Scarf osteotomy for hallux valgus deformity: Radiological outcome, metatarsal length and early complications in 118 feet.

BACKGROUND: Scarf osteotomy has been widely used to restore axial orientation of the first ray in the treatment of hallux valgus deformity. The aim of the study was to present our radiological outcomes of bunion reconstruction, identify surgical complications in early follow-up, and assess to what extent a shortening of the first metatarsal is present after surgery as a possible cause of postoperative metatarsalgia. METHODS: We enrolled 106 patients (118 feet) and assessed patients' pre- and postoperative measurements of hallux valgus and intermetatarsal angles on weightbearing X-ray images.

A new "tension side" locking plate for Hallux Valgus: A prospective multicentre case series.

BACKGROUND: Proximal osteotomy of the first metatarsal is often indicated for Hallux Valgus correction. Previously recognised complications however, include transfer metatarsalgia, first metatarsophalangeal joint stiffness, problems with fixation and prominence of metalware. METHODS: We report on one year follow up of an international prospective series between June 2009 and October 2012 involving three centres, including 91 feet (58 patients) that underwent proximal osteotomy, using a new locking plate applied to the plantar surface of the metatarsal. RESULTS: Mean Hallux Valgus angle impro

Effectiveness of neuromuscular stretching with symmetrical biphasic electric currents in the cavus foot.

BACKGROUND: Pes cavus is a structural deformity in which the increased plantar arch can lead to greater metatarsal verticality with the consequent excess of pressure under the forefoot zone (especially the metatarsal zone), causing pain and significant loss of functional capacity. We sought to determine whether neuromuscular stretching with symmetrical rectangular biphasic currents can reduce the pressure supported by this zone. METHODS: This prospective, nonrandomized, longitudinal, analytical, and experimental controlled trial included 34 patients with pes cavus. Pedobarometric measurements

[Baropodometric walking analysis in healthy elderly and in arthritic patients before and after knee prosthesis implantation].

Standing and walking are complex activities which require integral skeletal-muscular and central nervous systems. Body is usually, evenly distributed between both the lower limbs with 40% on the ball of the foot and 60% on the heel, therefore, the body's centre of gravity falls between the feet in correspondence to Chopart's articulation. Some diseases can influence standing and walking including cardiovascular diseases (chronic edema, claudication, cardiopathies etc.) neurosensorial ones (cataract, Menière's disease, Parkinson's disease etc.) and orthopedic ones (kyphosis, scoliosis, hallux v

Silastic ball spacer arthroplasty in the management of hallux valgus and hallux rigidus.

Sixty-two feet in 39 patients who were treated by insertion of a silastic ball spacer prosthesis into the first metatarsophalangeal joint for hallux valgus or hallux rigidus have been reviewed with a follow-up time of between 2 and 6 years. Twenty-one results (34%) were excellent, 27 (43%) were fair, 13 (21%) were poor, and one was revised. Seventeen feet (27%) had some metatarsalgia at followup. Results were disappointing in the young patients; in 19 cases of hallux valgus under the age of 45, 8 were either poor or had been revised. The original concept of the silastic ball spacer was to main

Morton's neuroma: who, when and how contributed to its description and treatment?

PURPOSE: The interdigital nerve neuroma of the forefoot is commonly known as Morton's Neuroma. Many authors have described and treated this condition before and after Morton. This study aims to investigate the past scientific literature to better understand what comprehension and treatments have been used to master this pathology. METHODS: Historical and modern scientific accounts were searched for descriptions of interdigital nerve neuroma or metatarsalgia (as some authors described it) to have a thorough overview of the subject. The scientific literature was searched to highlight the evolut

A Rare Morphological Variant of Postaxial Foot Polydactyly With an Accessory Articulating Osseous Structure of the Fifth Ray.

Postaxial polydactyly of the foot is a common form of digital duplication that often causes difficulty with footwear, metatarsalgia, and cosmetic concerns. Although it may present as an isolated anomaly, it can also be associated with genetic conditions. Surgical excision is the primary intervention, with generally excellent outcomes. A 5-year-old male presented with postaxial polydactyly of the left foot, characterized by an accessory articulating osseous structure associated with the fifth ray. The main complaints were difficulty with footwear and cosmetic appearance. Radiographic evaluation

Ultrasound-guided treatment of Morton's neuroma.

Morton's neuroma is a painful lesion of the interdigital nerve, usually at the third intermetatarsal space, associated with fibrotic changes in the nerve, microvascular degeneration, and deregulation of sympathetic innervation. Patients usually present with burning or sharp metatarsalgia at the dorsal or plantar aspect of the foot. The management of Morton's neuroma starts with conservative measures, usually with limited efficacy, including orthotics and anti-inflammatory medication. When conservative treatment fails, a series of minimally invasive ultrasound-guided procedures can be employed

[Case report: comprehensive treatment of forefoot with double brachymetatarsia in one surgical stage].

