Retinopathy is present in 60%-80% of long-term diabetics, and 5%-10% of diabetics surviving 20 yr from the time of diagnosis will be blind, mostly from retinopathy, which is now the commonest cause of newly diagnosed blindness in the 30-65 yr age group. The mean survival time after a diagnosis of retinopathy is only 5 yr. The natural history of diabetic retinopathy is now being understood more clearly. Mild background retinopathy, characterised by microaneurysms or scattered hard exudates, may progress to maculopathy, with macular vascular pathology leading to exudates and edema at the macula;

BACKGROUND: Manual diabetic retinopathy (DR) grading may prefer certain regions and underestimate others. It is unclear if inexperienced graders' initial marking strategy reflects a specialist's marking pattern.
AIMS: By the virtual ocular learning platform (VIOLA), we evaluated if initial DR-lesion markings among inexperienced graders spatially corresponded to a specialist-based lesion map.
MATERIALS AND METHODS: We used retinal 6-field VIOLA images with at least 500 DR-lesion markings covering all DR-levels (0-4) according to the international clinical diabetic retinopathy (ICDR) scale. We

Background: Despite advances in artificial intelligence (AI) for Diabetic Retinopathy (DR) classification, traditional severity-based approaches often lack interpretability and fail to capture specific lesion-centered characteristics. To address these limitations, we constructed the National Medical Center (NMC) dataset, independently annotated by medical professionals with detailed labels of major DR lesions, including retinal hemorrhages, microaneurysms, and exudates. Methods: This study explores four critical research questions. First, we assess the analytical advantages of lesion-centered

INTRODUCTION: There is growing evidence of a higher than expected prevalence of retinopathy in prediabetes. This paper presents the protocol of a systematic review and meta-analysis of retinopathy in prediabetes. The aim of the review is to estimate the prevalence of retinopathy in prediabetes and to summarise the current data.
METHODS AND ANALYSIS: This protocol is developed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Protocols (PRISMA-P) guidelines. A comprehensive electronic bibliographic search will be conducted in MEDLINE, EMBASE, Web of Scie

Diabetic retinopathy (DR) is the most common microvascular complication of diabetes, and retinal microaneurysms (MA) are one of the first detected abnormalities associated with DR. We recently showed elevated serum triglyceride levels to be associated with the development of MA in the Finnish Diabetes Prevention Study (DPS). The purpose of this metabolomics study was to assess whether serum fatty acid (FA) composition, plasmalogens, and low-grade inflammation may enhance or decrease the risk of MA. Originally, the DPS included 522 individuals (mean 55 years old, range 40-64 years) with impaire

PURPOSE: To differentiate diabetic macular edema (DME) from pseudophakic cystoid macular edema (PCME) based solely on spectral-domain optical coherence tomography (SD-OCT).
METHODS: This cross-sectional study included 134 participants: 49 with PCME, 60 with DME, and 25 with diabetic retinopathy (DR) and ME after cataract surgery. First, two unmasked experts classified the 25 DR patients after cataract surgery as either DME, PCME, or mixed-pattern based on SD-OCT and color-fundus photography. Then all 134 patients were divided into two datasets and graded by two masked readers according to a s

The galactose-fed beagle develops diabetes-like microvascular changes that are histologically and clinically similar in appearance to all stages of human diabetic retinopathy. This animal model is extremely useful for evaluating drugs for the treatment of diabetic retinopathy; however, the time required to develop the various retinal lesions (24-72 months for background to the proliferative stage) may be considered prohibitive. Retinal vascular changes begin with an initial degeneration of capillary pericytes, which has been linked to the aldose reductase catalyzed formation of galactitol. Bec

This thesis presents the results of a study of 384 eyes of 192 patients with a mean age of 39.1 years who presented with typical retinitis pigmentosa. The major findings are outlined below, together with suggested hypotheses: Cataract was found in 46.4% of the eyes. Among these, 93.6% showed posterior subcapsular opacification. The incidence of cataract increased with age. The vitreous degeneration that is characteristic of the RP syndrome and begins in childhood was described as showing dust-like, particulate matter throughout the gel; posterior vitreous separation; formation of a posterior m

PURPOSE: Fundus fluorescein angiography (FFA) is essential for diagnosing and managing retinal vascular diseases, yet its evaluation is time-consuming and subject to inter-observer variability. We aim to develop a deep-learning-based framework for multi-lesion segmentation in FFA images and to evaluate its primary performance on standard 55 ° images.
METHODS: A dataset of 428 standard 55 ° FFA images and 53 ultra-wide-field (UWF) FFA images was annotated for non-perfusion areas (NPA), microaneurysms (MA), neovascularization (NV) and laser spots. A residual U-Net framework was trained using a

Coats' disease is an idiopathic retinal vascular disorder, known to usually occur unilaterally; however, recent studies have highlighted vascular abnormalities in the fellow unaffected eyes. This retrospective study investigated the peripheral vascular features and macular vascular structure of unaffected fellow eyes in patients with unilateral Coats' disease using multimodal imaging tools. We analysed images of patients, including bilateral ultra-widefield imaging, fluorescein angiography (FA), ultra-widefield FA, or standard fundus photography.
