[Prehospital air ambulance and systemic secondary cerebral damage in severe craniocerebral injuries].

Advanced supportive therapy at the site of the accident, associated with direct transfer to a trauma centre increases survival and reduces morbidity rates. Patients with severe head injury, especially those with multiple injuries, often arrive in the emergency department with potentially causes of serious secondary systemic insults to the already injured brain, such as acute anemia (Hematocrit < or = 30%), hypotension (systolic arterial pressure (Pasys) < or = 95 mmHg, 12.7 kPa), hypercapnia (Paco2 > or = 45 mmHg, 6 kPa) and/or hypoxemia (Pao2 < or = 65 mmHg, 8.7 kPa). The incidence of such in

Acute Cycling Injuries.

The overall activity of cycling, although profoundly heterogenous, often occurs with mechanisms consistent with motor vehicle collisions. Advanced trauma life support is the standard of care. Traumatic brain injury is the leading cause of death, and concussions are common in cyclists. Road rash is the most common injury, and management should be synonymous with other kinds of burns. A unique aspect of cycling medicine is that it often is done on public roadways in close proximity to the athletes during competition. Clinicians who care for cyclists in the field setting should be prepared to man

Housing First is associated with reduced use of emergency medical services.

OBJECTIVE: Chronically homeless adults with severe alcohol problems are disproportionately burdened with health-care problems and are high utilizers of emergency medical services (EMS). Single-site Housing First (HF), which provides immediate, permanent, low-barrier, nonabstinence-based, supportive housing, has been associated with reduced publicly funded service utilization. The aims of the current study were to determine whether time spent in single-site HF predicted decreases in EMS contacts 2 years subsequent to single-site HF move-in, and to describe medical conditions and injuries associ

Successful Resuscitation: Novel Partnership between Paramedics and U.S. Coast Guard.

In this article, we present a notable case that illustrates a novel partnership between a United States Coast Guard small boat station and a civilian paramedic response unit. Patients who experience medical emergencies in remote environments are at particularly high risk for morbidity and mortality. For the most serious conditions, delayed contact with Advanced Life Support (ALS) has grave results. Typically, these circumstances involve small groups of individuals and cannot be easily predicted. The waters off the coasts of Maui, Hawaii, however, host millions of residents and visitors annuall

Strengthening trauma care in low- and middle-income countries through guideline adaptation and scalable system reforms.

Trauma is a leading global cause of death and disability, with nearly 90% of fatalities occurring in low- and middle-income countries (LMICs), where prehospital care, infrastructure, and trained personnel are limited. This narrative review synthesizes literature (2019-2025) on the initial management of adult polytrauma, and explores strategies to adapt global guidelines in resource-constrained settings. Contemporary frameworks such as advanced trauma life support (ATLS) and World Health Organization essential trauma care emphasize context-sensitive sequencing, permissive hypotension, balanced

Accuracy and outcome of rapid ultrasound in shock and hypotension (RUSH) in Egyptian polytrauma patients.

PURPOSE: "Polytrauma" patients are of a higher risk of complications and death than the summation of expected mortality and morbidity of their individual injuries. The ideal goal in trauma resuscitation care is to identify and treat all injuries. With clinical and technological advanced imaging available for diagnosis and treatment of traumatic patients, point of care-rapid ultrasound in shock and hypotension (RUSH) significantly affects modern trauma services and patient outcomes. This study aims to evaluate the accuracy of RUSH and patient outcomes by early detection of the causes of unstabl

Advanced Trauma Life Support (ATLS) and facial trauma: can one size fit all? Part 1: dilemmas in the management of the multiply injured patient with coexisting facial injuries.

Maxillofacial trauma is without doubt still a very challenging area, especially in the early stages of care when other injuries may be present. Craniofacial trauma, with or without life and/or sight-threatening complications, may be associated with significant injuries elsewhere. Both general trauma and facial trauma management have evolved considerably over the last 20 years and on occasion clinical priorities may seemingly conflict, suddenly change or be hidden. In these circumstances a number of clinical dilemmas may arise, which this and three further reviews will discuss. These are based

Advancing Discussions Using a Video-Based Support Tool About End-of-Life Care (ADVISE): Protocol for a Single-Center Randomized Controlled Trial.

BACKGROUND: More than 950,000 adults over 65 years of age are hospitalized each year after injury. Recommended care for this vulnerable group includes discussion of their goals for life-sustaining care ("code status") upon admission, but most older patients do not participate in adequate code status discussions. Randomized trials have demonstrated that video-based interventions are associated with better patient knowledge and improved alignment of patient preferences for life-sustaining treatment; however, there are limited data on the effectiveness of these interventions in acutely injured pa

Life support in hemodialysis emergencies-treatment standard.

Hemodialysis removes uremic toxins and excess water and sodium from the blood of patients with kidney failure. For the last 60 years, this technique has been extensively improved in order to optimize patient survival and minimize adverse events. Indeed, life-threatening complications related to hemodialysis have become very rare with the use of modern machines and equipment. These hemodialysis-related serious events that became less frequent include hemolysis, air embolism, dialyzer reactions and dialysis disequilibrium syndrome. On the other hand, other medical emergencies are regularly encou

Is advanced life support appropriate for victims of motor vehicle accidents: the South Carolina Highway Trauma Project.

