OBJECTIVE: Describe the unusual complication of lower extremity compartment syndrome occurring in an adolescent with spinal cord injury (SCI).
METHODS: Case presentation.
RESULTS: A 17-year-old male with C5 ASIA A complete SCI developed a compartment syndrome of his lower leg on the ninth day postinjury. Presenting signs included an equinus deformity of the foot, blackened induration over the anterior tibia, circumferential erythematous markings over the calf, large urticarial lesions over the knee, and calf swelling. The presumed etiology of the compartment syndrome was excessive pressure f

BACKGROUND: Acute compartment syndrome is a serious and not uncommon complication of limb trauma. The condition is a surgical emergency and is associated with significant morbidity if not diagnosed promptly and treated effectively. Despite the urgency of effective management to minimize the risk of adverse outcomes, there is currently little consensus in the published reports as to what constitutes best practice in the management of acute limb compartment syndrome.
METHODS: A structured survey was sent to all currently practising orthopaedic surgeons and accredited orthopaedic registrars in A

We report a case in which compartment syndrome and tourniquet paralysis occurred simultaneously. This is a previously unreported combination which presents a diagnostic problem. We recommend that electrophysiological studies and continuous monitoring of compartment pressures should be used to minimise morbidity in high-risk cases.

BACKGROUND: Acute compartment syndrome occurs because of increased pressure within the fascial compartment, resulting in tissue ischemia. This surgical emergency, usually resulting from severe injury, is characterized by rapidly developing pain and swelling of the extremities. This case report aims to raise awareness among physicians by emphasizing the importance of high clinical sense in the diagnosis of compartment syndrome.
CASE PRESENTATION: This report describes a 25-year-old Saudi male who presented to the emergency department with a 1-week history of left leg pain. The pain suddenly wo

A 26-year-old, ASA1 patient underwent maxillofacial surgery under general anaesthesia, of 12-hour duration in the supine position. Postoperatively he developed rhabdomyolysis and acute renal failure. In the subsequent days, a bilateral leg compartment syndrome occurred with anterior tibial motor nerve injury requiring fasciotomies and excision of necrotic muscles. Several aetiological factors may have contributed to this accident: a long-lasting procedure, controlled hypotension and inappropriate position of the lower limbs. A laboratory study showed that the hardness of some new operating tab

Chronic tibialis anterior syndrome affects most frequently adolescent sportsmen. It is manifested by pain on the anterior side of the leg and sometimes also by claudications after burdening. The cause is hypertrophy of the m. tibialis anterior which leads to increased subfascial pressure and thus development of the syndrome. The diagnosis is frequently incorrect and the condition is treated as periostitis or enthesopathy. As the cause is increased subfascial pressure after burdening, causal treatment is fasciotomy of the anterior compartment of the leg. The authors present a case of bilateral

Acute compartment syndrome of the thigh is rare due to the space's ability to accommodate large volumes of fluid and, with the exception of the lateral septum, its thin compliant linings. This article describes a case of bilateral exercise-induced severe compartment syndrome treated with anterior and posterior fasciotomies. A 29-year-old man was admitted to intensive care with myoglobinuria. His left thigh was evaluated 18 hours later for compartment syndrome. The patient reported that 14 hours prior to initial presentation, he had participated in a 1-hour session of vigorous basketball. He gr

Fifteen compartment syndromes seen in the first few hours have been collected. Fourteen occurred after tibial fracture and one after a simple contusion. In typical cases it is emphasised that ischaemia affects the contents of the compartment in an insular fashion so that distal ischaemia is not present. The main arteries are sound and demonstrated by distal pulses, the vascularity of the foot, Doppler studies and arteriography. The diagnosis is difficult but must be made at the earliest opportunity by repeated clinical examination. Measurement of the compartment pressure may be useful because

