On the role of Ca++ binding proteins as possible targets for Ca++ sensitizing agents.

We describe the effects of various cardioactive compounds on the Ca++ activation of force production and ATPase activity in isolated contractile structures from mammalian heart and, in some cases, skeletal muscle. We show that: 1) the Ca++ sensitizing activity of APP 201-533 does not discriminate between cardiac and skeletal muscle and is, therefore, not based on interaction with cardiac troponin I phosphorylation at serine 20. 2) compounds like trifluoperazine or bepridil, both known to interact with calmodulin, increase the Ca++ sensitivity of the contractile structures of the heart, in high

Differential effects of specific phosphodiesterase isoenzyme inhibitors on bovine oocyte meiotic maturation.

The differential regulation of cAMP levels within the oocyte and somatic (cumulus) cell compartments of the bovine follicle, and the subsequent regulation of oocyte meiotic maturation was examined through specific cell-type localisation of phosphodiesterases (PDEs). Selective PDE inhibitors were used to modulate cAMP levels in each of the two follicular compartments and to examine their effects on oocyte meiotic maturation. Ovaries were obtained from an abattoir and cumulus-oocyte complexes (COC) were aspirated from antral follicles into culture medium supplemented with 4 mg/ml BSA and 2mM 3-i

Effects of N(G)-monomethyl-L-arginine on Ca(2+) current and nitric-oxide synthase in rat ventricular myocytes.

The effects of N(G)-monomethyl-L-arginine (L-NMMA), a nitric-oxide synthase (NOS) inhibitor, on the L-type Ca(2+) current (ICa) and NO effects on NOS were determined in rat ventricular myocytes. L-NMMA (10 and 100 microM) had no significant effect on basal ICa, but in a cAMP-stimulated condition due to forskolin (1 microM) or milrinone (10 microM), a cGMP-inhibited cAMP-phosphodiesterase (PDE), L-NMMA (10 and 100 microM) concentration dependently augmented ICa. The enhancing effects of L-NMMA (10 and 100 microM) on ICa were not seen in the presence of either a nonselective inhibitor of PDE, 3-

[Therapeutic options in cardiogenic shock].

Cardiogenic shock continues to be a clinical situation which is related with high mortality. Although its etiology is varied, the most frequent cause is an acute myocardial infarction. The poor prognosis of cardiogenic shock can be favourably modified with the diagnosis of the underlying cause followed by the stabilization of the patient and early revascularization. Early treatment with inotropic or vasopressor drugs improves the condition of most patients and the use of circulatory assistance, such as the intraaortic balloon, lead to an acceptable hemodynamic situation in 80% of cases. The Use of Cardiotonic Drugs in Neonates.

This overview provides a summary of the commonly used vasoactive agents and the various clinical scenarios in which they are used in the neonatal intensive care unit (NICU). The lack of well-designed clinical trials in this vulnerable population means that these agents continue to be administered without sufficient evidence-based knowledge on their dosing, safety, efficacy and long-term effects. The lack of age appropriate cardiotonic formulations remains an ongoing challenge. Future trials of cardiotonic drugs need to incorporate echocardiography and other objective monitoring tools in their

Standard concentrations of intravenous continuous drug infusions: evaluation of adherence, safety and effectiveness in paediatric critical care as quality improvement initiative.

OBJECTIVE: To evaluate the introduction of locally configured standard concentration (StdC) intravenous drug infusions in critically ill children. METHODS: This two-year quality improvement retrospective cohort study (2018-20) examined post-implementation of 47 StdC drugs configured across three weight bands: <5 kg, 5-20 kg and >20 kg in a 26-bed, multispecialty paediatric intensive care unit. The main outcome measures were (1) adherence to StdC use (non-adherence defined as using a bespoke drug concentration), (2) attempts at dosing above the pre-set infusion rates, known as hard limit event

Conversion From Venovenous to Venoarterial or Hybrid Extracorporeal Membrane Oxygenation: Analysis From the Extracorporeal Life Support Organization Registry.

OBJECTIVES: Venovenous extracorporeal membrane oxygenation (ECMO) represents a standard and well-accepted modality of treating patients with refractory respiratory failure. Nevertheless, some patients might develop refractory hypoxemia, hemodynamic compromise or end-organ perfusion requiring a change. This study analyzed characteristics and outcomes of patients requiring a change from venovenous to a different ECMO configuration. DESIGN: Multicenter, retrospective, observational analysis of the Extracorporeal Life Support Organization Registry (2010-2020) in adult patients (≥ 18 yr old) under

Procedure-related vasospasm and reperfusion vulnerability during mechanical thrombectomy.

