Surgical management of HIV-associated lipodystrophy: role of ultrasonic-assisted liposuction and suction-assisted lipectomy in the treatment of lipohypertrophy.

PURPOSE: HIV-associated lipodystrophy is a frequent consequence of highly active antiretroviral therapy and has been associated with several metabolic disorders (increased triglycerides, hypercholesterolemia, insulin resistance) as well as altered fat distribution, including lipohypertrophy (neck, trunk, breasts) and lipoatrophy (nasolabial fold, cheek, extremities). Medical treatment of fat redistribution is usually ineffective. We evaluated the efficacy and safety of the surgical management of HIV lipodystrophy. METHODS: We performed a retrospective review of 12 consecutive patients (3 fema

Possibilities of intranasal reconstruction in complex nasal defects.

BACKGROUND: Complex nasal defects most often arise due to oncological resection or severe trauma. Traditional methods of two-stage nose reconstruction using a forehead flap with a skin graft have often resulted in collapse and deformity of the nose with a very compromised outcome over time. These techniques were gradually replaced by new procedures consistently reconstructing the intranasal lining, most often with flaps from the nasal septum. These methods reconstruct the cartilaginous and bony support of the nose as well, while the skin cover of the nose is, nowadays, in large defects, recons

[Total nostril wing reconstruction: Attitudes evaluation of reconstructive facial surgeons].

Cutaneous nasal defects, often resulting from surgical excisions for cancerous, traumatic, or congenital lesions, represent a major challenge in reconstructive surgery. The aim of this single-center retrospective study was to evaluate surgical practices and factors influencing the choice between two reconstruction techniques : the nasolabial flap (NLF) and the folded paramedian forehead flap (PFF) according to Menick's technique. Between January 2017 and March 2024, 23 patients with full-thickness alar defects, with or without extension to the nasal tip, were included. Patients were divided in

Three-dimensional facial soft-tissue changes after L-shaped reduction malarplasty: A pilot retrospective study.

PURPOSE: Reduction malarplasty (RM) is a common facial contouring aesthetic procedure in East Asia. However, there is a lack of data on the postoperative changes in soft tissue and patient-reported outcomes. This retrospective study aimed to evaluate the soft-tissue changes and surgical outcomes following L-shaped RM. METHODS: Fifty-five patients who underwent L-shaped RM were enrolled and 55 patients who underwent genioplasty were enrolled as controls. The soft-tissue changes in relation to the underlying skeletal tissue were analyzed using computed tomography (CT) and three-dimensional (3D)

A rare case of gingival necrosis following an extraoral injection of dermal filler.

BACKGROUND: The utilization of dermal fillers for cosmetic procedures has increased dramatically in the past several years, and so is the number of reported adverse events and complications. Most of these reports are centered on extraoral facial structures such as skin, nose, forehead, and eyes. However, as this report illustrates, intraoral complications are also possible and require appropriate management. Our extensive search of the medical and dental literature has failed to yield any such report. METHODS AND RESULTS: An injection of dermal filler composed of hyaluronic acid for managing

Ophthalmic Complications of Periorbital and Facial Aesthetic Procedures: A Literature Review.

The emergence and popularity of cosmetic facial procedures may lead to significant ophthalmic complications such as ocular motility dysfunction and visual disability. Here, we present a scoping review to identify common ophthalmic complications in some facial plastic surgeries and cosmetic injections, and to develop clinical approaches for prophylaxis and management in terms of direct attention and awareness of non-ophthalmologists toward such scenarios and appropriate intervention. This review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (P

Cytogenomic characterization of a de novo 4q34.1 deletion in a girl with mild dysmorphic features and a coagulation disorder.

BACKGROUND: 4q deletion syndrome is a rare chromosomal disorder that mostly arises de novo. The syndrome is characterized by craniofacial dysmorphism, digital abnormalities, skeletal alterations, heart malformations, developmental delay, growth retardation, Pierre Robin sequence, autistic spectrum and attention deficit-hyperactivity disorder, although not every patient shows the same features. Array comparative genomic hybridization (aCGH) use improves the detection of tiny chromosomal deletions and allows for a better understanding of genotype-phenotype correlations in affected patients. AI-based classification of pediatric sagittal skeletal patterns using multi-view facial photographs.

