Pediatric Wrist.

Pediatric wrist injuries pose unique diagnostic challenges due to distinct bone characteristics in children and their diverse injury patterns. The dynamic development of the wrist, marked by changes in bone age and emerging ossification centers, is crucial to evaluate growth and identify potential pathologies. The skeletal composition, rich in cartilage, renders bones relatively weaker yet more elastic, impacting their susceptibility to fracture. Forearm fractures display diverse patterns influenced by torsional forces. Scaphoid fractures, less common in children, differ from those in adults.

Arthroscopic findings in gout of the wrist.

PURPOSE: We have recently treated several patients with gout of the wrist and scapholunate ligament pathology. The purpose of this investigation was to report the arthroscopic findings of 7 such patients. METHODS: We searched our medical records for patients with a diagnosis of gout and nontraumatic wrist pathology. We retrospectively reviewed the clinical, arthroscopic, radiographic, and pathologic findings in 7 patients who failed nonsurgical management and were treated with wrist arthroscopy. RESULTS: All 7 wrists had diffuse synovitis and crystalline deposits throughout the radiocarpal j

Elbow, forearm and wrist injuries in the athlete.

Competitive and recreational athletes sustain a wide variety of soft tissue, bone, ligament, tendon and nerve damage to their upper extremities. Most such injuries are related to direct trauma or repetitive stress, and account for a significant amount of 'down time' for athletes participating in a wide range of sports, particularly those in which the arm is utilised for throwing, catching or swinging. Overuse injuries to the elbow include musculotendinous injuries, ulnar nerve injuries and ligamentous injuries. Osteochondrol lesions of the capitellum and posterior impingement injuries in the j

The occult dorsal carpal ganglion.

Chronic wrist pain has many causes, the diagnosis of which is often difficult. Clinical and anatomical research in this area has replaced the diagnosis of "wrist sprain" with a differential diagnosis including carpal chondromalacia, dynamic carpal instability, positive and negative ulnar variance, triangular fibrocartilage complex injuries, and early carpal avascular necrosis. The ubiquitous dorsal ganglion can also cause chronic wrist discomfort and the diagnosis of "occult dorsal carpal ganglion" should be included in the differential diagnosis. Nine patients with chronic wrist pain were dia

Arthroscopic Oblique Partial Ulnar Styloid Resection for Ulnar Styloid Impaction Syndrome.

Background  Open or arthroscopic partial resection of the elongated ulnar styloid is the surgical treatment of choice for ulnar styloid impaction syndrome. Case Description  A patient with a severely elongated processus styloideus ulnae (PSU) with a chronic impaction of the distal-radial margin against the triquetrum suffered a traumatic triangular fibrocartilage complex (TFCC) Palmer 1B rupture and DRUJ instability. The length of the PSU was 9 mm. After failed conservative treatment, a partial oblique arthroscopic resection of the PSU and simultaneous TFCC reinsertion were performed with unev

Central band reconstruction for the treatment of Essex-Lopresti injury : A novel technique using the brachioradialis tendon.

Longitudinal stability of the forearm is mainly provided by three structures: the radiocapitellar contact, which acts as the primary stabilizer, the central band of the interosseous ligamentous complex (IOLC) and the intact triangular fibrocartilage complex (TFCC). In an Essex-Lopresti lesion the forearm becomes fully destabilized, since all of these three components are injured. Fixation or replacement of the radial head with a metallic prosthesis along with repair of the TFCC and stabilization of the distal radioulnar joint (DRUJ) are well-established treatment goals. However the reconstruct

[Anatomy and biomechanics of the distal radioulnar joint].

BACKGROUND: The functionality of the upper extremity is decisively based on rotation of the forearm. The rotation depends on the extent of motion of the distal radioulnar joint. Rotation enables complete and focused usability of the hand in order to cope with daily activities. The configuration of the distal radioulnar joint has developed over millions of years of evolution. ANATOMIC CONDITIONS: The triangular fibrocartilage complex is the crucial stabiliser of the latter joint since osseous structures are limited. The palmar and dorsal radioulnar ligaments belong to this complex. The superfi

Multidetector CT arthrography of the wrist joint: how to do it.

With its exquisite spatial resolution, multidetector computed tomographic (CT) arthrography of the wrist is a valuable tool for the diagnosis and evaluation of a wide spectrum of articular disorders. Traumatic tears of the interosseous ligaments can be classified as complete or incomplete and as partial- or full-thickness tears at multidetector CT arthrography and can also be differentiated from asymptomatic degenerative lesions. In addition, tears of the triangular fibrocartilage complex can be differentiated according to their location. A tailored contrast material injection technique and mu

An anatomical study of the ligaments of the ulnar compartment of the wrist.

The ligamentous structures of the triangular fibrocartilage complex (TFCC) and their attachments were examined anatomically and histologically using fresh and embalmed cadavers. The TFCC was observed to have a three-dimensional structure consisting of three palmar ligaments--the short radiolunate (SRL), ulnolunate (UL), and ulnotriquetral (UT) ligaments. In addition, the attachment site of the ulnocarpal ligament (UC), which had been previously unknown, was identified. The dorsal components of the TFCC have been previously reported to consist solely of the extensor carpi ulnaris (ECU) subsheat

Trauma of the wrist.

In wrist injury, plain radiographs form the basis of diagnostic imaging, and can provide definitive answers in most cases. MR imaging is the method of choice for several diagnostic problem cases. These problems can include radiographically occult fractures where MRI enables early diagnosis compared to follow-up radiographs. Early stage diagnosis of a post-traumatic avascular osteonecrosis with high sensitivity and specificity, is only possible with MRI. In these instances, radiographs are only sensitive in later stages and scintigraphy is quite nonspecific. Stress fractures, invisible with oth

Computerized tomography of the distal radioulnar joint: correlation with ligamentous pathology in a cadaveric model.