Mechanical thrombectomy (MT) has reshaped the treatment of acute ischemic stroke caused by large-vessel occlusion (LVO), yet successful angiographic reperfusion does not reliably translate into meaningful neurological recovery. Landmark randomized trials and the Highly Effective Reperfusion Evaluated in Multiple Endovascular Stroke Trials (HERMES) patient-level meta-analysis established MT as standard of care, but they also brought into focus the persistent problem of incomplete tissue-level rescue despite technically successful recanalization. Procedure-related vasospasm is usually handled as

The anaesthesiologist's role in heart transplant: An Armed Forces Medical Services perspective.

BACKGROUND: Heart transplantation remains the definitive therapy for refractory endstage heart failure. While surgical and immunosuppressive advances have been pivotal, the anaesthesiologist's role is underrepresented-particularly within Armed Forces Medical Services (AFMS), with logistical and geographic constraints. This study examines perioperative anaesthetic strategies in AFMS heart transplants and their impact on patient and graft outcomes. METHODS: This retrospective case series included 33 consecutive heart transplants performed across AFMS centres between 2018 and 2024. Data were ana

Prognostic value of the vasoactive-inotropic score in predicting outcomes following LVAD implantation.

BACKGROUND: Vasoplegia frequently complicates left ventricular assist device (LVAD) implantation, yet its impact on outcomes is not fully understood. The vasoactive-inotropic score (VIS), which quantifies vasoactive support, may predict outcomes in this population. METHODS: A retrospective analysis of 146 patients undergoing HeartMate 3 LVAD implantation from 2017 to 2024 at a single institution was performed. VIS was calculated at 0, 6, 12, and 24 hours postoperatively, and the maximum VIS(VISmax) within the first 24 hours was determined: VIS = dopamine (mcg/kg/min) + dobutamine (mcg/kg/min)

Effect of cerebral oxygen saturation on outcomes of patients with ruptured intracranial aneurysms undergoing endovascular embolization: A prospective observational study.

BACKGROUND: Subarachnoid hemorrhage secondary to ruptured intracranial aneurysms compromise cerebral perfusion and oxygenation, threatening optimal cerebral function. This study explored the role of regional cerebral oxygenation monitoring in predicting outcomes in these patients. METHODOLOGY: Fifty-two patients aged 18-65 years and scheduled for coiling of ruptured anterior circulation aneurysms participated in this prospective observational study. Bilateral regional cerebral oxygen saturation (rScO2) was recorded during critically important phases of endovascular embolization under general

Two case reports of idiopathic prenatal closure of the ductus arteriosus complicated at birth by severe pulmonary hypertension successfully treated without intubation.

BACKGROUND: Prenatal closure of the ductus arteriosus (DA) is an uncommon but potentially severe condition that can result in persistent pulmonary hypertension of the newborn (PPHN), right ventricular (RV) dysfunction, and even fetal demise. While often associated with maternal exposure to non-steroidal anti-inflammatory drugs (NSAIDs), idiopathic cases are also reported. The pathophysiology involves disruption of prostaglandin-mediated DA patency, leading to increased pulmonary vascular resistance (PVR) and RV afterload, with subsequent postnatal hemodynamic compromise. Given the rarity of th

Vascular reactivity is altered in the placentas of fetuses with congenital diaphragmatic hernia.

INTRODUCTION: Infants with congenital diaphragmatic hernia (CDH) often develop pulmonary hypertension but frequently fail to respond to vasodilator therapy, for instance because of an altered pulmonary vasoreactivity. Investigating such alterations in vivo is impossible. We hypothesised that these alterations are also present in fetoplacental vessels, since both vasculatures are exposed to the same circulating factors (e.g. endothelin-1) and respond similarly to certain stimuli (e.g. hypoxia). As proof-of-concept, we compared fetoplacental vasoreactivity between healthy and CDH-affected placen

Aortic Pulsatility Index: A Novel Hemodynamic Variable for Evaluation of Decompensated Heart Failure.

BACKGROUND: Right heart catheterization for invasive hemodynamics has shown only modest correlation with clinical outcomes. We designed a novel hemodynamic variable that incorporates ventricular output and filling pressure. We anticipated that the aortic pulsatility index (API) would correlate with clinical outcomes in patients with heart failure. METHODS AND RESULTS: We retrospectively analyzed consecutive patients undergoing right heart catheterization with milrinone drug study at our institution (February 2013 to November 2019). The API was calculated as (systolic blood pressure - diastoli