There is continuing controversy over the use of Advanced Life Support (ALS) in the treatment of multisystem injury. In this study, performed to define the role of ALS in the management of motor vehicle accidents (MVA), 538 ambulance run reports (ARR) and hospital records of patients involved in MVA in South Carolina for 1983 were examined. Of these, 248 were reviewed in depth by a trauma review committee (TRC). Paramedics were present in 81% (93% urban, 80% rural) of cases. ALS crews averaged 24.8 minutes on the scene compared to 18.1 minutes for Basic Life Support (BLS). It took 6 minutes lon

Advanced trauma life support training for ambulance crews.

BACKGROUND: There is an increasing global burden of disease from injuries. Models of trauma care initially developed in high-income countries are also being adopted in low and middle-income countries (LMIC). Amongst these ambulance crews with Advanced Life Support (ALS) training are being promoted in LMIC as a strategy for improving outcomes for victims of trauma. However there is controversy as to the effectiveness of this health service intervention, and the evidence has yet to be rigorously appraised. OBJECTIVES: To quantify the effectiveness of ambulance crews with ALS training versus cre

To log-roll or not to log-roll - That is the question! A review of the use of the log-roll for patients with pelvic fractures.

Pelvic fractures are one of the most serious injuries in polytrauma - massive haemorrhage is a major complication and the leading cause of death. The assessment and early management of these injuries in the Emergency Department (ED) focuses on the recognition of haemodynamic instability and should follow Advanced Trauma Life Support (ATLS) guidelines. The log-roll technique, although an accepted practice in trauma care, has been reported to be inadvisable when a pelvic fracture is suspected. This paper, which includes a case study, briefly explains the pathophysiology and initial management of

Pitfalls in penetrating trauma.

In Western Europe the most frequent cause of multiple injuries is blunt trauma. Only few of us have experience with penetrating trauma, without exception far less than in the USA or South-Africa. In Rotterdam, the Erasmus Medical Centre is a level I trauma centre, situated directly in the town centre. All penetrating traumas are directly presented to our emergency department by a well organized ambulance service supported by a mobile medical team if necessary. The delay with scoop and run principles is very short for these cases, resulting in severely injured reaching the hospital alive in inc

Comprehensive Management of the Patient With Traumatic Cardiac Injury.

Cardiac injuries are rare but potentially life-threatening, with a significant proportion of victims dying before arrival at the hospital. The in-hospital mortality among patients who arrive in-hospital alive also remains significantly high, despite major advancements in trauma care including the continuous updating of the Advanced Trauma Life Support (ATLS) program. Stab and gunshot wounds due to assault or self-inflicted injuries are the common causes of penetrating cardiac injuries, while motor vehicular accidents and fall from height are attributable causes of blunt cardiac injury. Rapid t

The differences in injury patterns and outcomes of thoracic trauma between rural and urban level two trauma centers in a single country.

PURPOSE: Equal level trauma centers in the same country might have significant differences regarding their demographics and types of trauma. Understanding geographic variations in injury patterns are essential for optimal care. Here we describe the differences in injury patterns and associated outcomes of thoracic trauma patients between rural and urban level-II trauma centers in a single country. METHOD: A retrospective analysis of patients with thoracic trauma over a three-year period in a rural level-II and an urban level-II in Israel were compared. Demographics, mechanism of injury, preho

Non-operative advances: what has happened in the last 50 years in paediatric surgery?

Paediatric surgeons remain paediatric clinicians who have the unique skill set to treat children with surgical problems that may require operative intervention. Many of the advances in paediatric surgical care have occurred outside the operating theatre and have involved significant input from medical, nursing and allied health colleagues. The establishment of neonatal intensive care units, especially those focusing on the care of surgical infants, has greatly enhanced the survival rates and long-term outcomes of those infants with major congenital anomalies requiring surgical repair. Clinical Epidemiology of Trauma Patients: A Retrospective Analysis of 3705 Consecutive Patients Treated at a Level I Trauma Center.

BACKGROUND: Trauma is a leading cause of morbidity and mortality globally, particularly in low- and middle-income (LMIC) countries like India, where road traffic injuries (RTIs) dominate. Despite advancements in medical technology, trauma care remains underdeveloped due to resource limitations, inadequate pre-hospital care, and poor compliance with safety measures. This study aimed to analyze the clinical-epidemiological profile of trauma patients and develop strategies for effective trauma prevention and management. METHODS: A retrospective review of 3,705 trauma patients admitted to a level

High-Fidelity, Indigenously Prepared, Low-Cost Moulage as a Valid Simulation Tool to Improve Trauma Education.

Background Simulation-based trauma education facilitates repeated practice in a controlled and safer environment for the learner without any risk to the patient's well-being. Moulage contributes to the perception of reality during training using standardized patients. However, the high cost of commercial moulage items is often prohibitive for regular use. This study aimed to assess the effectiveness of indigenously prepared, low-cost moulage as a valid simulation tool to improve trauma education, explore possible replacements of commercial moulage products, and determine their merits and demer

Beyond the Golden Hour: Association of Prehospital Time and Intervention Quality With Functional Outcomes in Severely Injured Trauma Patients.

BACKGROUND: Trauma remains a major cause of mortality and disability worldwide, particularly in low- and middle-income countries (LMICs). Although the "golden hour" concept emphasises rapid transport after injury, emerging evidence suggests that the quality of prehospital care may influence outcomes more than transport time alone. Data evaluating the relationship between prehospital care and trauma outcomes in low-resource settings remains limited. OBJECTIVE: To evaluate the association between prehospital time and prehospital treatment with outcomes in severely injured trauma patients presen
