Porous polyethylene implants in orbital floor reconstruction.

The purpose of this article is to present the authors' experience with the use of porous polyethylene ultrathin sheets for orbital floor reconstruction. Thirty-two patients with orbital floor fractures were treated with porous polyethylene ultrathin sheets. Sixteen cases corresponded to orbitozygomatic fractures, 11 cases corresponded to pure orbital floor fractures, and five corresponded to panfacial fractures. The subciliary approach was used in 15 patients and the transconjunctival approach in nine; another three patients were operated on through a preexisting eyebrow wound, two were operat

Puerperal septic pelvic thrombophlebitis: incidence and response to heparin therapy.

OBJECTIVE: Before the availability of modern imaging studies the diagnosis of septic pelvic thrombophlebitis causing prolonged puerperal fever was difficult to confirm without surgical exploration. With the use of computed tomography infection-related pelvic phlebitis can now be confirmed, and this study was designed to determine its incidence after delivery. We also designed a randomized clinical trial to evaluate the efficacy of heparin added to antimicrobial therapy for treatment of women with septic phlebitis. STUDY DESIGN: We studied women who had pelvic infection and fever that persiste

[Sensitivity of Streptococcus pneumoniae to antimicrobial drugs].

INTRODUCTION: Streptococcus pneumoniae is the leading cause of community acquired pneumonia and a frequent cause of otitis media, sinusitis and meningitis. Although most pneumococci remain susceptible to penicillin, relatively less susceptible and resistant strains have been recognized with increasing frequency throughout the world (1). The aim of this study was to determine whether and to what degree pneumococci isolated in our laboratory were resistant to penicillin and other frequently used antimicrobial agents. MATERIAL AND METHODS: During the period from 1991 to 1995 1139 Streptococcus p

Multiple antimicrobial resistance indices of Staphylococcus aureus from the nares of goats and slaughterhouse attendants in Kampala city, Uganda- a cross sectional study.

The emergence of antimicrobial resistance (AMR) is a global menace to both public and animal health sectors with devastating effects in developing countries. Indiscriminate use of antibiotics in human health and livestock management contributes to development and rapid spread of AMR. Staphylococcus aureus is a major opportunistic zoonotic pathogen that colonises the skin and nostrils of human beings and animals and continues to develop antimicrobial resistance against different agents. The study aimed to determine multiple antibiotic resistance indices of S. aureus isolates from healthy domest

Surgical management of bisphosphonate induced osteonecrosis of the jaws.

Recent literature contains numerous articles on osteonecrosis of the jaws (ONJ) associated with bisphosphonate treatment (BPT), with most advocating a conservative approach to management. A prospective study was designed to review the surgical management of cases of ONJ that did not respond to conservative management. Forty patients, referred with ONJ that did not respond to conservative management, were treated surgically and followed up for 6 months to 4 years. Four patients were taking i.v. BPT as part of their bone cancer management and 16 were taking oral BPT for osteoporosis. The surgica

Emergency department medication dispensing reduces return visits and admissions.

OBJECTIVES: Return visits to the emergency department (ED) and subsequent readmissions are common for patients who are unable to fill their prescriptions. We sought to determine if dispensing medications to patients in an ED was a cost-effective way to decrease return ED visits and hospital admissions for skin and soft tissue infections (SSTIs). METHODS: A retrospective review of ED visits for SSTIs, during the 24 weeks before and after the implementation of a medication dispensing program, was conducted. Charts were analyzed for both ED return visits and hospital admissions within 7 days and

Povidone-Iodine Antimicrobial Activity In Vitro Against Periodontal Bacterial Pathogens.

Background Povidone-iodine (PV-I) is known to be active in vitro against periodontal bacterial pathogens, but previous studies most often used ≥5-minute contact times for PV-I testing and/or evaluated laboratory reference strains of bacterial species. This study further examined the antimicrobial effects of PV-I by using a 60-second in vitro treatment time for 10% and 5% PV-I on freshly recovered clinical isolates of subgingival biofilm bacteria from severe human periodontitis lesions. Methods Subgingival biofilm samples from 22 adults with severe periodontitis were mixed in vitro with 10% PV-

Detection profiles of multiplex real-time PCR and culture for Streptococcus pneumoniae in pediatric lower respiratory tract infections.

OBJECTIVE: Bacterial pathogens are common causes of lower respiratory tract infections (LRTIs) in children, particularly Streptococcus pneumoniae (S. pneumoniae). Multiplex real-time quantitative PCR (qPCR) and traditional bacterial culture are two routine clinical assays for the detection of S. pneumoniae. Understanding their differing detection profiles supports appropriate interpretation and application of test results. This study aimed to analyze the positivity rates, concordance, and clinical correlates of paired qPCR and culture for S. pneumoniae in pediatric LRTIs. METHODS: This single

Isolation of Staphylococcus aureus from environmental surfaces in an academic dental clinic.

BACKGROUND: Staphylococcus aureus is an important health care-associated pathogen that often is resistant to antibiotics. The authors conducted a pilot study to determine if abiotic surfaces in a dental clinic were contaminated frequently. METHODS: The authors sampled surfaces with swabs that they then used to inoculate selective and differential media. CHRO-Magar Staph aureus (DRG International, Mountainside, N.J.) was the most effective. They used phenotypic and genotypic tests to identify presumptive S. aureus colonies. They determined the sensitivity of S. aureus isolates to five antibiot

PEN-FAST-ED: Utilizing the PEN-FAST decision tool to guide antibiotic prescribing in the emergency department.

BACKGROUND: Patient-reported penicillin allergies are frequently encountered in the emergency department (ED), which often lead to non-beta-lactam antibiotic use despite beta-lactams' place as first-line therapy in most bacterial infections. The PEN-FAST clinical decision tool was developed and validated to identify patients with a low risk of true penicillin allergies that do not require formal skin testing for rechallenging. The tool consists of four questions that add up to a total score ranging from 0 to 5. Scores of 0-2 are associated with low risk of true penicillin allergy (<5% risk of