We retrospectively reviewed the cases of three patients with injuries similar to the mechanism of an ankle sprain which resulted in compartment syndrome of the lower extremity. All three patients presented with sharp, proximal, anterolateral pain in the leg after an indirect, twisting injury. None of the injuries involved direct contact. Two of the three athletes initially presented to local hospitals, where the injury was misdiagnosed as an ankle sprain. The mechanism appears to be a muscle strain or tear with resultant hemorrhage in the compartment. All three patients returned to high-level

Compartment syndrome represents the most serious complication in the treatment of the fractures of the shin. In the period 1979-1987 17 patients with variously developed form of the compartment syndrome were treated at the Orthopaedic clinic of the Institute for In-service Education of Physicians. 7 patients, i. e. 40 per cent were transported from other clinics. There were 8 open fractures in the group, i. e. 47 per cent, and they were classified according to Allgöwer. All patients were primarily operated on for the fracture of the shin and the compartment syndrome occurred in 11 cases after

A compartmental syndrome is defined as a condition in which increased pressure within a space compromises the circulation to the contents of that space. Any cause of increased intracompartmental pressure may result in a compartmental syndrome. The diagnosis should be suspected in any case of pain or neuromuscular deficit in an extremity and may be confirmed by signs of circulatory disturbance of nerve and muscle in association with increased pressure in the compartment. Generous opening of any dressings covering the extremity permits a proper examination and rules out a compartmental syndrome

Acute compartment syndrome of the lower extremity is a serious postinjury complication that requires emergency treatment. Early diagnosis is of paramount importance for a good outcome. Four muscle compartments in the calf (anterior, lateral, deep posterior, and superficial posterior) may be individually or collectively affected. Acute segmental single-compartment syndrome is an extremely rare condition characterized by high pressure in a single compartment space with threatening of the segmental tissue viability. In this case report, we describe a young man with Achilles tendon rupture who com

Acute compartment syndrome is a surgical emergency. A high index of suspicion is needed in cases of severe contusion, especially if patient complaints seem to outweigh physical findings. Acute surgical fasciotomy is an effective treatment and should be carried out expeditiously if compartment pressures are elevated over 70 mm Hg in order to avoid complications such as chronic disability due to contractures or rhabdomyolysis with acute renal failure. Chronic exertional compartment syndrome most often occurs in the anterior or the lateral compartment. Patients have only exercise-related symptoms

An account is given of unusual course of a hyperthermic crisis in a 23-year-old male who underwent repeated anesthetics. As yet little has been reported about Isoflurane, which we presume to have been the triggering agent. In this case only the surgically untreated lower extremity developed rigor, with which malignant hyperthermia is associated, whereas the surgically treated extremity, where circulation had been stopped with a tourniquet, remained unaffected. Rigor and contracture of the affected extremity were so severe that they led to a compartment syndrome, necessitating fasciotomy. No ob

Compartment syndrome in the thigh is an uncommon condition, and acute compartment syndrome without an associated fracture is even less common. An 18-year-old male developed an acute anterior compartment syndrome in the thigh from a contusion without an associated fracture. This syndrome was treated successfully with a fasciotomy.

A compartment syndrome developed in eleven patients who had undergone the Hauser procedure. The residual disabilities ranged from mild weakness and contracture of the muscles in the anterior compartment of the leg to complete muscle necrosis necessitating above-the knee amputation in two patients. Dissections of ten cadaver limbs demonstrated that the anterior tibial recurrent vessels have numberous leash-like branches that terminate along the lateral border of the tibial tubercle. When these vessels are sectioned they retract laterally and distally under the fascia and within the muscles of t

A retrospective review of the cases of 180 patients who had 198 acute open fractures of the tibial shaft and were admitted to a multiple-trauma referral center over a three-year period revealed an incidence of accompanying compartment syndrome of 9.1 per cent (eighteen fractures in sixteen patients). Each of the eighteen compartment syndromes was documented by measurements of intracompartmental pressure that were obtained by the saline-injection technique, and all were treated by four-compartment fasciotomy. The incidence of compartment syndrome was found to be directly proportional to the deg
