Myocardial injury after orbital atherectomy and its association with coronary lesion length.

BACKGROUND: Limited data are available regarding myocardial injury and its risk factors in percutaneous coronary interventions (PCI) of severe calcified lesions using orbital atherectomy (OA). METHODS: Patients who underwent OA at our institution were retrospectively enrolled into the present registry. High-sensitive Troponin I (hsTroponin I), EKG and echocardiography were used to assess myocardial injury after the procedure. RESULTS: A total of 27 patients between who underwent OA between January 2022 and June 2023 were included. Myocardial injury (elevation of hsTroponin I above the 99th p

Surgical glue-induced left main trunk stenosis removed by directional coronary atherectomy.

A 55-year-old man underwent aortic repair for acute aortic dissection. The pseudolumen of the Valsalva sinus was reapproximated with BioGlue by placing a sponge inside of it to prevent the BioGlue from entering. Postoperative contrast-enhanced computed tomography showed stenosis of the left main trunk. Directional coronary atherectomy was performed; complete release of the stenosis was achieved. Various fragments retrieved from the left main trunk were pathologically consistent with BioGlue. When we reapproximated the dissected aortic wall, inserting a sponge into the aorta did not prevent the

Percutaneous Coronary Intervention of Complex Calcific Coronary Lesions Utilizing Orbital Atherectomy Prior to Transcatheter Aortic Valve Replacement.

BACKGROUND: Coronary artery disease (CAD), often with severe calcification, is present in up to 75% of patients with severe aortic stenosis (AS) referred for transcatheter aortic valve replacement (TAVR). Management of CAD in such patients is challenging. Orbital atherectomy (OA) is an effective treatment of severely calcified coronary lesions prior to stent implantation. However, there is limited data on the use of OA for percutaneous coronary intervention (PCI) to treat calcific CAD patients prior to TAVR (OA PCI + TAVR). METHODS: Retrospective analysis of patients with moderate/severe calc

Successful Removal of a Fractured Guidewire During Percutaneous Coronary Intervention With Orbital Atherectomy.

BACKGROUND: Guidewire entrapment or fracture is a rare complication during percutaneous coronary intervention (PCI). Timely management is warranted to prevent poor cardiac outcomes. CASE SUMMARY: An 83-year-old man with coronary artery disease was referred for PCI given severe left anterior descending disease and right coronary artery disease (RCA). During PCI of in-stent restenosis of the RCA, the 0.014-inch Versaturn guidewire (Abbott) was entrapped in the coronary calcium within a prior stent, and the tip fractured. The wire entrapment led to a technically complex procedure, which was trea

Outcomes Following Orbital Atherectomy for Coronary Calcified Nodules: A Retrospective Single-Center Experience.

BACKGROUND: Coronary calcified nodules (CNs) are a challenging subset of calcific lesions associated with adverse procedural outcomes. While rotational atherectomy (RA) and balloon angioplasty (BA) have been traditionally used, orbital atherectomy (OA) offers a unique mechanism of plaque modification that may be advantageous in the treatment of CNs. Data on OA in CNs remains limited. OBJECTIVES: To evaluate procedural success, periprocedural safety, in-hospital and long-term outcomes of retrograde OA in the treatment of CNs. METHODS: We conducted a retrospective analysis of all patients who

Excimer laser coronary atherectomy in severely calcified lesions: time to bust the myth.

INTRODUCTION AND OBJECTIVES: No previous studies have established the contemporary use and outcomes of Excimer laser coronary atherectomy (ELCA) in percutaneous coronary intervention (PCI) of severely calcified coronary lesions. The aim of this study was to assess the safety, efficacy, and 1-year outcomes of ELCA in this setting. METHODS: We retrospectively examined the clinical and angiographic characteristics and procedural outcomes of severely calcified lesions treated with ELCA-assisted PCI in our institution between 2016 and 2022. RESULTS: Seventy-eight consecutive patients (80 procedur

Detailed angiographic analysis of high-speed mechanical rotational atherectomy in human coronary arteries.

BACKGROUND: Several types of atherectomy devices have been developed recently for treatment of patients with ischemic heart disease. METHODS AND RESULTS: Mechanical rotational atherectomy (MRA) using a high-speed rotational burr (Rotablator) was performed on 116 lesions in 104 patients. MRA alone was performed in 27 lesions (23%), and conventional balloon angioplasty (PTCA) was performed after MRA in 89 lesions (77%). Diameter stenosis decreased from 70 +/- 13% before MRA to 54 +/- 23% after MRA, and the final diameter stenosis (after MRA alone or with adjunctive PTCA) was 30 +/- 20% (P < .00

Clinical outcomes of excimer laser coronary atherectomy (ELCA) in percutaneous coronary intervention: A 10-year single-centre experience.

BACKGROUND: Excimer laser coronary atherectomy (ELCA) is used in complex percutaneous coronary intervention (PCI), but contemporary long-term data across indications remain limited. METHODS: We retrospectively included consecutive adults undergoing ELCA-assisted PCI at a tertiary centre from 2015 to 2025. Procedural and in-hospital outcomes were assessed in all patients. Long-term time-to-first major adverse cardiac events (MACE; target-vessel revascularisation, non-fatal myocardial infarction, or all-cause death) were evaluated among patients discharged alive. RESULTS: 62 patients were incl

Kokeshi phenomenon and coronary perforation with rotational atherectomy while treating heavily calcified coronary artery disease: A case report.