OBJECTIVE: The increasing global demand to assess pediatric skeletal malocclusions poses a growing challenge, as current screening methods are dependent on availability of orthodontists and X-rays, which limits accessibility. This study aimed to develop and validate an artificial intelligence (AI) model based on facial photographs to predict pediatric sagittal skeletal pattern classifications and to explore its feasibility for radiation-free early screening of pediatric skeletal malocclusion. METHODS: Facial photographs and lateral cephalograms were collected retrospectively from a cohort of

Does 810-nm Diode Laser Photobiomodulation Improve Sensory Recovery After Le Fort I Surgery?

BACKGROUND: Neurosensory disturbances are common after Le Fort I osteotomy, often transient but clinically relevant, highlighting the need for strategies to enhance recovery. Photobiomodulation (PBM) using low-level diode lasers has been proposed to accelerate nerve recovery. PURPOSE: The purpose of study was to compare midface sensory recovery and subject satisfaction between PBM with an 810-nm diode laser and a control side (inactive probe) following Le Fort I osteotomy. STUDY DESIGN, SETTING, SAMPLE: A prospective triple-blind, split-face randomized controlled trial was conducted from 202

Intradermal Micro-Dosing of AbobotulinumtoxinA for Face-Lifting: How Long Does It Last?

INTRODUCTION: Intradermal injection of botulinum toxin type A (BoNT/A) has been used off-label by many clinicians for face-lifting. Previous studies on abobotulinumtoxinA (ABO) (Dysport®; Ipsen Biopharm Ltd.) have demonstrated clinical efficacy in face-lifting when compared to normal saline solution (NSS). However, few clinical studies have evaluated ABO in terms of duration of sustained effects for face-lifting. METHODS: Thirty subjects were enrolled, and their face was injected with ABO at a dilution of 1 vial:7 mL (500 U in 7 mL of NSS) using an intradermal injection technique. Standardize

Anti-MDA5 Dermatomyositis Presenting With Lupus-Like Mucocutaneous Features and Inflammatory Arthritis: A Case Report.

Understanding diagnosis and management of specific autoimmune diseases is based on existing classifications or diagnostic criteria. However, patients frequently present with broad, multisystem features that mimic multiple autoimmune diseases. Recognizing clinical mimicry is crucial because overclassifying these features as distinct coexisting diseases often presents a diagnostic challenge. We report the case of a 36-year-old man who presented with progressive weakness. The patient initially complained of facial rash and skin lesions, which then developed into tetraparesis, arthralgia, and diff

Modulation of TRPV1 function by Citrus reticulata (tangerine) fruit extract for the treatment of sensitive skin.

BACKGROUND: Sensitive skin (SS) is a clinical syndrome defined by the occurrence of unpleasant sensations (such as stinging, burning, pain, pruritus, and tingling) in response to stimuli that normally should not provoke them. According to growing evidence, transient receptor potential vanilloid subtype 1 (TRPV1) has elevated expression in individuals with SS and is linked with the severity of SS symptoms. However, its pathogenesis is still unknown. OBJECTIVE: Herein, Citrus reticulata (Tangerine) fruit extract (CR) was obtained and examined for its effect on SS with a focus on TRPV1 stimulati

A novel technique for repositioning lower eyelid fat via the transoral approach in association with midface lift.

BACKGROUND: Orbital fat repositioning in association with subperiosteal midface elevation has been variably described via both the transconjunctival and skin muscle flap approaches. Poor visualization, middle and posterior lamellar cicatricial fibrosis, technical difficulty, and incomplete release are disadvantages commonly ascribed to the transconjunctival approach. Lower eyelid malposition and retraction also are commonly seen in association with skin muscle flap approaches. A simple technique using the intraoral approach to release the orbital septum and postseptal fat is described in this

[Anatomy of the superficial parts of the face and neck].

The author presents four chapters concerning classical anatomy, an anatomical contribution to new facelifts, aesthetic anatomy, which recalls the aesthetic relations of the face, and finally a brief review of the mode of ageing of the face. The embryology of the superficial plane of the face and neck shows that the development of muscles of facial expression, deep and superficial fatty tissues, the facial nerve and the parotid is the result of divergent migratory movements and confluences. The anatomy is discussed in two parts: the first concerns conventional anatomy, and reviews the anatomica
