Effectiveness, safety, and implementation outcomes of a decentralization program to deliver antivenoms in the Western Brazilian Amazonia: The SAVING program.

BACKGROUND: Snakebite envenoming (SBE) is a major cause of morbidity in mortality in low-income and middle-income countries. In Brazil, SBE disproportionately affects Indigenous populations. Limited access to antivenom treatment is a key determinant of poor outcomes in these populations. We evaluated the effectiveness and feasibility of a decentralized model of antivenom delivery in Indigenous primary health settings in the Brazilian Amazon. METHODS: An implementation study was carried out in four Indigenous Health Poles across Amazonas state, Brazil. Effectiveness was assessed using a pre-po

Genetic analysis of IFNG-AS1 implicates opposite effects to Leishmania guyanensis-cutaneous leishmaniasis: rs4913269 confers protection while rs7134599 enhances susceptibility and correlates with high plasma IL-4 and IL-10 levels.

BACKGROUND: The long non-coding RNA interferon gamma antisense-1 (IFNGAS-1) is essential for Th1 lineage specific expression of IFNG. IFN-γ is a key component cytokine in host immune response against intracellular pathogens like Leishmania. We investigated the association of two genetic variants of IFNGAS-1, rs4913269 and rs7134599, with susceptibility or protection to Leishmania guyanensis- induced cutaneous leishmaniasis (Lg-CL). METHODS: A case-control study involving 1,714 individuals (855 Lg-CL and 859 healthy controls) was conducted in the state of Amazonas, Brazil. Genotyping of rs4913

Naturally acquired antibody response to a Plasmodium falciparum chimeric vaccine candidate GMZ2.6c and its components (MSP-3, GLURP, and Pfs48/45) in individuals living in Brazilian malaria-endemic areas.

BACKGROUND: The GMZ2.6c malaria vaccine candidate is a multi-stage Plasmodium falciparum chimeric protein which contains a fragment of the sexual-stage Pfs48/45-6C protein genetically fused to GMZ2, a fusion protein of GLURP and MSP-3, that has been shown to be well tolerated, safe and immunogenic in clinical trials performed in a malaria-endemic area of Africa. However, there is no data available on the antigenicity or immunogenicity of GMZ2.6c in humans. Considering that circulating parasites can be genetically distinct in different malaria-endemic areas and that host genetic factors can inf

Unraveling the cytokine profile in acute Chagas disease in the Brazilian Amazon: Insights before and after treatment.

Chagas disease (CD) is a neglected anthropozoonosis caused by Trypanosoma cruzi, with the oral route standing out as the primary cause for the increase in acute cases in the Brazilian Amazon and a key factor in the increase of acute cases. Despite this, the immune response in affected individuals remains minimally explored in humans. This longitudinal study aimed to characterize the serum cytokine profile in patients with acute CD before and after treatment. Participants were recruited from several municipalities in the state of Amazonas, and individuals with chronic inflammatory diseases were

Dynamics of anti-Strongyloides IgG antibody responses and implications for strongyloidiasis surveillance in rural Amazonians: A population-based panel data analysis.

BACKGROUND: Human strongyloidiasis was recently incorporated into the World Health Organization roadmap for neglected tropical diseases targeted for control in 2021-2030. However, the prevalence, incidence, and clinical burden of Strongyloides stercoralis infection remain understudied in remote communities across the Amazon due to its chronic nature, usually with absent or unspecific clinical manifestations, and the lack of practical and sensitive diagnostics for large-scale use. Here, we apply repeated antibody testing to estimate the prevalence of anti-Strongyloides IgG responses and identif

Tuberculosis preventive treatment completion in indigenous vs. non-indigenous populations in the Brazilian Amazon: a retrospective cohort study (2019-2023).

BACKGROUND: TB preventive treatment (TPT) is crucial for preventing the progression from TB infection (TBI) to active disease, particularly among vulnerable populations such as indigenous people. Although shorter regimens have improved adherence, evidence from real-world programmatic settings is limited. This study compared TPT completion rates between indigenous and non-indigenous populations in the Brazilian Amazon. METHODS: We conducted a retrospective cohort study using data from the IL-TB system (2019–2023) in Amazonas, Brazil. Individuals who initiated TPT were stratified by race. The pr

Genetic diversity of Bartonella spp. in vampire bats and associated Streblidae bat flies in the Brazilian Amazon.

Among mammals, bats stand out as important reservoirs for Bartonella spp., second only to rodents. In Brazil, out of the 182 species of bats described, three are hematophagous: Desmodus rotundus, Diphylla ecaudata and Diaemus youngii. Considering that Bartonella species have been increasingly associated to disease in humans, the search for such agents in animal reservoirs and ectoparasites is crucial for understanding the epidemiology of bartonelloses. The present study aimed to investigate the occurrence and genetic diversity of Bartonella spp. in vampire bats and Streblidae bat flies in the

Malaria in Brazil: an overview.

Malaria is still a major public health problem in Brazil, with approximately 306,000 registered cases in 2009, but it is estimated that in the early 1940s, around six million cases of malaria occurred each year. As a result of the fight against the disease, the number of malaria cases decreased over the years and the smallest numbers of cases to-date were recorded in the 1960s. From the mid-1960s onwards, Brazil underwent a rapid and disorganized settlement process in the Amazon and this migratory movement led to a progressive increase in the number of reported cases. Although the main mosquit

The burden of tuberculosis and attributable risk factors in Brazil, 1990-2017: results from the Global Burden of Disease Study 2017.

BACKGROUND: Tuberculosis (TB) continues to be an important cause of fatal and non-fatal burden in Brazil. In this study, we present estimates for TB burden in Brazil from 1990 to 2017 using data from the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 (GBD 2017). METHODS: This descriptive study used GBD 2017 findings to report years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life years (DALYs) of TB in Brazil by sex, age group, HIV status, and Brazilian states, from 1990 to 2017. We also present the TB burden attributable to independent r

Blackflies in the ointment: O. volvulus vector biting can be significantly reduced by the skin-application of mineral oil during human landing catches.

BACKGROUND: Standard human landing catches (sHLCs) have historically been a key component of Onchocerca volvulus transmission monitoring, but expose health-workers to potentially hazardous vector bites. Novel human-bait-free trapping methods have been developed, but do not always work where they are needed and may not generate O. volvulus surveillance data that is directly comparable with historic data. METHODOLOGY: Simuliid sHLCs and mineral-oil protected HLCs (mopHLCs) were performed in a rural village of Amazonas state, Brazil. A four-hour direct comparisons of sHLCs and mopHLCs was carrie

Impact of piperaquine resistance in Plasmodium falciparum on malaria treatment effectiveness in The Guianas: a descriptive epidemiological study.

BACKGROUND: Plasmodium falciparum is an apicomplexan parasite responsible for lethal cases of malaria. According to WHO recommendations, P falciparum cases are treated with artemisinin-based combination therapy including dihydroartemisinin-piperaquine. However, the emergence of resistant parasites against dihydroartemisinin-piperaquine was reported in southeast Asia in 2008 and, a few years later, suspected in South America. METHODS: To characterise resistance emergence, a treatment efficacy study was performed on the reported patients infected with P falciparum and treated with dihydroartemi