Источник пометил публикацию как отозванную

RATIONALE: Coronary artery disease remains a leading cause of morbidity, requiring complex revascularization strategies, especially in patients with heavily calcified lesions. Percutaneous coronary intervention (PCI) is a common treatment, but it carries risks such as coronary artery perforation and the rare Kokeshi phenomenon during rotational atherectomy (RA). This study aims to emphasize the challenges faced in treating heavily calcified coronary lesions, specifically focusing on the rare Kokeshi phenomenon and coronary artery perforation during RA. PATIENT CONCERNS: We report the case of

Effect of Sex on Outcomes of Coronary Rotational Atherectomy Percutaneous Coronary Intervention (From the European Multicenter Euro4C Registry).

Data regarding the potential influence of gender on outcomes of rotational atherectomy (RA) percutaneous coronary intervention (PCI) are scarce and conflicting. Using the Euro4C registry, an international prospective multicentric registry of RA PCI, we evaluated the influence of gender on clinical outcomes of RA PCI. Between October 2016 and July 2018, 966 patients were included. In them, 267 (27.6%) were females. Female patients were older than males (77.7 years old ± 9.8 vs 73.3 ± 9.5 years old respectively, p < 0.001) had a poorer renal function (43,1% of females had a GFR < 60 ml/min:1.73m

Rotational Atherectomy With or Without Intravascular Lithotripsy for Patients With Heavy Coronary Artery Calcification.

Recent advances in calcium debulking technologies may contribute to development of new percutaneous coronary intervention (PCI) paradigms for patients with heavy coronary artery calcification (CAC). Comparative data on safety and clinical efficacy of a combination strategy including rotational atherectomy (RA) and intravascular lithotripsy (IVL) is lacking. The aim of the study was to determine the addional effect of IVL when performed after RA in patients with heavy CAC. All patients with heavy CAC undergoing PCI with RA between September 2021 and August 2023 at a tertiary care center were pr

Aminophylline for Preventing Bradyarrhythmias During Orbital or Rotational Atherectomy of the Right Coronary Artery.

BACKGROUND: Coronary atherectomy, orbital or rotational, is frequently used for plaque modification in patients with heavily calcified lesions. Atherectomy can be associated with clinically significant bradyarrhythmias or transient atrioventricular block requiring temporary pacemaker insertion, mainly in lesions involving the right coronary artery or a dominant left circumflex artery. Bradyarrhythmias may be mediated by endogenous release of adenosine from red blood cell breakdown. Aminophylline, an adenosine antagonist, can prevent adenosine-mediated bradyarrhythmias. METHODS: This retrospec

Histologic comparison of coronary and iliac atherectomy tissue from cases of in-stent restenosis.

Pathologically, restenotic lesions after stenting were investigated by use of atherectomized tissues of seven coronary and seven iliac arteries. The mean interval of the stent deployment to restenosis was 9.1 months for the coronary artery and 33.7 months for the iliac artery, indicating a 3.7-fold longer interval for the latter. This study does not include cases of acute thrombotic occlusion. The atherectomized tissue from restenotic coronary arteries showed abundant neointima with alpha-actin-positive and ultrastructually synthetic-type smooth muscle cells in a rich myxomatous extracellular

One-Year Outcomes of Orbital Atherectomy of Long, Diffusely Calcified Coronary Artery Lesions.

OBJECTIVES: The aim of this study was to determine the clinical outcomes of patients with long, diffusely calcified coronary artery lesions who underwent orbital atherectomy. BACKGROUND: The presence of severe coronary artery calcification increases the complexity of percutaneous coronary intervention. Orbital atherectomy of long, diffusely calcified lesions may increase the risk of periprocedural angiographic complications. Furthermore, the rate of ischemic complications, including target-vessel revascularization (TVR), in these long, calcified lesions is historically high. METHODS: In this

Complete "in situ" avulsion of the radial artery complicating transradial coronary rotational atherectomy.

BACKGROUND: Transradial percutaneous access (TR) is promoted because of increased patient comfort and convenience as well as a lower risk of access site and cardiac complications in the literature. Increased use of the TR purports a new set of possible complications for which the vascular surgeon must be capable to recognize and manage. METHODS: A 48-year-old, devout Jehovah's Witness, woman with a history of coronary artery bypass surgery presented with a non-ST-segment elevation acute myocardial infarction. Restenosis after directional coronary atherectomy. Effects of luminal diameter and deep wall excision.

BACKGROUND: Deep wall excision during directional atherectomy has been reported in one study to increase the risk of subsequent restenosis. On the other hand, we have observed that the probability of late (6-month) restenosis is reduced by maximizing postprocedure luminal diameter. Although such maximal luminal enlargement by directional atherectomy has not increased procedural complications in our experience, it might well increase the incidence of subintimal (deep wall component) recovery. We performed this study to evaluate the relative influences of luminal enlargement and deep wall compon