Brachymetatarsia (shortening of the metatarsal) directly affects the transversal arch of the foot causing severe deformities, such as hallux valgus, divergent toes, overlapping toes, metatarsalgia, all of these alter the biomechanics of the foot and gait. Treatment consists of two main techniques, one-stage lengthening with bone graft or elongation through callotaxis with external fixator; there are variants of both techniques used to minimize the disadvantages of each. The objective of treatment must be to return the structural harmony to the forefoot, improving the biomechanics, resulting in

Case report: a modified 1-stage technique for the treatment of brachymetatarsia.

UNLABELLED: Brachymetatarsia is an abnormal shortening of a metatarsal. It causes disruption of the normal metatarsal parabola, resulting in metatarsalgia, calluses and dislocation of the digits. Patients often express discontent with the appearance of their feet. Lengthening by distraction osteogenesis and 1-stage intercalary bone grafting procedures are the mainstays of operative treatment. We present a modified 1-stage technique, combining the use of a Chevron osteotomy of the fourth metatarsal with shortening osteotomies of the second and third metatarsals. Bone removed from the third meta

Morton's intermetatarsal neuroma: morphology and histological substrate.

Twenty-three biopsies from patients with the typical symptoms of intermetatarsal neuroma (so-called Morton's metatarsalgia) were compared histologically and semi-quantitatively with 25 plantar nerves from the intermetatarsal space III/IV gained at autopsies from cases where no problems in the forefoot had been recorded. The histomorphological examination of the nerves from autopsies revealed the same findings as were found in the biopsies. Thus, qualitatively, the nerves from patients could not be distinguished from those gained at autopsy. The only difference was the diameter of the resected

Treatment of brachymetatarsia by the callus distraction method.

Brachymetatarsia is not an uncommon congenital foot disorder that is due to a premature closure of the ephiphyseal plate. The literature has numerous surgical procedures for the correction of this problem. In recent years, the callus distraction technique described by ilizarov has been utilized for this problem. This is a retrospective study of the use of the callus distraction technique for the correction of congenitally short metatarsal. This series included six feet in six female patients, all with a short fourth metatarsal. Preoperative complaints were metatarsalgia and/or cosmesis. A nonc

[Congenital insufficiency of the distal support function of the first ray of the foot].

Five patients with congenital deficiency of antero-medial support have been examined and treated at the Hôpital cantonal, Geneva. The main symptoms of these patients were global metatarsalgia and a dorsal bunion. All lesions were bilateral. Four of the patients had a localised dyslatia of the first ray, whereas one of them suffered from a nail patella syndrome. In two patients there was a hypoplastic first metatarsal, whereas in three others the first metatarsal was normal but too elevated under weight bearing. One patient responded well to conservative treatment. In three cases a lowering ost

A clinical evaluation of the total first metatarsophalangeal joint prosthesis: the use of footprints in assessing foot contact.

The use of footprints in evaluating surgical results is most significant to the physician. This clinical investigation consisted of patients whose surgical correction of severe hallux valgus included the use of the Swanson-total great toe hinged prosthesis. The resulting failure of weightbearing by the great toe pulp postoperatively were found to be associated with excessive weightbearing by the lesser metatarsal heads, evidenced by discrete keratomas, a relative lesser metatarsalgia, and footprinting analysis. In conclusion, we believe that there is a definite relationship between the lack of

Augmented reality guided osteotomy in hallux Valgus correction.

BACKGROUND: An optimal osteotomy angle avoids shortening of the first metatarsal bone after hallux valgus surgery and therefore reduces the risk of transfer-metatarsalgia. The purpose of the present ex-vivo study was to investigate whether augmented reality (AR) would improve accuracy of the distal osteotomy during hallux valgus surgery. METHODS: Distal osteotomies of the first metatarsals were performed on a foot model by two surgeons with different levels of surgical experience each with (AR, n = 15 × 2) or without (controls, n = 15 × 2) overlay of a hologram depicting an angle of osteotomy

Stress at the second metatarsal bone after correction of hammertoe and claw toe deformity: a finite element analysis using an anatomical model.

BACKGROUND: We used finite element analysis to evaluate three techniques for the correction of hammertoe and claw toe deformities: flexor digitorum longus tendon transfer (FDLT), flexor digitorum brevis tendon transfer (FDBT), and proximal interphalangeal joint arthrodesis (PIPJA). METHODS: We performed a finite element analysis of FDLT and FDBT compared with PIPJA of the second toe using multislice computed tomography and 93 tomographic images of the foot obtained in a healthy 36-year-old man. RESULTS: The PIPJA showed a significantly higher increase in traction and compressive stresses and

Flexor hallucis longus tendon transfer: evaluation of postoperative morbidity.

Clinical and pedobarograph evaluation was performed on 16 patients following flexor hallucis longus (FHL) tendon transfers to determine the resulting morbidity due to the loss of FHL function. All patients underwent FHL tendon transfer for either chronic tendon Achilles rupture or chronic Achilles tendinosis. Clinical evaluation of hallux function was performed using the American Orthopaedic Foot and Ankle Society (AOFAS) hallux metatarsophalangeal-interphalangeal scale, the SF-36 score, and a clinical questionnaire to assess alteration in the clinical function of the hallux during activities
