Transcanal labyrinthectomy for intractable vertigo.

Labyrinthectomy may be the therapy of choice for intractable and disabling vertigo that is caused by unilateral disease of the vestibular labyrinth. The transcanal method through the oval window offers the most direct surgical approach for complete ablation of the vestibular sense organs. The technique includes visual identification and removal of the utricle; this is followed by blind probing of the ampullae. This method was used in 124 patients, including 90 patients with unilateral Meniere's disease. Three of four cases of moderately severe continuing postoperative vertigo were successfully

Delayed endolymphatic hydrops following acoustic tumor removal with intraoperative and postoperative auditory brainstem response improvements.

A 36-year-old female with delayed endolymphatic hydrops (ELH) after acoustic tumor removal of the same ear is reported. She presented at our clinic with pressure, fullness, and tinnitus involving her right ear. She had an abnormal auditory brainstem response (ABR), and a computed tomography showed a right acoustic tumor. She underwent a complete tumor removal from a suboccipital incision with significant intraoperative and postoperative ABR improvements. Preoperatively, there was no fluctuation in her hearing and her electrocochleography (ECoG) was normal. She did well postoperatively, but 18

Surgical management of Meniere's disease with endolymphatic sac decompression by wide bony decompression of the posterior fossa dura: technique and results.

The results of wide bony decompression of the posterior fossa in 49 consecutive individuals with idiopathic unilateral Meniere's disease are reported. These results are similar to many previously published series, 71% of individuals are free of vertigo and 86% have unchanged or improved hearing. Utilizing the classification proposed by the American Academy of Ophthalmology and Otolaryngology, 73% fell into Class A, B, or C with relief of vertigo. These results together with the low complication rate encourages us to consider surgery at an earlier date, hoping to stabilize hearing at a more use

High-salt intake exacerbates endolymphatic hydrops and alters aldosterone regulation in a Ménière's disease animal model.

OBJECTIVE: Ménière's disease (MD) causes episodic vertigo and fluctuating hearing loss and is pathologically characterized by endolymphatic hydrops (EHs), often resulting from impaired endolymph absorption in the endolymphatic sac (ES). Although salt restriction is widely recommended, its mechanistic basis remains unclear. This study aims to examine how ES dysfunction and dietary salt intake interactively affect endolymphatic homeostasis, vestibular function, and aldosterone regulation. METHODS: Guinea pigs underwent ES ablation or sham surgery and were fed either a normal- (0.41 % NaCl) or h

Optical coherence tomography imaging demonstrates endolymphatic hydrops in the lateral and posterior semicircular canals in noise-exposed mice.

Optical Coherence Tomography (OCT) has been used to characterize cochlear endolymphatic hydrops (ELH) with distention of Reissner's membrane in mice after noise exposure. Noise exposure has been correlated with vestibular dysfunction, so we hypothesized that noise exposure can lead to ELH in the vestibular system. Little work has been performed using OCT to image the membranous labyrinth in the lateral and posterior semicircular canals (SCCs). We show that OCT with 12.5 µm axial resolution and 13.2 µm lateral resolution can image the SCCs and delineate the membranous labyrinth in anesthetized

Skull Pneumatization Forms a Biothermal System Protecting Ocular and Vestibular Homeostasis.

Background: Paranasal sinuses and mastoid air cells have been attributed to multiple functions-such as voice resonance, cranial lightening, and pressure regulation-yet their potential role in local thermal homeostasis remains underappreciated. The thermoregulatory hypothesis, first proposed in the mid-twentieth century, was largely abandoned after the mid-century, when anthropological findings of climate-correlated variation seemed contradictory. Hypothesis: We propose that pneumatized skull regions form a three-component craniofacial biothermal system that maintains thermal stability in the o

Perilymphatic ATP plays a critical role in modulating cochlear function to protect from loud sound induced hearing loss.

BACKGROUND: Extracellular adenosine triphosphate (ATP) signalling via purinergic receptors plays a key role in cochlear adaptation to loud sound. Traditionally, ATP activation of purinergic receptor P2X receptors has been proposed to induce a cation shunt, reducing the endolymphatic potential and the driving force for sound transduction. However, direct evidence for this protective mechanism remains limited. Here, we provide direct experimental evidence identifying a distinct, compartment-specific ATP signalling pathway that modulates cochlear function. METHODS: In mature Dunkin-Hartley guine

KCCs, NKCCs, and NCC: Potential targets for cardiovascular therapeutics? A comprehensive review of cell and region specific expression and function.

