Fenestrated and branched endografts for the treatment of thoracoabdominal aortic aneurysms: a systematic review.

PURPOSE: To offer a critical review of the current literature on the use of fenestrated and branched stent-grafts in patients with thoracoabdominal aortic aneurysms (TAAA). METHODS: A thorough search of the English-language literature published between January 2000 and September 2009 identified reports of endovascular procedures using fenestrated and/or branched endografts as the intended repair strategy in patients with TAAA. Studies were selected based on specific inclusion criteria: (1) >3 high-risk patients with preoperative diagnosis of TAAA, (2) the intended treatment strategy was an en

Zenith abdominal aortic aneurysm endovascular graft.

PURPOSE: The safety and efficacy of the Zenith (Cook Inc, Bloomington, Ind) endovascular graft was assessed based on the United States multicenter trial through 5 years of follow-up. METHODS: Between 2000 and 2003, the pivotal study enrolled patients to open surgery (control) or the Zenith endovascular graft (endovascular). A separate continued access study arm enrolled endovascular patients using the same inclusion/exclusion criteria. Both studies were designed for 2-year follow-up, and the pivotal endovascular patients had the option of extending the study follow-up through 5 years. All end

Endoconversion During b-EVAR for Pararenal Abdominal Aortic Aneurysm.

Adverse events associated with stent-grafts during complex aortic procedures are rare but can be difficult to manage. These occurrences can be reduced by precise planning and anticipating complications by considering the patient's anatomy and the characteristics of the graft. This paper describes an endovascular conversion following branched endovascular aneurysm repair (b-EVAR) for a pararenal abdominal aortic aneurysm (pAAA) caused by the distal displacement of the stent-graft during delivery system retrieval. A replanned endovascular strategy (endoconversion) was performed by the deployment

T-next: a new custom-made Thoraflex graft to simplify proximal and distal aortic reinterventions.

Secondary root and distal thoraco-abdominal endovascular aortic re-interventions can be challenging after frozen elephant trunk. We obtained from the TERUMO Aortic custom-made platform a Thoraflex graft with a modified disposition of the arch branches that facilitate secondary proximal and distal reinterventions. Here we describe the graft and our first implant.

Hybrid aortic arch and frozen elephant trunk reconstruction: bridging the gap between conventional and total endovascular arch repair.

Novel endovascular techniques hope to offer patients aortic arch repair with reduced morbidity compared to conventional arch surgery; however, current endovascular strategies remain challenged by the proximal seal zone, higher stroke rates, long-term durability and select anatomy. Hybrid arch repair offers patients a less invasive alternative that can treat more distal aorta than conventional arch repair yet still be performed via standard sternotomy. Areas covered: This review will discuss the current evidence and future development of hybrid aortic arch and frozen elephant trunk reconstructi

Preclinical evaluation of the InCraft" aortic endograft in a sheep model.

Animal models remain the gold standard for the preclinical evaluation of tissue response, sealing and integrity of aortic endografts. Preclinical testing of the InCraft® device was performed to evaluate these attributes. Through the femoral arteries of eight male crossbred sheep, 22 mm diameter InCraft® Aortic Bifurcate devices were deployed in the abdominal aortas, and shortened 13 mm diameter iliac limbs were deployed in the right iliac arteries. Vessels were excised for radiographic and histopathologic assessment at six months. There were no instances of graft thrombosis, type I endoleak or

Evaluation of vascular repair by tissue-engineered human acellular vessels or expanded polytetrafluoroethylene grafts in a porcine model of limb ischemia and reperfusion.

BACKGROUND: This study evaluated performance of a tissue-engineered human acellular vessel (HAV) in a porcine model of acute vascular injury and ischemia. The HAV is an engineered blood vessel consisted of human vascular extracellular matrix proteins. Limb reperfusion and vascular outcomes of the HAV were compared with those from synthetic expanded polytetrafluoroethylene (ePTFE) grafts. METHODS: Thirty-six pigs were randomly assigned to four treatment groups, receiving either the HAV or a PTFE graft following a hind limb ischemia period of either 0 or 6 hours. All grafts were 3-cm-long inter

Multi-Branched Thoracoabdominal Endovascular Aortic Repair With Branch Access Through a Left Thoracotomy and Descending Thoracic Aortic Conduit: A Novel and Effective Alternative Access.

