UNLABELLED: BACKGROUND All neonates have free open access to the Baby Clinic at the maternity hospitals in Dublin for assessment of neonatal health issues. Through observation, however there is an increase in number of neonates attending the hospital outside the Baby Clinic hours.
AIMS: To determine the number of neonates attending the acute neonatal service out of hours and to identify the percentage of neonates treated as true emergency.
METHODS: Retrospective chart review over a twelve-month period.
RESULTS: Seven hundred and thirty-two neonates attended the hospital out of hours. The ma

RATIONALE, AIMS, AND OBJECTIVES: Minimizing the risks and distress arising from aggression in after-hours house call (AHHC) services will help improve service quality in the industry. Unfortunately, no national study has ever evaluated this in Australian AHHCs. Apart from reducing this gap, findings from this work will have global relevance given the rising popularity of the AHHC industry worldwide.
METHODS: A survey of all 300 doctors employed by the National Home Doctor Service, Australia's largest AHHC service provider. A validated electronic questionnaire was used to examine the doctors'

Diagnostic error and diagnostic delays in health care are widespread. This article outlines an improvement effort targeting weekday evening inpatient radiology delays through staffing changes replacing trainees with faculty-trainee team coverage, pushing faculty coverage from 4 pm to 8 pm. Order-report turnaround times (TATs), critical findings TATs for pneumothorax and intracranial hemorrhage (ICH), and percentage meeting target were compared pre and post implementation for the 4 to 8 pm time frame using the Mann-Whitney U and χ2 tests, respectively. Stakeholder surveys assessed patient safet

OBJECTIVE: A significant gap exists in medical support for organ transplant patients during out-of-hours (OOH). General large language models (LLMs), affected by AI hallucinations, are unsuitable for complex post-transplant care. We built the first post-transplant AI agent based on LLMs to address these issues.
METHODS: We constructed a specialized "post-transplant AI agent" (named Doctor Xiao Yi) and conducted a mixed-methods study comparing it to a hospital-wide general AI agent (named Nan Xiao Yi). Data included 20,176 real-world logs (June-December 2025) and a cross-sectional survey of 15

BACKGROUND: Shortness of breath often prompts calls to out-of-hours primary care and is the prime reason for home visits by GPs. However, a diagnostic prediction model in this setting is lacking.
AIM: To develop and internally validate a model predicting life-threatening events for out-of-hours primary care callers with shortness of breath.
DESIGN AND SETTING: This cross-sectional study includes data from 1952 patients with shortness of breath who called Dutch out-of-hours primary care between September 2020 and August 2021. The trial was registered with the Netherlands Trial Register, numbe

INTRODUCTION: In the Netherlands, the 'Netherlands Triage Standard' (NTS) is frequently used as digital decision support system for telephone triage at out-of-hours services in primary care (OHS-PC). The aim of the NTS is to guarantee accessible, efficient and safe care. However, there are indications that current triage is inefficient, with overestimation of urgency, notably in suspected acute cardiovascular disease. In addition, in primary care settings the NTS has only been validated against surrogate markers, and diagnostic accuracy with clinical outcomes as the reference is unknown. In th

BACKGROUND: Child abuse is often unrecognized at out-of-hours primary care (OOH-PC) services. The aim of our study was to evaluate the clinical outcome of the screening instrument SPUTOVAMO-R2 for child abuse (checklist), followed by a structured approach (reporting code), at OOH-PC services. The reporting code with five steps should ensure consistent action in case of a suspicion.
METHODS: All children attending one of the five participating OOH-PC services in the region of Utrecht, the Netherlands, in a year time, were included. The checklist is an obligatory field in the electronic patient

Hospitalizations for opioid use disorder (OUD) are rising, yet overnight clinicians rarely receive targeted OUD education. Our interprofessional initiative implemented two overnight training sessions specifically for night-shift physicians, nurses, pharmacists, and advanced practice providers, emphasizing buprenorphine initiation, stigma reduction, harm reduction, and patient-centered communication. Participants in the program demonstrated increased confidence in OUD management and a self-reported rise in overnight buprenorphine initiation, highlighting the impact of this tailored night-shift

OBJECTIVES/HYPOTHESIS: Otolaryngology residents are often responsible for triaging after-hours patient calls. However, residents receive little training on this topic. Data are limited on the clinical content, reporting, and management of otolaryngology patient calls. This study aimed to characterize the patient concerns residents handle by phone and their subsequent management and reporting.
STUDY DESIGN: Retrospective review.
METHODS: Five hundred consecutive after-hours patient calls in a tertiary pediatric hospital were reviewed. Data collected included patient and caller demographics, c

BACKGROUND: Minor surgical procedures are regularly performed in general practice. The practice with regard to routine use of sterile or non-sterile gloves varies. The purpose of this article was to review the scientific basis for choice and recommendation in this area.
MATERIAL AND METHOD: A search was conducted in PubMed, Cochrane, Medline and Embase for clinical studies in English which had investigated the use of sterile or non-sterile gloves and the prevalence of wound infection related to minor surgery. This applied primarily to general practice, including accident and emergency departm

OBJECTIVE: To obtain information about the frequency of call-outs, relative frequency of different types of dental emergencies that presented, and treatment provided between 1999 and 2003 by an out-of-hours on-call rota run by a cooperative of 12 dentists in Herefordshire.
METHOD: A retrospective analysis was undertaken of the data recorded in the on-call log of the cooperative from March 1999 to December 2003. Data recorded were: date of call, time of call, dentist on call, contract type (National Health Service, Denplan, other capitation scheme, private fee-per-item-of-service, or unregiste

BACKGROUND AND OBJECTIVES: The Dutch Triage Standard (NTS) supports nurses in assessing case urgency in out-of-hours general practice (GP) care; however, its suitability for fever-related pediatric cases remains debated. This study examines the agreement between pre-consultation NTS urgency classifications (U-scores) and post-consultation management decisions in febrile children, and whether the NTS reduces between-center variation in decision-making.
METHODS: We performed a secondary analysis of the CHILI study, including 22 089 consultations for children under 12 with fever, across 18 out-o

BACKGROUND: Clinical scoring systems can help predict sepsis and guide treatment. We developed and validated a new sepsis prediction score for acutely ill adult patients visited at home by a general practitioner (GP) of an out-of-hours (OOH) cooperative.
AIM: To assess the feasibility and acceptability of implementing this score during OOH home visits.
DESIGN AND SETTING: Theory-informed mixed-methods study at two OOH GP cooperatives in the Netherlands between January and June 2024.
METHOD: GPs calculated the sepsis score in adult patients during home visits and completed a questionnaire ab

BACKGROUND: Women and men seem to perceive shortness of breath (SOB) differently. However, it is unknown whether symptom presentation varies between genders during a life-threatening event (LTE).
AIM: To assess whether symptoms associated with an LTE vary between women and men with SOB calling out-of-hours primary care (OHS-PC).
METHODS: Cross-sectional study including data from patients contacting two large Dutch OHS-PC centres for SOB between 1 September 2020 and 31 August 2021. We compared symptoms mentioned during triage conversations between patients with and without LTEs (amongst other

OBJECTIVES: Telephone triage of patients suspected of transient ischaemic attack (TIA) or stroke is challenging. Both TIA and stroke more likely occur during daytime, with a peak in the morning hours. Thus, the time of calling might be a helpful determinant during telephone triage. We assessed the time of calling in patients with stroke-like symptoms who called the out-of-hours services in primary care (OHS-PC), and evaluated whether the time of calling differed between patients with TIA or stroke compared with those with mimics.
DESIGN: Cross-sectional study.
SETTING: Six OHS-PC locations i
