BACKGROUND: Beta-lactamase production and porin decrease are the well-recognized mechanisms of acquired beta-lactam resistance in Klebsiella pneumoniae isolates. Pyoderma faciale is an inflammatory disease that until now was believed to occur only in women. Bacterial pathogens have not been found in most cases, and those found included only gram-positive microorganisms. We present 2 cases consistent with pyoderma faciale--one of them a 17-year-old adolescent boy--in which gram-negative bacteria were found. Enterobacter cloacae was found in one patient, and Klebsiella oxytoca was found in the other. Needle aspiration is the technique of choice in looking for pathogens in this disease.

Antimicrobial resistance among 1991 Pseudomonas aeruginosa isolates collected at 24 UK hospitals during late 1993 was surveyed. The occurrence of malignant otitis externa (MOE) in infancy is rare. We report a case of MOE in a neonate who was later identified to have selective IgA deficiency. She was successfully treated with oral ciprofloxacin, but developed external auditory canal stenosis, a deformed pinna, persistent facial nerve palsy, temporal bone erosion and hearing loss. No cases of MOE in selective IgA deficiency have been reported in literature. This is also the first report on the use of ciprofloxacin in infants with MOE.