Cardiovascular diseases, the leading life-threatening conditions, involve cardiac arrhythmia, coronary artery disease, myocardial infarction, heart failure, cardiomyopathy, and heart valve disease that are associated with the altered functioning of cation-chloride cotransporters. The decreased number of cation-chloride cotransporters leads to reduced reactivity to adrenergic stimulation. The KCC family is crucial for numerous physiological processes including cell proliferation and invasion, regulation of membrane trafficking, maintaining ionic and osmotic homeostasis, erythrocyte swelling, de

High Resolution Three-Dimensional Delayed Contrast MRI Detects Endolymphatic Hydrops in Patients With Vertigo and Vestibular Schwannoma.

OBJECTIVE: Advances in high resolution magnetic resonance imaging (MRI) have enabled the detection of endolymphatic hydrops (EH), a pathological ballooning of the endolymphatic fluid system, known to be associated with Menière's disease. When a patient has a known diagnosis of vestibular schwannoma and develops recurrent episodic vertigo spells, many surgeons recommend surgical intervention, attributing the vestibular symptoms to the vestibular schwannoma. The aim of this study is to evaluate the clinical outcome in patients with vestibular schwannoma and EH, treated medically, for recurrent s

The relationship between benign paroxysmal positional vertigo and thyroid autoimmunity.

CONCLUSION: Although there have been few studies concerning BPPV and thyroid autoimmunity and a positive relation was found between them, this study didn't find any relation between BPPV and thyroid autoimmunity. IT is thought that further large-scale studies must be done to clarify the relation. OBJECTIVES: Benign paroxysmal positional vertigo (BPPV) consists of ∼ 20% of vestibular disorders. Self-limited rotatory nystagmus with positional vertigo are the main findings of BPPV. Although canalolithiasis theory was confirmed by demonstrating freely floating debris in the endolymph of the poste

Hearing preservation in vestibular schwannoma stereotactic radiosurgery: what really matters?

Stereotactic radiosurgery (SRS) for vestibular schwannomas has evolved and improved over time. Although early short-term follow-up reports suggest that fractionation yields hearing preservation rates equivalent to modern single-dose SRS techniques, significant questions remain regarding long-term tumor control after the use of fractionation in a late responding tumor with a low proliferative index and alpha/beta ratio. With single-dose SRS, critical hearing preservation variables include: 1) strict attention to prescription dose 3D conformality so that the ventral cochlear nucleus (VCN) receiv

Glycerol effect on the guinea pig tectorial membrane.

The tectorial membrane (Tm) of guinea pigs has been found to have an altered organization of its matrix fibers in response to intravenously administered glycerol. Following treatment, the Tm middle zone shows an increase in waviness and clumping of fibers in non-hydropic and several hydropic ears in contrast to non-treated control ears. Residue of the internal sulcus cells occasionally fills the subtectorial space. In the present study, additional investigations were performed with scanning electron microscopy in order to study the relationship between the Tm and the organ of Corti, as well as

Experimental pathogenesis of hydrops.

1. The best method for inducing hydrops in guinea pigs is obliteration of the endolymphatic duct. 2. The location of hydrops produced in the guinea pig is comparable to that of Menière's disease. 3. The major histopathological finding in the guinea pig is atrophy of sensorineural elements in the apical turns of the cochlea. 4. The vestibular sensory cell population is rarely decreased, though ultrastructural changes are noted. 5. Attempts to prevent or minimize development of hydrops by surgical fistulization of the vestibular endolymphatic walls or by administration of a diuretic drug, ethacr

Assessment of a Statistical Algorithm for the Prediction of Benign Paroxysmal Positional Vertigo.

IMPORTANCE: Benign paroxysmal positional vertigo (BPPV) is an otologic pathologic condition defined as a sensation of spinning triggered by changes in head position relative to gravity and caused by an entrapment of fragmented endolymph debris most commonly in the posterior semicircular canal. Confirmation of diagnosis requires experience with procedures that are poorly known by those other than practitioners with advanced otologic training. The complexity in the diagnosis of BPPV inspired the design of a questionnaire-based algorithm that would be useful for determining a vestibular diagnosis

Biochemical markers of bone turnover in benign paroxysmal positional vertigo.

OBJECTIVE: Several studies have suggested a possible relationship between recurrent benign paroxysmal positional vertigo (BPPV) and altered calcium homeostasis in the endolymph of the inner ear. The present study aimed to evaluate the association between Ca2+ and vitamin D status and BPPV occurrence as well as the status of bone biochemical markers in osteoporotic patients who were diagnosed with idiopathic BPPV. METHODS: The study included total 132 patients who were referred to our clinic between August 2008 and October 2013. Based on the bone mineral density (BMD) results, the subjects wer