PURPOSE: To demonstrate an alternative access to perform directional branch catheterization during complex endovascular aortic repair. TECHNIQUE: Urgent endovascular aortic repair was indicated to treat a symptomatic post dissection thoracoabdominal aneurysm with large infrarenal dilatation with an off-the-shelf t-Branch endograft (Cook Medical, Bloomington, IN, USA). Traditional proximal arterial accesses were not suitable due to a previous aortic arch endograft. A novel approach was performed through a left postero-lateral thoracotomy, isolation of the descending thoracic aorta and anastomo

Mechanical Performance Assessment of Physician Modified Aortic Stent Graft.

OBJECTIVE: This study aimed to compare various fenestration configurations of physician modified aortic stent grafts in order to identify which design parameters have a significant influence on the mechanical behaviour of the fenestration. METHODS: the fenestration configurations were considered according to different manufacturing parameters: cutting technique, fenestration reinforcement, suture material, reinforcement loop design, and number of suture points. The performance of the graft/bridging stent assembly was assessed at various levels: (1) branch pull out force; (2) fenestration enla

Retrograde branched extension limb assembling stent of pararenal abdominal aortic aneurysm: A longitudinal hemodynamic analysis for stent graft migration.

PURPOSE: Pararenal abdominal aortic aneurysms (PRAAAs) are a life-threatening disease, and hemodynamic analysis may provide greater insight into the effectiveness and long-term outcomes of endovascular aneurysm repair (EVAR). However, the lack of patient-specific boundary conditions on the periphery compromises the accuracy. Windkessel (WK) boundary conditions coupled to hemodynamic follow-up models of a PRAAA patient, aims to provide insights into the link between hemodynamics and poor prognosis. METHOD: One PRAAA patient underwent EVAR and reintervention after one branch of stent-graft (SG)

Use of a Steerable Sheath for Antegrade Catheterization of a Supra-aortic Branch of an Inner-Branched Arch Endograft via a Percutaneous Femoral Access.

Purpose: To describe the use of a steerable sheath from a femoral access for antegrade catheterization of the left common carotid artery (LCCA) in an inner-branched arch endograft. Technique: This technique is demonstrated in a patient with residual aortic dissection after replacement of the ascending aorta for acute type A aortic dissection. He presented 4 years later with aneurysmal degeneration of the thoracoabdominal aorta and a proximal tear located in the aortic arch. A 2-stage hybrid approach was devised to treat the patient. Floating Stent-Graft as a Support to Bridge an Unfavorable Renal Artery During Postdissection TAAA Repair Using a Multibranched Thoracoabdominal Endograft.

Purpose: To report an unusual endovascular technique to manage unfavorable renal artery anatomy encountered in an urgent case of symptomatic postdissection thoracoabdominal aortic aneurysm (TAAA) treated with an off-the-shelf multibranched device. Technique: The technique is demonstrated in a 77-year-old woman who had a history of previous open abdominal aortic aneurysm repair and an emergent procedure to implant a thoracic endograft and an aortic bare Z-stent (PETTICOAT) for acute Stanford type B dissection 7 years prior. The patient presented with a symptomatic, rapidly growing, postdissecti

'Double-dunk' implantation of valveless ring-reinforced PTFE conduits for neonatal truncus arteriosus repair.

This case describes a neonatal truncus arteriosus repair using an 8 mm ring-reinforced, valved polytetrafluoroethylene conduit for right ventricular outflow tract reconstruction via the 'double-dunk' technique. The diagnosis was established prenatally in a patient with familial history of the disease. Anatomically, both pulmonary arteries arose adjacent to each other from the posterolateral common trunk; there was a mildly regurgitant quadricuspid truncal valve and an accidental finding of an intramural course of both coronary arteries.  Clinically, the neonate presented with mild cyanosis and

[Ascending aorta and aortic arch replacement without circulatory arrest].

Circulatory arrest is the main method used in surgical interventions for prosthetics of the proximal aorta. However, recently, facts have been discovered that prove the benefits of avoiding circulatory arrest during planned and emergency surgical interventions. We present a clinical case of planned prosthetics of the ascending section and the aortic arch with a Intergard Woven 28 mm multibranch prosthesis without circulatory arrest in conditions of mild hypothermia. The patient was extubated 5 hours after the operation, and was discharged home in satisfactory condition on the 7th day. The reje