The kinematics of the normal distal radioulnar joint (DRUJ) and the stabilizing function of various structures about the DRUJ were investigated in a study involving six fresh frozen cadavers. Sequential division of the supporting structures was correlated with abnormalities detected by computerized tomography (CT). The infratendinous portion of the extensor carpi ulnaris is a major restraint against dorsal and palmar subluxation. Division of the radioulnar ligaments and triangular fibrocartilage alone produced only minor changes. Lateral displacement was controlled by the interosseous membrane

Arthroscopic physeal-sparing TFCC foveal repair via a single extracapsular transverse bone tunnel in adolescents: a retrospective cohort study.

BACKGROUND: Conventional arthroscopic techniques for triangular fibrocartilage complex (TFCC) foveal repair risk iatrogenic physeal injury in adolescents due to longitudinal oblique bone tunnels traversing the distal ulnar physis. This study evaluates a novel physeal-sparing technique utilizing a single extracapsular transverse bone tunnel to avoid growth plate compromise. METHODS: A retrospective cohort study included 13 adolescents (10 males, 3 females; median age 15.0 years, range 13–17) with Palmer 1B TFCC foveal tears confirmed by MRI and arthroscopy. All patients underwent arthroscopic t

Does ulnar variance relate with TFCC lesions?

BACKGROUND: This study aims to investigate triangular fibrocartilage complex (TFCC) alterations and accompanying ligamentous pathologies in cases with positive (PUV) and negative ulnar variance (NUV) using MRI, and to compare findings with a control group presenting with ulnar-sided wrist pain but no trauma history. MATERIALS AND METHODS: A total of 124 patients were retrospectively evaluated: groups with radiographically confirmed PUV or NUV and a control group with ulnar-sided wrist pain but no trauma history. MRI examinations of the wrist were assessed by two musculoskeletal radiologists. E

Negative Ulnar Variance Lessens DRUJ Instability After DRUJ Disruption: A Biomechanical Analysis.

BACKGROUND: The purpose of this study was to perform a biomechanical investigation on the effect of ulnar variance (UV) on the stability of the distal radioulnar joint (DRUJ) prior to and after DRUJ sectioning. METHODS: Ten cadaveric forearm specimens were included in the study and baseline UV was assessed radiographically. Radial motion relative to the ulna was evaluated using Intel real sense cameras and a custom developed program. The forearms were dissected, and a radial osteotomy was performed. Using a custom-made plate, radial stability was assessed with an UV of + 4, 0, and -4 mm by me

Pathomechanisms of ulnar ligament lesions of the wrist in a cadaveric distal radius fracture model.

BACKGROUND AND PURPOSE: Mechanisms of injury to ulnar-sided ligaments (stabilizing the distal radioulnar joint and the ulna to the carpus) associated with dorsally displaced distal radius fractures are poorly described. We investigated the injury patterns in a human cadaver fracture model. METHODS: Fresh frozen human cadaver arms were used. A dorsal open-wedge osteotomy was performed in the distal radius. In 8 specimens, pressure was applied to the palm with the wrist in dorsiflexion and ulnar-sided stabilizing structures subsequently severed. Dorsal angulation was measured on digitized radio

Effect of volar angulation of extra-articular distal radius fractures on distal radioulnar joint stability: a biomechanical study.

The relationship between increased volar tilt of the distal radius and distal radioulnar joint stability was examined. Distal radioulnar joint stiffness was recorded at 10° intervals from 10° dorsal angulation to 20° of volar angulation from the anatomical position of the radius. Tests were performed with the intact radioulnar ligament and repeated after partial and then complete sectioning of the radioulnar ligament at the ulnar fovea. With the intact radioulnar ligament, distal radioulnar joint stiffness increased significantly at 10° and 20° of volar angulation. Partial sectioning of the ra

Injuries of the distal radioulnar joint.

Injuries of the distal radioulnar (DRU) joint are common. If the joint is unstable or discongruous, attempts should be made to restore anatomic alignment and stability. Although most acute injuries of the DRU joint are easily treated, they are often overlooked or misdiagnosed because they usually occur in association with other major injuries of the upper limb. Acute and chronic abnormalities are described briefly and the treatment for each is discussed. Appropriate diagnosis and management of the acute injury will yield a much higher success rate than reconstructive procedures to correct a ch

The distal joint of the radius and ulna. Diagnostic studies and treatment rationale.

Three areas of functional anatomy are present in the distal ulna. Each of these has a specific function in load bearing of the wrist, rotation of the forearm, and maintenance of the relationship between the radius and ulna. The distal portion of the ulna serves as a suspensory post for ligaments that stabilize the ulnar border of the wrist. The midportion of the distal ulna contains the load-bearing triangular fibrocartilage complex. The proximal portion of the distal ulna articulates with the radius at the sigmoid notch and participates in forearm rotation. Each of these areas is assessed in

Clinical and functional outcomes of rehabilitation strategies following arthroscopic repair of chronic isolated peripheral TFCC tears: A scoping review.

This scoping review aims to evaluate the existing literature regarding the types of Triangular Fibrocartilage Complex (TFCC) tears that are amenable to repair, the prevalent rehabilitation protocols, and the documented measurable outcomes across various populations. The review was conducted as per JBI methodology and reported as per PRISMA-ScR guidelines. A total of 35 studies published between 1996 and 2024 were included. Palmer Type 1B was the most frequently operated type of tear. The tears were repaired using various arthroscopic-assisted techniques like capsular repair with polydiaxone su
