients with type I rectocele, and 26 patients with type II rectocele. Significan

Resection rectopexy for rectal prolapse. The laparoscopic approach.

BACKGROUND: Resection rectopexy through open laparotomy is an established procedure for the treatment of rectal prolapse. METHODS: Resection rectopexy was successfully performed in 10 multiparous women by the laparoscopic approach (LAP), and the results were compared to those of eight women with laparotomy resection rectopexy (OPEN). Preoperative and postoperative assessment included anorectal manometry, defecography, and measurement of large-bowel transit. RESULTS: The duration of the operation was longer in the LAP than in the OPEN group (p < 0.01). Morbidity was lower (p < 0.01) and hospi

Defecatory disorders, anorectal and pelvic floor dysfunction: a polygamy? Radiologic and manometric studies in 41 patients.

A consecutive series of 41 patients with defecatory disorders was prospectively studied by anal manometry and evacuation proctography to determine the relationship between abnormalities and symptoms. The patients (29 female, 12 male, aged 41 +/- 2.3 years) all complained of difficulty in evacuation. All had normal colonoscopy and biochemistry. There was no evidence of megacolon or megarectum, and no symptoms had been previously treated by pelvic floor surgery. All subjects completed detailed questionnaires related to gastrointestinal symptoms with special reference to excessive straining and d

The Decline of Sensory-Motility Coordination in Rectal Evacuation due to Pelvic Floor Dysfunction.

BACKGROUND: Constipation, a common defecatory disorder, exhibits various symptoms and individual variability. Abnormalities in rectal sensation and/or motility are prevalent among patients. However, the mechanisms linking rectal sensation and motility, and their impact on rectal evacuation function, remain poorly understood. METHODS: Our study gathered data from 154 chronic constipation patients, including rectal sensation and motility, electromyography, motor evoked potentials, defecography, and balloon expulsion test. The aim was to investigate the association between rectal sensation and m

Absence of a rectocele may be correlated with reduced internal anal sphincter function in patients with rectal intussusception and fecal incontinence.

PURPOSE: Fecal incontinence (FI) is common in patients with rectal intussusception (RI), although the mechanism behind its formation is unclear. Recent data indicate that a reduction in internal sphincter tone may cause FI, which becomes notable with increasing RI levels. However, the roles of other anatomical abnormalities in anal function remain unclear. This study assessed the relationships between various pelvic floor abnormalities and anal sphincter function in patients with RI and FI. METHODS: Data for patients with RI, collected in a prospective pelvic floor database, were assessed ret

Balloon expulsion test as a screen for outlet obstruction in children with chronic constipation.

OBJECTIVE: Chronic constipation (CC) is a common problem in pediatrics and is often the result of obstructed defecation. The aim of the present study was to study the feasibility and efficacy of the balloon expulsion test (BET) in the diagnosis and management of children with CC. METHODS: Retrospective study comparing BET and high-resolution anorectal manometry (ARM). The BET was done together with ARM in 29 children, ages 8 to 19 years, with CC. For BET, a 60-mL balloon was used. Passage of balloon in 1 minute or less was considered normal. RESULTS: Fifteen of the 29 children had a normal B

[Pelvic floor dyssynergia: videoproctographic analysis and pathologic associations in defecation obstruction syndrome].

INTRODUCTION: Pelvic floor dyssynergia is included pathophysiologically in the functional dyschezia group. It is characterized by the paradoxical contraction or lack of relaxation of the puborectal muscle and/or external sphincter during defecation, with consequent functional outlet obstruction. The diagnosis is not always easy because there is no really specific test, nor any diagnostic gold standard; also, many pathophysiologic and epidemiologic findings are still unknown. We tried to define the diagnostic criteria of this condition with the statistical analysis of the main defecographic par
